We paid over $30,000 to the clinic as part of the guarantee cycle (plus office visits + ultrasounds)...The FIRM has several clinics in the area. This is one of the reasons I chose them. They had clinics near where I worked and also where my husband worked. So he was able to do his semen analysis and things like that at the clinic near his work, and I did ultrasounds and appointments at the clinic closest to my work. The staff at all three clinics we wound up going to were friendly and helpful.

Dr. Sproul von Goeben was invaluable to us as we went through donor egg IVF. What's one piece of advice would you give a prospective patient of Kari Sproul von Goeben at Florida Institute for Reproductive Medicine?
I would highly recommend Dr. Sproul von Goeben. She (and the staff at FIRM) are very helpful and responsive. When we try for a second child in another year or two, there is no doubt that we will return to see Dr. Sproul von Goeben!

Dr. Sproul von Goeben is very warm and friendly, while still maintaining professionalism and is honest and thorough when discussing diagnosis and treatment options.
Describe the protocols Kari Sproul von Goeben used in your cycles at Florida Institute for Reproductive Medicine and their degree of success.
For the transfer cycle, I was told the clinic’s typical protocol is Estradiol pills (Estrogen support), Endometrin vaginal suppositories, and injectable progesterone in oil (Progesterone support). I started estradiol pills 4 days after stopping birth control pills and then the Endometrin and injectable progesterone in oil the day our eggs were thawed & fertilized. So just those 3 medications unless Dr. Von Goeben specifically states otherwise. 1. Last birth control pill on ____ (date) 2. Start Estradiol 1mg taking 3 pills twice a day Four days later 3. Cryo Monitoring appointment was about 12 days after I started Estradiol. 4. Thawed and fertilized eggs the next day. And started progesterone support 5. Our transfer was 5 days later! Of our 6 frozen eggs, all six made the thaw, 5 fertilized, and 3 made it to day 5 blastocysts.

Strengths were that she was great and timely with communication. Amy is the Donor Nurse Coordinator and she is very good at what she does! My only complaint was that as we were deciding which cycle plan was right for us, there were criteria we had to hit that was not told to us up front. So I would feel like we were good to move forward, and then was told we needed to do something else/make sure we hit certain criteria to qualify for one of the programs. I understand the need for this, I just wish those things had been told to us upfront.

The FIRM has several clinics in the area. This is one of the reasons I chose them. They had clinics near where I worked and also where my husband worked. So he was able to do his semen analysis and things like that at the clinic near his work, and I did ultrasounds and appointments at the clinic closest to my work. Describe Kari Sproul von Goeben's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Florida Institute for Reproductive Medicine.
For us, Dr. Sproul von Goeben recommended single embryo transfer. She explained how getting two babies for one cycle can seem like a good idea, but oftentimes pregnancy with multiples can come with extra risks for mother & babies, and that twins can be delivered early and need time in the NICU, etc. I was comfortable with the decision. I had a failed embryo transfer with another doctor years ago (fresh cycle) and transferred 2 embryos, and neither implanted. I won’t dive into issues with the other doctor/clinic here. I will just reiterate that Dr. Sproul von Goeben and the FIRM have my full trust in taking care of my health and helping us to achieve a healthy pregnancy.

Describe the protocols Kari Sproul von Goeben used in your cycles at Florida Institute for Reproductive Medicine and their degree of success.
We started in 2017 with all the fertility diagnostic testing. We then had a Clomid cycle and IUI which did not result in a pregnancy. We decided to wait a while before going back and moving forward with IVF. In 2018, we had our first egg retrieval and created 6 embryos, after PGS testing we had 2 normal embryos. We transferred both, and did not get pregnant. We immediately did another egg retrieval but added the drug Omnitrope to our protocol, this resulted in 11 embryos. After PGS testing, we had 3 healthy embryos, and 1 that needs retesting. We did a frozen embryo transfer in March of 2019 that gave us our daughter. We did another frozen embryo transfer in January of 2021 and are currently pregnant with our son.

All the nurses, no matter what location I went to were always so kind and helpful. I always felt like a friend and welcomed when I came to the clinic.

Dr. Sproul von Goeben has provided great bedside manner and answered all questions thoroughly... We only have one other good embryo so I am now doing an ERA and Receptiva test.

Weakness would be sometimes a little rushed and there can be slight miscommunications stemming from this...Nursing staff still always very pleasant 1 year into seeing them....

Update: I have now had a failed FET with Dr. von Goeben - she's still a very compassionate Doctor and I'll be moving forward trying for our baby again with her despite our failure. Dr. Sproul von Goeben has provided great bedside manner and answered all questions thoroughly. She seems to genuinely care and I hope to see her for IVF one day. While I haven't yet become pregnant I have hypothalamic amenorrhea and lean PCOS so I think that it may be a tougher diagnosis to tackle than some.
What's one piece of advice would you give a prospective patient of Kari Sproul von Goeben at Florida Institute for Reproductive Medicine?
Since I work 40 hours a week I can't be as selective with my appointment times but if there is flexibility then I would try to see Dr. Sproul von Goeben in this office. I had to see another doctor a couple of times and didn't find that he was as good a fit for me.

Review update: I have now had my first frozen embryo transfer which failed. Dr. von Goeben has met via phone with my husband and I to discuss what we can do to identify why our PGS normal (great grading) embryo didn't implant. We only have one other good embryo so I am now doing an ERA and Receptiva test. I was concerned because I had expressed interest in a kitchen sink protocol and wanting to do these test before we lost our girl embryo but she didn't think it was necessary at that point which makes sense and I trusted and of course, I didn't want to add to the cost if she thought it was overkill (we pay all OOP and don't have family help so we've spent $35k at this point and may have to do another retrieval). I have only undergone monitored ovulation induction at this point but she does go out of her way to be kind and explain everything in detail which I appreciate. I'm the type to come with a great deal of research of my own and to ask questions which she facilitated. She also is very kind and explained that she herself underwent IVF. I hope to do IVF with her one day.

Updated review: Failed FET with low dose (I have a AMH of 15 but low egg quality so OHSS is a huge concern) Gonal-F, Menopur, Lupron trigger as retrieval meds and oral estradiol and PIO / endometrin suppositories as transfer meds. If we can manage to afford another retrieval she suggested even lower doses of Gonal-F (I had 56 eggs retrieved but I only had 2 PGS normal 5-day blastocysts and 1 PGS normal 7-day blast which may have a likelihood to miscarry). I did not get OHSS from my retrieval so the Lupron only trigger definitely avoided it. My concern is that I had so many immature eggs so I think perhaps a higher dose Lupron trigger would have matured them (I have a 16 BMI hence the low dose). I also had a long length of stims which I've read can cause some egg quality deterioration. Previous review: I used Letrozole and trigger shots 6 times (one with Clomid combined) but have yet to become pregnant (I'm 29 with hypothalamic/anovulation/lean PCOS)

Updated review: Nursing staff still always very pleasant 1 year into seeing them. The nursing staff has always been pleasant here. No complaints at this point though it's a little early since I haven't yet had the opportunity to do IVF.

I imagine we'll have to do another retrieval so financially a hurdle in the near future. Previous review: I would estimate that every cycle of ovulation induction cost me about $600 all OOP not including the testing before beginning treatment

The protocol was to be on endometrim, pio shots, and estrogen pills before and after embryo transfer. It took 8 days to get a positive result and wouldn’t trade it all for anything. I stopped treatments at 12 weeks and was supported by firm and dr van goeben until the very last day. Overall great experience.

Amy my nurse was phenomenal. She went above and beyond and answer my questions and concerns. She emailed me in a timely manner as picking up the phone is hard to communicate while I am at work. We spoke on the phone when we needed to but mainly emailed which helps me remember details of our conversations. Amazing work.

In the beginning I was being quoted more than I was prompted to. But with a simple phone call, we were able to settle the price well. Since they had recently moved to a different billing place, I do have a balance with them I’m still disputing.

She has been very patient and thorough with my husband and I which has helped us a lot....I’ve been treated for unexplained infertility since late 2017. I began with the normal testing, including testing for my husband. We then went into clomid and clomid plus trigger shot plus IUIs. I ended up needing surgery to remove some cysts that were not resolving on their own. We are now moving into IVF.
Strengthens- Relatively easy to get an appointment, including weekends and same day. Having multiple locations [Florida Institute for Repro Med] is very nice. Weaknesses- The administrative staff is not the best (particularly their billing staff). With infertify, some tests are very time sensitive and once my account was put on hold, because the staff collected the wrong amount.... IUI Cycles-$500-$600 a cycle. I really wanted to do a fresh transfer, especially since we live on the opposite side of the country from the FIRM in Jacksonville, but she [Dr. Sproul von Goeben] felt like we were going to be happier long term if we went for a larger wuantity of embryos and a frozen transfer. I really wanted to do a fresh transfer, especially since we live on the opposite side of the country from the FIRM in Jacksonville, but she felt like we were going to be happier long term if we went for a larger wuantity of embryos and a frozen transfer. I’m glad I trusted her and followed through with that treatment plan, although she did offer to back off the medications and allow us to go for a fresh transfer with less embryos. I used Gonal F (225 units for 13 days) and Menopur (1 vial-75 units for 13 days), along with cetrotide (7 days) and 2 Lupron triggers. The medications were easy to inject, and I ended up with zero side effects. I had 16 mature eggs at my retrieval, and 12 day 2 embryos. I’ll find out the final embryo count in 2 days.

Dr. Von Goeben is such a blessing! We had seen two other providers prior to her and my care was subpar. One of whom even missing underlying conditions and denying testing even though I asked for it. Once I switched to Dr Von Goeben she immediately saw what I saw, Ordered the appropriate tests, explained that she felt like we needed to start at the beginning so that nothing is missed. So we did. She re-evaluated me as if I were a new patient. She looks at me as a whole and not just another person she is trying to get pregnant. She is thorough. Patient. First thing we did was reevaluate my case. From sonohistograms to a TON of blood work. She also discovered I had an underlying condition that needed to be treated by an Endocrinologist so she referred me to an amazing one. She also referred me to an amazing OBGYN. Once all of the underlying “mess” was streightened out we did a round of birth control to keep my lining thin and allow a cyst to heal. Then a round of 100mg of Clomid for 6 days, followed by a trigger shot & an IUI. Which I am currently in the tww from. Because of my past treatment, she knew I would respond to the Clomid & She feels IUI will work for us since I did get pregnant (ended in loss) with my last IUI. She feels that my underlying undiagnosed Hashimoto’s with Hypothyroidism was the cause of my previous losses... I can’t share my results yet.... because I don’t have them! (Hellos two week wait.)

At this clinic you are always seen my your Dr, even on the weekends you’ll see the on call Dr, never a nurse practitioner or nurse. They are very flexible with scheduling and work to get you in quickly, including weekend appointments.

With my first pregnancy my progesterone was dangerously low after my first beta & I was told progesterone suppositories were called into the pharmacy- so of course I went to get them as soon as I could & my pharmacy didn’t have it. I was able to call the on call Dr and get it called in and filled that night. Unfortunately that pregnancy ended in loss. Another issue was with a different provider at this clinic whom refused to order blood work to test my thyroid even though I had symptoms and specifically requested the test. Eventually he ordered the test and the results were abnormal and he didn’t call to let me know as he said he would. I learned about the abnormal result once quest posted the results on my app.... I called several times and the results were dismissed. This is what prompted me to switch to my current physician, Dr. Von Goeben and she immediately saw my results as a red flag.

First went to letrozole since I was already on that protocol, got pregnant right away, miscarried. So continued with letrozole for a while. Then switched to clomid because it tends to work better for pcos patients and gradually increased dosage, tried IUI and timed intercourse during this time about 18-24 mos. Conceived and miscarried again and had an ectopic pregnancy that ruptured had to have a tube removed. Changed course to IVF due to ectopic pregnancy and 1 tube. Have done 2 rounds of IVF stimulation with menopur, gonal f, and Ovidrel trigger shot and 2nd round with menopur, clomid, and hgh, also with an Ovidrel or novarel trigger.

I sometimes felt like it was all about money. They refused to test me for things I thought would help but did me explanations as to why. I always had my questions answered but sometimes felt rushed. I did feel like a human for the most part as my doctor did hug me after my failed cycle and could feel my pain

For my IUI's I was on Clomid which all failed. I don't remember much from those cycles. For my IVF I was taking Gonal F and Menopur. I believe I was on 300 of Gonal and 150 of Menopur. Then Cetrotide was added. Unfortunately I had to stay on the Gonal and Menopur a little longer than normal because my follicles were not exactly where they wanted them. I took the Lupron trigger shot and they retrieved 22 eggs. Of those 22, 19 were mature and fertilized with ICSI. Only 9 made it to biopsy and freeze and of this 9 PGS tested embryos, 7 were normal. Dr. Von Goeben was unable to do my actual retrieval. I then had a polypectomy done for multiple polyps. Then I was placed on birth control for about 2 weeks. After stopping that, I started taking estradiol 1 mg tabs, 3 in the morning and 3 at night (worked up to that). I had my day 13 scan and my lining was not thick enough. So she extended my estrogen by a week. After the next scan my lining was good and was told to start Progesterone supp. I took those for 6 days and on the 6th day had my embryo transfer. My transfer went very smoothly. Unfortunately it failed.

The nurses were nice but sometimes it was hard to get ahold of them. However they always got back to me. I didn't have a nurse I wasn't happy with

As far as strengths the staff are all very nice and welcoming. For the most part they worked with my schedule for appointments and made it easier to be seen. As far as weaknesses, the waits were usually very long. The clinic was always freezing and waiting forever in an ice cold waiting room was not easy.

Everything was out of pocket. My IUI's were each over $700 and that didn't include the $50 bottle of Clomid for each cycle. My IVF was $6000 for ultrasound monitoring appointments, egg retrieval, and transfer. On top of that we had to pay extra for the anesthesia and ICSI. Also the medications which were almost $5,000. Then the PGS testing which was $2,000 for the biopsy paid to the clinic and over $1,700 for the testing paid to the lab.

Every appointment she always explains , walks ya through next steps, etc. I would highly recommend her to anyone of my friends...If you are looking for a compassionate, yet knowledgeable doctor. She’s the one to go too!
One weakness [of Florida Institute for Reproductive Medicine] would be there billing returning phone calls to explain billing information. There strength would be very helpful, warm, caring, positive, and knowledgeable. They even help work around your work schedule...Around $800 each IUI cycle...My experience has been wonderful. My husband and I are under the umbrella of unexplained infertility. Before going to Dr. Von Goeben we tried clomid twice through our GYNO and had an HSG test, all we clear. Unfortunately, the clomid didn’t work after over a year and half of trying a little help with clomid we decided to see a fertility specialist. First steps was bloodwork to check health and egg reserve which is normal. Next times was IUI we have that that four times with not success. Here we are 2.5 years later still trying to start a family. It’s very frustrating when my husband and I are health.

...I liked that if I came in with something I'd read off the internet she [Dr. Kari Sproul von Goeben] was willing to have a discussion about why or why not that particular treatment would apply to me. I did not like that when I wanted to try things she would say there wasn't a "medical indication" for what I wanted to try- when Infertility is unexplained that is exactly the issue, that no one knows what might work, and I wish she were more open to anecdotal information and not entirely research reliant.
Whether or not they run late depends on what doctor you're seeing which is frustrating. She felt strongly about single embryo transfer. That is what I wanted so I didn't push it, but I felt that had we wanted a multiple transfer it would not have been received well.

She always explained everything in full detail and gave me very realistic expectations even when they weren’t the best. She went above and beyond researching information for a complication I was having and I honestly think if I had any other doctor I would not be holding my son right now... will cover everything you need

All of the [Florida Institute for Reproductive Medicine] staff seems very friendly with great personalities...The initial consultation was $100.00. All test besides semen analysis were already done by my gynecologist. The follow up was also $100.00. I have spent about $800-$900 per iui...The clinic is very welcoming and I haven’t had any issues scheduling appointments. They have multiple office which works out well when trying to schedule. All 3 Doctors had a good bedside manner. I’m hoping for a positive outcome with my situation. I haven’t had any bad or negative experiences with this office.

After going over all my test results that were previously done by my gynecologist office. I was told I wouldn’t need Ivf that she would like to start with iui. After 2 unsuccessful iuis I was told by another Dr at the practice that it looks like iui won’t work and IVF is needed.

She recommended medicated IUIs with trigger shot. She said less aggressive cycles (meds along or IUI alone) were typically ineffective for people with unexplained infertility. She said medicated IUI gave best odds other than IVF.

This clinic uses very research-driven practices. They are professional. But, overall the treatment is very impersonal. It feels like you are always seeing someone different whether it be for treatment or for monitoring.

Most of the testing costs were covered by insurance, but each visit I had a 65 dollar copay. My IUI cycles were around 1200 including medication and not covered by insurance.
