After decades of being told that my insanely heavy periods were not a big deal, years of trying to conceive unsuccessfully, and a recent misdiagnosis of PCOS, meeting Dr. Silverberg was like a breath of fresh air. I was originally referred to him because polyps and a mullerian anomaly was detected in a saline ultrasound. In our first visit, we went over my records, administered a transvaginal ultrasound, and determined:
- I didn't have PCOS
- I had a septum
- My endometrium may have been one of the thickest he'd seen in his career, I probably make too much estrogen, and it's no wonder my periods are insane
Bam, right out of the gate! His level of confidence in that visit and familiarity with my whacked out reproductive system was reassuring. He'd seen issues like these before, and he knows how to fix them. This felt wonderful after so many doctors being mystified in the past or claiming I must have been exaggerating symptoms. Silverberg never doubted or down-played anything I said.
We worked together over the course of a couple months to get my endometrium thin enough so he could perform a laparoscopy and hysteroscopy. In addition to removing the septum and polyps, he said he would laser ("zap!") any endometriosis I had, to which I replied that I'd been told several times in the past that I don't have endo. And, of course, he found and removed several instances of Stage 1 and 2 endometriosis. He also verified that my tubes were clear, just to make sure, which was nice. He performed the hysteroscopy with way more care and precision than a previous doctor, and I would absolutely trust him again with any surgery of this nature. We then went on to try a few medicated cycles and IUIs under his care.
The good:
If you have an appointment with Silverberg, you see him every time. Sometimes the appointments are extremely fast, as it is with REs, but if you feel overwhelmed by information, he will take the time to slow down. After one appointment, I felt so overwhelmed that his nurses secured another meeting room and he returned to walk me through additional information. He genuinely seemed invested in my level of comfort.
He's also incredibly knowledgeable, and seems to truly love what he does. He geeks out over this stuff in the best way possible. It's important that he tries to find the root cause of a patient's infertility, not just throw random treatments at it. He has great bedside manner and I always enjoyed interacting with him.
The OK:
I might be too cynical, but his level of confidence made me suspicious at times. He would always state that my ovaries were "beautiful" and I had a lot of eggs, and inferred that I should have a smooth time getting pregnant. I may have a lot of eggs, but my baby count is still zero, so it doesn't mean it's easy for me.
This could be positive or negative depending on the style of RE you're seeking, but Silverberg's approach in rooted in science. He's not going to tell you to take a ton of supplements and herbs or eat a special diet, but he will make recommendations based on studies, science, and stats. I liked this science-based approach, but I guess I also would like to have explored additional options that help my chances, even if only incrementally.

Depending on which RE you see at the Texas Fertility Center, they have a dedicated nursing staff, so you're usually dealing with the same nurses each time. You won't always get them on the phone when you call, but they do a good job calling you back the same business day and are generally very responsive and helpful. The weekend nurse was also extremely helpful when I had an urgent issue on a Saturday.
The only issue I encountered with the nursing staff is there seems to be miscommunication sometimes. Specific example: I was experiencing spotting (not pregnant, preparing for an IUI), so I called the nurse to see if the doctor wants me to increase a certain dosage. She called back and told me to start taking estrogen pills. I questioned this twice, because according to Silverberg, I literally make too much estrogen in my body -- it's the reason I have issues like polyps. The nurse said she asked the doctor, and he said to start the estrogen. Low and behold, my next in-person appointment with Silverberg, he said I shouldn't have been put on estrogen pills. I imagine miscommunications like this in fast-paced situations happen at a lot of fertility clinics, though, so you just have to question things if you feel they aren't right.
Describe your experience with Texas Fertility Center.
I have no complaints, the clinic itself is nice. The facility is welcoming enough and the staff are always friendly. Sometimes you have to wait a while for your appointment, but that's just the nature of visiting a fertility clinic: wait 45 minutes for a five-minute appointment.
Pro tip: I have always had to wait the longest if the appointment is between 4 and 5 on a weekday, and sometimes first-thing in the morning on the weekends takes a while because Silverberg will have new patient consultations (which is really a plus, because you know he'll take the time to discuss things when it's your turn). But if you can swing making appointments in the morning during the week, there's hardly any wait.

For anything related to my septum or polyps, it was billed to insurance and covered. My insurance does not cover fertility treatments, but there are a few good options at Texas Fertility Center like pre-paying for bloodwork that can help with some of the costs.
For IUI, it was about $800 per IUI cycle without insurance, including the prescriptions (including $200 trigger shot), appointments, ultrasound monitoring, semen testing and preparation, and IUI procedure.
They also have third-party financing options, which I did not use.

Be ready with your questions, but also don't be afraid to any any question. I asked him what I thought were probably the silliest and most ridiculous, uneducated questions I could think of and he never made me feel like I was stupid or ridiculous. Also, know that his sense of humor is very dry and matter of fact and that might feel to some people (especially those who are just starting their infertility journey) like he doesn't care. I can assure you that he cares about all of his patients and their success and it is evident in every visit you have with him or the staff. Again, I cannot say enough good things about him - I have so much respect for him as a doctor.

My husband and I went through a lot of lab work and testing before we were presented with our specific treatment options (it took about 3 months for everything to be done and results in). Once we got our results back, we were presented with a range of options from IUI to IVF with ICSI. He gave us the statistics of success with each option and my diagnosis. We decided to go with IVF with ICSI because it was the most aggressive treatment that would ensure fertilization of any eggs retrieved and with a DOR diagnosis, time is most assuredly, not, on my side. I immediately started the process to start the IVF injections, but had to wait for a month because of my hormone levels. I did the injections and had a successful egg retrieval and the number of eggs was almost exactly what was predicted based on my lab work.

The nurses are excellent. They are there to talk you through any questions you have. They always call you back in a timely manner and do everything in their power to make sure you have what you need. IVF was very confusing for me at first with everything you have to remember and not wanting to make a misstep, but my stress was alleviated by the nurses always being available and ready to help me in whatever way I needed.

I love the clinic. The entire staff is amazing. For example, I thought my insurance wouldn't cover any part of my treatment and the staff said I really should run it anyway and it ended up covering my diagnosis visits, many of my labs, and most of my medications. This saved me literally thousands of dollars and I wouldn't have known if the staff had not made it a priority to answer my questions and look at my coverage. I always enjoy the check out process - it's easy and everyone is helpful. The waiting room was rarely crowded and I never had to wait long for my appointments and often got seen early - this isn't always the case, but it was for me which I appreciated because I had to drive 2 hours to each appointment. The only problem I ever had was parking because of construction and limited spaces so you might want to make sure you get there early or have someone to drop you off while they find a space.

Great working partners with Ovation Fertility and Austin Anesthesiology - all went smoothly and there was great communication there as well. Nurses are quick to respond and are actually in the office later than phone hours, often answering email so you may get a response after hours. The on-call nurse (Pam) is VERY quick to respond. Check-out is easy and they always have the info needed and lab slips for the next visit all ready when you come out of the exam room.

- Dr. Silverberg is a busy man and frequently lectures all over the world. We wasn't available at all screening appointments or retrievals, but kept up with my case remotely. No protocol adjdustments were made unless he signed off on it, so his schedule didn't impact us.

TFC sees about 80% of the IVF cases in Austin, so they're a high volume center. That being said, Dr. Silverberg always kept up with my case, got to know us personally, and made us feel like we were human, not a number.

I was diagnosed with low ovarian reserve, so Dr. Silverberg recommended the micro-dose lupron flare protocol. Several days of lupron leading up to the cycle. In cycle, lupron, follistim (350), and omnitrope (HGH) in the morning. Follistim (150), menopur (150), and lupron in the evening. I did 3 cycles with Dr. Silverberg and he adjusted my protocol each time.

Dr. Silverberg takes the time to get to know his patients. In our initial consult, he asked questions, like what were our occupations, etc. Every visit I had with him, he asked what was going on in our lives, how our work was, things like that. He always presents multiple options for treatment and lets you pick what works for you. He is upfront in setting expectations, even if they aren't great. However, when you decide on a path, he is your biggest cheerleader, and with do everything he can to make you successful.

We used high dose protocols due to being diagnosed with DOR. We started with a microdose lupron flare, and only had two follicles. We converted to an IUI. Next cycle was Clomid/Cetrotide with HGH. We retrieved two eggs, one mature, one not. The most recent cycle was MLF again but adding HGH. He retrieved three eggs.

There were a few appointments where the doctors would run late, but this goes with the territory of fertility specialists. I never minded waiting, because when I saw the doctors, I was always given plenty of time to discuss my questions and concerns.

We were charged for ultrasound visits ($220 each). The surgery costs were ~$1300 to the Dr and $1300 to the surgery center. The anesthesia was $450. Lab fees were about $5k (without PGS). Drugs were about $5k through a pharmacy he partners with, which provides a slight discount to his patients.
He offers a 25% military discount, along with the lab offering a 20% military discount, which helps tremendously.

They forgot to order a simple blood test. Luckily, when I went to the lab, they had the back-office number on file and were able to get it handled within about 15 mins. On my way home, the nurse called and apologized for the error, and let me know everything was fixed.
In their defense, my cycle started earlier than we all planned so it was rushed, along with the complications due to COVID (reduced staff, split shifts, etc).
He [Dr. Silverberg] speaks quickly and is very up to date and knowledgeable on all fertility topics but takes time to review all details with you personally. He cared when our first round wasn’t successful enough to give us his personal cell for any questions or concerns along the way. He hugged me and commiserated the initial loss (lack of success) upon learning of unsuccessful transfers.... Their knowledge base is invaluable! The doctors [Texas Fertility Center] all seem to be very professional and ensure that your care is provided at the highest level. The nurses are responsive and compassionate. They are organized as well. The only weakness I would begin to discuss would be the wait time at regular office visit is but I understand that surgeries often go longer than expected and I would rather have the great care they provide.

Our first stimulation and egg retrieval resulted in a lower number of eggs retrieved than originally predicted. Dr Silverberg adjusted my next cycle appropriately with more estrogen injections to yield more eggs for fertilization. He also carefully monitored me to ensure that I didn’t become hyper stimulated. The second round of stimulation was also accompanied by a freeze all cycle rather than fresh which was easier on my emotions and body to have the break between the retrieval and the transfer. The research showed that freeze all is typically more successful which was true for our situation as well.

Their knowledge base is invaluable! The doctors all seem to be very professional and ensure that your care is provided at the highest level. The nurses are responsive and compassionate. They are organized as well. The only weakness I would begin to discuss would be the wait time at regular office visit is but I understand that surgeries often go longer than expected and I would rather have the great care they provide.

All in all (for two rounds of stimulation, 3 egg retrievals) we spent $35K but that included our medications, the embryology (through Ovation), the surgery center, and TFCs services.
The costs associated with TFC directly were very reasonable (for the ultrasounds, office visits, retrevial and transfer).

Dr Silverberg strongly stuck to transferring only one embryo at a time. He said he was part of the initiative that wrote guidelines for age and transfer and that he had been responsible for too many NICU babies and exorbitant hospital bills. He felt it was a more responsible move to transfer one embryo at a time based on age factors. He said once I was >35 he would be willing to transfer 2 at a time.
He [Dr. Kaylen Silverberg] is incredibly knowledgeable and has a ton of experience with IVF. I always felt like I was receiving the best care. When I was unsure of what to try next, he offered examples of previous patients in similar situations. He always presented multiple options and clearly pointed out the advantages and disadvantages of each. We paid a total of about $70,000 out of pocket (no insurance coverage) for 2 own-egg IVF cycles (~$17k and $15k), one donor egg cycle (~$33k), and one transfer cycle (~$3k). We elected to freeze all and PGS test our embryos for each cycle. We were presented with an estimate of expenses before treatment started so there were no surprise charges.

He was willing to spend as much time as I needed explaining procedures, answering my questions, and presenting me with options. He was never rude and always made an effort to make sure I left the clinic happy.
Describe the protocols Kaylen Silverberg used in your cycles at Texas Fertility Center and their degree of success.
My first IVF cycle used a long lupron (agonist) protocol since I had never tried injectable drugs before. I responded slowly and my medication was increased to compensate. A slightly lower than average number of eggs were retrieved and only one embryo made it to the blastocyst stage. It was tested PGS abnormal and incompatible with life.
My second IVF cycle used a microdose lupron flare protocol since I am a poor/slow responder to fertility drugs and there was a question about my egg quality. I was on the maximum dose for most of the cycle and ended up with a normal number of eggs. We did not have any embryos make it to the blastocyst stage.
We did not get any viable embryos using my own eggs (poor quality) so we moved on to an egg donor after 2 failed IVFs. Our donor used an antagonist protocol because that is the fastest and easiest cycle for my doctor to manage. She had donated before so my doctor used her previous cycle as a starting point. Her dosage was updated every two or three days based on her response, and we ended up with a higher than average number of eggs and PGS normal embryos.
Describe your experience with your nurse at Texas Fertility Center. (Assigned nurse: Kat (non IVF cycles), Shanna (IVF cycles), and Jennifer (donor egg cycles))
My first nurse (Kat) was assigned to me for clomid cycles. She was kind and answered all of my questions in a timely manner.
My second nurse (Shanna) was assigned to me for own egg IVF cycles. She was amazing and went above and beyond to answer all of my questions. She even asked the embryology team directly to help me understand why my first IVF didn't work.
My third nurse (Jennifer) was assigned to me for a donor egg IVF cycle. She was awesome and answered all of my questions. She also sent me information ahead of time to make sure I had everything I needed and didn't have to make any last minute decisions. She kept me updated with my donor's cycle and I never had to worry about what was going on.
Describe your experience with Texas Fertility Center.
TFC is one of the larger clinics in the state. They don't turn away patients based on diagnosis and will do everything they can to help you conceive. According to SART, their success rates are above the national average. They strongly recommend eSET when possible and I never felt pressured to try a treatment before I was ready. I saw my doctor for almost all of my appointments, even the quick, simple sonograms. My doctor performed all of my sonograms and a nurse was in the room taking notes. If my doctor was unavailable I was given the option of rescheduling or choosing another doctor at the clinic for that day.
The only downside is that TFC sees a lot of patients and sometimes runs behind in the afternoon. The longest I have had to wait for my appointment was 45 minutes when someone else's consultation ran long.

2 fresh freeze all retrieval cycles on Gonal F, Menopur and Lupron (starting with birth control and Lupron suppression). Resulted in 9 mature eggs; 2 day 6 blasts, but only 1 PGS normal embryo for 1st cycle; 8 mature eggs; 5 day 6 blasts and 2 PGS normal embryos for the 2nd cycle.
3 FETs on Crinone and Estradiol support. 1st FET = HCG of 41, chemical pregnancy. 2nd FET = BFN. 3rd FET = HCG of 28, chemical pregnancy.

Dr. Silverberg always greets me, asks me how I'm doing and I honestly feel that he's has my best interest in mind and that he's cheering for me.

I've had 5 IUIs that we got progressively more aggressive with. My first IUI was unmedicated. The second and third were in conjunction with Femara and my fourth and fifth cycles were with gonal-f injections and the ovidrel trigger. I also used progesterone suppositories after the IUIs on the last two cycles. I'm still trying to conceive so none of the IUIs produced a positive pregnancy test. So I have decided to move on to IVF treatment.

All of the staff at Texas Fertility Center are amazing. I've worked with a lot of the ladies in the office and have never met a negative person there. They are all very helpful and compassionate. Its a joy having that team during such a difficult time.

So many people working there but helping everyone out, being understanding of any type of situation, cheering every patient on, and overall very kind hearted people... Scheduling was easy. Although Dr. Silverberg is very quick, he is ready to answer questions at every appointment. Even two years after our first round of IVF, he remembered us.

I don't remember the type of protocols used but for both full IVF cycles, I was on birth control for about 3 weeks, lupron for a couple of weeks, lupron and gonal-f for 10 days the first round and 12 days the second round. I triggered with ovidrel both times and was on estrogen pills for a little over a week and crinone until my progesterone numbers were high enough.
We went to IVF, skipping IUI, because my husband has a history of trsticular cancer which greatly effected his numbers. I feel like that was the right call so we weren't spending so much money on countless IUIs that probably wouldn't have worked.
Our first IVF resulted in 6 embryos, only one in the blast stage. We transferred two, the rest arrested, and we had one healthy baby. The second round resulted in 10 embryos, 4 reaching the blast stage. We opted to freeze all in order to allow my body to adjust from being on the meds. We did a frozen transfer about 2 weeks later of one embryo which resulted in one healthy baby.

We had a different nurse for each round, but both were excellent. We were able to call with any questions at any time and both were prompt in returning those calls. I really believe they wanted treatments to be successful for us just as much as we did.

We always felt comfortable and like we were given every piece of information we needed.

There were times where I went to get my blood test taken and they forgot to call in the order. Also, Silverberg is ALWAYS late and there is always a long wait. But the way I look at it it's because he is spending his time with other patients and I appreciate that time too.
Dr. Silverberg is incredibly knowledgeable about the most up-to-date research related to infertility. He thoroughly answers any questions we have. We've be pleased with the treatment received thus far. At times we have a very long wait which it seems could be due to over-scheduling.... Dr. Silverberg does individualized treatment plans which is very appreciated. At times the long wait times made us feel more like a "number".
TFC seems to be a well-oiled machine. The staff are all knowledgeable and willing to work with you to be sure you understand your treatment, medications, process, etc. They work with you on scheduling. As noted, at times there are long wait times which can be frustrating.... Ashley has been wonderful to work with so far. She is thorough and answers all of my questions (more than once if needed). She makes sure things are in place for the weekend if you have to call the on-call nurse.

Ashley has been wonderful to work with so far. She is thorough and answers all of my questions (more than once if needed). She makes sure things are in place for the weekend if you have to call the on-call nurse.

TFC seems to be a well-oiled machine. The staff are all knowledgeable and willing to work with you to be sure you understand your treatment, medications, process, etc. They work with you on scheduling. As noted, at times there are long wait times which can be frustrating.

Dr. Silverberg seems pretty adamant about only transferring one embryo at a time, which we are comfortable with.
Candid and no fluff, he [Dr. Silverberg] shoots straight and took the time to talk me through each procedure. ... My first IUI I produced way too many eggs and I made the decision (with his advice) to cancel the cycle. ... We did multiple transfer and that seemed normal for my age. ... One strength of the clinic was the opportunity to be part of a trial --this provided for the almost the total cost of our IVF. The only downside to a trial is the amount of times you had to get your blood taken (every day) and I lived in Waco so I was driving to Austin every morning before I went to school. Overall I would ABSOLUTELY do it all again with Dr. KS again. Before we did that they did everything to try and have children in ways that were cost effective-two rounds of IUI and then this trial. I did have surgery to correct a blockage.

The only major issue was that Dr. S was chronically late - sometimes 2 hours or more.
