What's one piece of advice would you give a prospective patient of Lisa Farah-Eways at Kaiser Permanente NorCal?
Just trust her, bring her your questions and concerns. Also, if anything seems to be a problem, call until you get through to her or the staff. I was instructed by an answering service to wait until late in the day to call back, but it was the wrong advice, as that was the time the IVF office closed and I could no longer be helped until the next day.

Please don’t mistake this for her not caring or not being focused on you and your treatment. At the end of your consultation/appointment you will have a summary in the form of emails with all things discussed, recommendations, plan of care and a detailed summary of your visit. Her typing/inputting things on the computer as she is discussing processes with you, I believe, is her way to ensure that no information during your conversation is missed in the hard copy of your plan of care and that all test you need to have done are ordered as soon as she decides you need them. Because she documents things while she’s talking, you end up having a complete reference in terms of next steps. This is important because there can be so much information, many dates, and timed studies to complete. I’m the type of person that comprehends and processes information quickly, so I prefer information to be delivered straight forward in the form of what the plan for me is, what my next steps are, and when they steps need to be completed. Having a hard copy plan of care with dates/times to complete and contact numbers has helped me feel more control in a situation that can be so out of your control. If you like information delivered this way I think you will be happy to work with Dr. Farah-Eways, also note that if ever there is a time that you feel you didn’t understand or you need to verify what the plan is, she responds best by email to address your concerns.
What's one piece of advice would you give a prospective patient of Lisa Farah-Eways at Kaiser Permanente NorCal?
Understand that she works within the Kaiser Permanente health system, so at times because the Kaiser process calls for systematic approach since they see a greater volume of patients than private REI clinics, you won’t always see her at appointments for procedures such HSG test, IUI or ultrasounds to verify meds such as clomid, femara, etc produced sufficient follicles for IUI. While you have a care plan by Dr. Farah-Eways, these procedures will be performed by her Nurse Practitioners (NP) and not necessarily the same NP each time. It comes down to who is available at the time you need the appointment. As for diagnostic procedures such as Hysteroscopy, endometrial biopsy, trial transfers, and saline sonograms you will see her or one of her colleagues that is an REI doctor. If you want her specifically for those procedures , this goes for IVF retrieval and transfers as well, you would have to wait for an opening on her schedule. If you don’t want to wait there are other REI doctors that can perform the procedures at the earliest date and report the findings to Dr. Farah-Eways. The process at Kaiser works where a REI doctor like Dr. Farah-Eways may be doing diagnostic test 1 day, clinic appointments the next day and IVF retrieval and transfer the all day the entire next week. So if you’re looking to see the same REI doctor/nurse at each appointment and it wouldn’t be okay with you if you didn’t, she may not be the REI doctor for you.
During treatment, were you treated like a number or a human with Lisa Farah-Eways at Kaiser Permanente NorCal?
Although Kaiser’s REI department is extremely busy, Dr. Farah-Eways has always been extremely kind and explained the plan of care and next steps for my fertility treatment in great detail regardless of the amount of time it took. Also, I was scheduled to have a saline sonogram with trial transfer, and endometrial biopsy 2 days after shelter in place orders were mandated due to COVID-19. This order encouraged healthcare facilities to cancel procedures that are considered to be “elective”. Although my scheduled procedures were considered to be elective, Dr Farah-Eways had her staff immediately call me and reassure me that she was still willing to do my procedure if I still wanted to keep my appointment and that she understood how important it was for me to complete these procedures as they are scheduled only on specific days within a given cycle. The fact that she and her staff allowed me to keep my appointment made me feel like they respected both my time and the emotional toll it takes to have to wait a whole cycle before a procedure can be scheduled again. This made me feel like human and not just a number to them.

Lab work was completed to rule out any clotting disorders or genetic abnormalities prior to IUI
Pre-IUI supplements:
-Prenatal
-Co-Enzyme Q10 600mg/day
-Methylfolate 1mg (folic acid in prenatals may not be absorbed in its form)
2000U of vitamin D (prenatals amount not enough)
-Probiotics ( to help with gut health)
For each IUI the medication protocol was used to make me have a stronger ovulation and produce more than one egg in attempt to increase the chances of successful IUI. Dr. Farah-Eways confirmed with tests and from multiple previous pregnancies that unfortunately resulted in losses, I ovulate regularly on my own. The progesterone was used to proactively to help aid in preservation of pregnancy if the IUI was successful. IUI was chosen as a first line of treatment because we had not been able to conceive naturally for close to 1 year after our 3rd miscarriage:

We had a natural conception 1 month after the 4th IUI attempt. That natural conception resulted in our baby not growing after week 6. I had a D&C in February. Followed by a saline sonogram, endometrial biopsy and trial transfer done in March by Dr Farah-Eways to clear me for the IVF process. The IVF protocol will include a Retrieval with Freeze All and Frozen Embryo transfer so that the embryos can have PGT testing to rule out genetic abnormalities. Expect honesty and being a pen and notepad. Getting all this information is tough and when you walk out sometimes you forget what was said. I print the messages sent to me because I like to keep all the information in the folders they provide and bring everything to all visits so you can be prepared ask questions and follow up on things you don’t understand.

I was on an antagonist protocol and will continue to be on one when I can restart. I also was told to go gluten, dairy, soy and sugar free. This has been tough but I am working hard to do so (although in quarantine it has been hard). This diet she said helps reduce inflammation and she stated even though there isn’t evidence that will totally increase my chances, it can’t hurt. (I am pretty heavy so it’ll also help me lose weight which is good).

Dr. Eways sufficiently explained to me the trade-offs and risks of transferring multiple embryos. Her approach is very much in-line with the medical guidelines and recommendations. She does a thorough job of explaining her reasoning behind her approach to eSET. Generally speaking, she recommended only transferring one embryo per cycle because we used donor eggs and my embryos were pgs-tested, but there were times where she was open to transferring two.
You might find her [Dr. Farah-Eways] personality at times rigid because she is a perfectionist but her main goal is to get you pregnant and have a healthy baby, so you just have to trust her. ... To get my lining thick enough she recommended acupuncture and she was also OK with me taking the tincture they prescribed. She also adjusted my meds taking me off the estrogen patch and instead having me take it as oral pill and vaginally.
The only thing I can think of is that it takes a long time to get in [to Kaiser Permanente Sunnyvale]. ... From my understanding Kaiser does single embryo transfer and there are some criteria but if it’s not successful then it’s like a buy 1 get 2 free...like 3 tries for price if one. ... She was incredible and while it was a very long process, we trusted her and she told us more than once she “plays to win”...she is quite the perfectionist, but we appreciated her for that. I was supposed to have a fresh transfer March 2017 but my lining didn’t get thick enough, so she cancelled and had me wait to do a frozen transfer until August. All summer she was trying different medication combos to get my lining thick enough...the August date even got changed a few times and I don’t think my lining got to highest she would’ve liked it to, but she felt good about it and gave us the green light. I highly recommend her.
I also remember when we had our initial consultation, that evening she called me just to check in on me. She is extemely responsive as well.

To get my lining thick enough she recommended acupuncture and she was also OK with me taking the tincture they prescribed. She also adjusted my meds taking me off the estrogen patch and instead having me take it as oral pill and vaginally. Towards the end we ended up adding baby aspirin and eventually vaginal viagra. She also had me on terbutaline to prevent uterine contraction and gave me stronger dose of Valium the morning of transfer. I have a heathy 11 month old :)

Typical protocol re injections. 1st round put in a fresh 6-day embryo, resulted in miscarriage. 2nd round didn’t work. 3rd round did a polyp removal and had planned to do an endometrial scratch but didn’t do it because she said was unnecessary. Also agreed to put in 2 embryos instead of 1 since this protocol had worked for another patient. Resulted in a singleton whom I carried full term and is now 13.5 mo old.

She had me on CoQ10 and DHEA for 2-3 months before starting our medicated cycle, and also had me start acupuncture to improve blood flow to my ovaries during that time (continued through retrieval and transfer). I was on Lupron for 12 days, and Follistim and Menopur for 10 days before retrieval. The Menopur dosage was increased about 4 to 5 days in while the other two stayed constant the whole cycle. I had 3 follicles during the cycle, only 2 produced eggs and both were mature. Both fertilized but only one made it to Day 5 transfer.

When I went to the lab for my third Beta it was not in the computer, had to call the clinic and have them order it. Was taken care of while I was still standing at the counter.
Dr. Farah Eways is a mixed bag...She talks very quickly and multi-tasks in the appointment (for example, she is explaining something to you, but is also typing and trying to set up some blood tests. I didn't like her "busy" personality, she is not calm, and that made me feel nervous and uncertain... I felt like I was rushed in the appointment, even though we were in there for a while, she talks a mile a minute. I wish she was more calm, a better listener, and explained all the different levels of infertility treatment...
Nurse practioners [at Kaiser] are exceptional. They are warm, compassionate, and smart. They answer e-mails very promptly. The nursing staff was the best part of Kaiser...Strengths: Derien and all the front desk staff are very, very nice and kind and organized. What's one piece of advice would you give a prospective patient of Lisa Farah-Eways at Kaiser Permanente NorCal?
Change to Dr. Harris! He is the best one there. But I would never, ever do IVF at Kaiser. I would only stick to IUIs there. Their IVF is outdated compared to what is out there and their Fremont IVF center is a total mess (two inside sources told me this).

Dr. Farah Eways is a mixed bag. She seems fake with her empathy and tries to "connect" with you by hugging you when she meets you (awkward!). She talks very quickly and multi-tasks in the appointment (for example, she is explaining something to you, but is also typing and trying to set up some blood tests. I didn't like her "busy" personality, she is not calm, and that made me feel nervous and uncertain. Even my husband, who is pretty chill, told me that she was "strange". I felt like I was rushed in the appointment, even though we were in there for a while, she talks a mile a minute.
I wish she was more calm, a better listener, and explained all the different levels of infertility treatment. I wasn't until I changed REs and went with Dr. Harris that he actually explained the treatment process and I understood it better.
Kaiser can be fine for baseline treatments, like HSG, blood work, even Clomid/Menopur and IUI, BUT if you are thinking about IVF, I would go elsewhere.
We did 8-9 cycles of IUI with various drugs at Kaiser (with the diagnosis of Unexplained Infertility) and it wasn't until we got a second opinion elsewhere, that we actually got a male factor dianosis and mild female diagnosis. We were very surprised/upset/enraged that two Kaiser REs did not find anything wrong with us, but another doctor did. We were just spinning our wheels for a year!
We did an IVF consult with Dr. Harris and he's super nice and reassuring, but then we got a second opinion consult with Dr. Zouves in Foster City and he was the one who actually found "something" wrong with us, just with a simple telephone consult and reviewing our infertility files from Kaiser.
The Kaiser docs and Dr. Zouves have a dramatically different plan for IVF. Kaiser is very, very basic with their IVF. Dr. Zouves is incredibly detailed and believes in getting the body ready with supplements, acupuncture, diet, and exercise. Kaiser docs told us that "there is no evidence" to support diet, exercise, and acupunture. That is BS! There is so much out there on lifestyle.
Bottom line: Kaiser is great for BASIC stuff. I would not do anything BIG with them (infertility or otherwise). They tend to think IN the box and do not have the latest and greatest resources and opinions (they say they do).
If you need IVF I would call Dr. Zouves. Just look at his success rates. We're starting with him for IVF and I just feel better knowing we are out of Kaiser. I also plan to change my insurance after this experience with go to Mills Peninsula for my ob/gyn.

First, I did a HSG to see if my tubes are open. Everything was normal.
Second, she couldn't find anything "wrong" with us (someone else did 1.5 years later) so she started me with Femara (even though I ovulate beautifully on my own each month) and that only created at the most, 2 eggs. Not a big deal, but I did it 5 times unsuccessfully. The third round I did get pregnant, but it ended up being a blighted ovum (empty sac, no fetus). It wasn't until I suggested we switch to Clomid and lo and be hold, I was able to produce 5 eggs! But still no pregnancy.
I don't remember the rationale for Femara, she said she just likes it better than Clomid.

My plan with Kaiser for Infertility was 50 percent coverage. It wasn't too bad. It seemed pricy at first, but you get used to it and in the long run, it's pretty reasonable (if you actually get pregnant). We were not successful with Kaiser after 8-9 IUIs and it seemed as if we wasted a lot of money and were spinning our wheels month after month.

Dr. Eways never thought my husband's low sperm morphology was a big deal (it was 3%!!). In fact, we were diaglosed with unexplained infertility until we got a second opinion from Dr Zouves about a year and a half later, and he pointed out that out to us. Basically, we didn't know it was a big deal until Dr. Zouves actually pointed it out to us!
Nobody caught that about my husband at Kaiser, maybe because all of his numbers are good except morphology (kind of important!!). I think they have very basic parameters and are not as cutting edge.
She [Dr. Farah-Eways] is very straight forward and gets the job done. She is very through andnis great at her job. We were very confident with her decisions and my treatment. If she felt something wasn’t right, she would adjust my protocol.... She is from the East Coast and she can talk a bit fast but we are used to that dialect being from the East Coast. People have mentioned on this page she multi-task and isn’t paying attention. I am going to say false. She is dictating notes and things that she needs you to do or she’ll email another doctor that’s treating you for something separate. Dr Eways was very compassionate and explained things to me very thoroughly. She answered all mine and my husbands questions and was always honest on what the chances of us conceiving were... Dr Eways is very friendly, knowledgeable, sincere, accommodating and more. she's always keep informed on the questions i would ask. although i do wish she would have not rushed my treatment or appointments.... the nursing staff were great and kind.

The nurses were always compassionate and would answer questions without hesitation and if they didnt know the answers were great on getting back to you or finding out....
