Secondly, he is a very kind and attentive doctor. I had had a horrible experience in another clinic before, and the contrast was striking. Dr Irani first gave us the option between pursuing other IUI cycles after further testing that hadn't been done in the previous clinic or switching to IVF. He explained the pros and cons to us in detail. We chose IVF, and before starting, he did an ultrasound to check for polyps, diagnosed me with one, and performed the surgery to remove it. During the IVF cycle, since I have PCOS, I was extremely closely monitored, and Dr Irani made sure to see me regularly. I had 26 mature eggs retrieved and no problem during the cycle. This led to a successful embryo transfer a few weeks later after genetic testing.

Let me share backstory that I previously had two retrievals at my former clinic (NOT Dr Irani) where I had ~30 eggs retrieved each time from high doses of Gonal F, but only got one embryo from the first retrieval then 5 embryos the second time. These embryos failed.
In contrast, with Dr Irani, egg retrieval stimulation was low dose, lasted longer, and we retrieved nearly 40 eggs and 13 became good quality embryos!!!
First transfer was a fresh 5 day transfer, fully medicated (PIO, estrogen patches). It failed but we had untested embryos from the 13 we got with our retrieval so maybe abnormal.
Second was fully medicated frozen transfer, it failed, maybe an abnormal embryo.
Currently 8 weeks pregnant after a modified natural transfer with immune protocol.

The overall strategy was a combination of gonal F, and Menopur, in the beginning and adding Cetrotide closer to the retrieval, and then a leupron trigger shot. This seems to be the standard IVF strategy prior to egg retrieval, and produced excellent results for us. The key ingredient was the continuous monitoring via frequent (almost daily closer to the trigger) blood tests and ultrasounds, and adjusting the dosage based on the test results.

The nursing staff was excellent, explaining the medications, how to inject/ingest them, and going over things multiple times if needed. They were always prompt with answering our questions, and communicating the doctor's instructions. They were very kind and caring, and were compassionate when things didn't work out and were very happy for us when they did.

Dr. Irani was very warm and friendly, and didn't seem judgmental of my decision at all. I felt very comfortable with him from my first meeting with him and reassured in my decision to go through with egg freezing.
Describe the protocols Mohamad Irani used in your cycles at Weill Cornell Medical College and their degree of success.
I started with three days of cetrotide because I had a dominant follicle on my left ovary. I ended up using cetrotide for almost every single day of my 14 day treatment (save for a couple). He added menopur after the first three days, and then Gonal F after a couple more days. The Gonal F started at 225 units, and decreased toward the end of the cycle. I used Lupron as my trigger, and never used HCG. I didn't learn all the details of why Gonal was introduced after Menopur, or why the dosage was reduced, but I also didn't ask. I just assumed Dr. Irani knew how best to safely and effectively stimulate my ovaries.

Margaret called me the most, so I think she was assigned to me, or at least assigned to all of Dr. Irani's patients. She was incredibly clear when she called with injection instructions, and took all the time I needed to make sure I understood exactly what I needed to do.

They run a very efficient operation here, which I really appreciated because visits are so frequent during the treatment cycle. I want to spend as little time there as possible when going in for routine blood work and ultrasound.
The nurses at the Brooklyn location are incredible. They were so friendly and very good at drawing blood from my hard-to-get veins.
The nurses at the Upper East Side location were hit or miss. Some of them had a harder time drawing my blood and could be a bit brusk. One gave me an enormous bruise when she failed to hit the vein properly, and another hit a nerve that caused a lot of pain. This wouldn't change my decision to go through with egg freezing at Weill Cornell, but I wish the nurses on the Upper East Side were as kind, skilled, and patient as those in Brooklyn.

For the egg retrieval I was started on estrogen pills to prime the follicles and put me on Gonal-F since my insurance plan didn't cover Follistim. He put me on 500 units at the start and then increased it to 600 towards the end because my follicles were small. I was also on a low does HCG 5 units at the start and then 8 units towards the end. Closer to egg retrieval they had me start Ganirelix 250 mcg. Since my follicles were slow growing I went in almost everyday for ultrasounds and bloodwork towards the end of my retrieval cycle. I felt they did a great job pushing me to go a little longer to grow them before instructing me to do the trigger shot. I was miserable with the hormones given that I was on such a high dose but I am glad we stuck it out longer. Going into the procedure they were expecting to obtain between 4-7 eggs; however, they were actually able to obtain 11 after the trigger shot. We were very pleased.

We had an incident with a lost appointment that happened to be a procedure which required anesthesia. I communicated with a nurse through the portal to schedule the procedure (it was a polyp removal) and she gave me a date to be in Gurnee (Traveling from 2.5 hours away) and said I would recieve a call the day before with the time to arrive. I followed the instructions but never received the call. I tried to call the office but it was 4:10 pm and they close at 4 pm the day before my procedure. I sent a message through the portal and didn't hear anything. My husband and I still proceeded to the clinic and arrived in town when they opened and called promptly to find out what was going on. They didn't have me on the schedule and after showing them the messages they were able to fit me into the schedule that day and fix the issue. Dr. Irani addressed my concerns and we were happy with the solution provided.

Dr. Irani had a virtual initial consult to accommodate us being out of town. He didn't rush us one bit and answered all our questions. We were initially very optimistic after the consultation since he gave us fairly good odds. He didn't offer us the shared risk program since we had previous IVF failure but encouraged us to continue using our own eggs. I then saw him again for the ERA test, the egg retrieval, and then a phone consult after the genetic testing shown no viable embryo. He was sympathetic and explained again our new odds based on the last retrieval. We went ahead with the second IVF cycle with him currently since we purchased the 2 IVF cycle package in the beginning. He did offer a small refund ($1500-$3000 depending on whether we want to give up or use donor eggs). I feel like his use of medication had achieved the best retrieval numbers compared to our last clinic. After genetic testing, we had 3 chromosomally normal embryos, we transferred one embryo and resulted in identical twins! We're very glad that we gave him a try.

My stimulation lasted 14 days with retrieval on day 16, with lupron and low dose HCG trigger prior.

Dr. Irani strongly encouraged transferring one embryo due to maternal age (< 35) and no previous failed transfers. We fully agreed with this approach, and would not have been comfortable going against the current ASRM guidance to embryo transfers.

There was one appointment that was given in written instructions but not scheduled in the portal; a quick message to the staff to confirm that I should still come in easily fixed the issue.
Dr. Irani is a caring and optimistic provider who treats you with compassion and kindness...he manages a high volume of patients, but even so he's made himself very available to email and phone check ins...he never makes you feel rushed or that he has a million other things going on. My only issue has been a lack of information getting into my medical chart from conversations we did have, so I had to remind the staff of these conversations which resulted in additional stress + wasted time getting and starting medications.
I do believe this is a well run clinic [Advanced Fertility Center of Chicago] who hires great staff. Because of the popularity of Dr. Irani, there have been long wait times on occasion for monitoring appointments, but I know they were trying as best as they could to remedy the situation, and they were always apologetic. You have to be on top of your medical care and your best advocate.

Out of the three consults I had, I chose Dr. Irani because he seemed very knowledgeable and up to date about the best fertility practices for my case. He also provided me with hope and examples of how he has helped other women with DOR. When I had a question about the treatment approach, he called me promptly to discuss (vs. getting no answer at all, or dealing with nursing staff only). And when you are talking with him, he never makes you feel rushed or that he has a million other things going on. My only issue has been a lack of information getting into my medical chart from conversations we did have, so I had to remind the staff of these conversations which resulted in additional stress + wasted time getting and starting medications.

I would say to definitely be your best advocate and don't be afraid to push and ask questions when needed. You deserve the best.

Dr. Irani is a caring and optimistic provider who treats you with compassion and kindness. He has gained popularity due to his expertise, and therefore the clinic can get very busy for monitoring appointments. I imagine he manages a high volume of patients, but even so he's made himself very available to email and phone check ins. On two occasions, however, I had to remind the staff who had to communicate with Dr. Irani of a conversation we'd had to add in a medication and to switch to oral medications from injectables. Having to remind the staff and doctor of this did make me feel like a name on a spreadsheet and created more back and forth. However, overall I do believe the staff at AFCC has the best intentions.

To date, I have completed three unsuccessful IUIs. My goal was to do egg retrievals but I have not had enough follicles (3+) for an egg retrieval due to my DOR. The first IUI, I took IVF injectables which was converted to an IUI when I did not produce enough follicles. I primed with Estrace, started with Omnitroupe, Letrozole, and 300 units of GonalF daily. The second cycle, I just took Letrozole, and the third I took Clomid + 450 units of GonalF daily.

In my limited experience with fertility clinics, I do believe this is a well run clinic who hires great staff. Because of the popularity of Dr. Irani, there have been long wait times on occasion for monitoring appointments, but I know they were trying as best as they could to remedy the situation, and they were always apologetic. You have to be on top of your medical care and your best advocate. Fertility treatments are a steep learning curve but the staff and Dr. Irani are there to help you learn and understand, never making you feel that you were asking a silly question. Dr. Irani was very responsive to my questions and my advocating to try certain things. He collaborates very well with his patients.

Some of these costs are covered by my medical insurance, Progyny. Initial Consult ~$500. Hysteroscopy under anesthesia was ~$1,000. Each IUI is ~$1,800. Plus Medications Cost (~$0.00 for orals plus thousands for the Gonal F, Lupron, Cetrotide that I bought). Omnitroupe was a pricey out of pocket expense that I wish I would have not used.

I asked to biopsy my uterus for Endometriosis out of extreme precaution and never received these results.
Dr Irani has called me personally with test results on more than one occasion, when it's usually the nurses...He has a somewhat soft-spoken demeanor and he's not intimidating or brash like some other male doctors I've seen before. He always takes the time to explain everything very thoroughly and answers any questions we have along the way.
The energy is very calming at then [Advanced Fertility Center of Chicago] office and all the staff are so warm and comforting for you. I really can't think of any weaknesses. My experience has been wonderful in terms of what the doctor and nurses can control...Ashley is one of the nicest nurses I've ever had. My tests came back all normal, except I had one fallopian tube removed due to ectopic in 2019. My husband's analysis showed slightly low morphology (3.5%).
Dr I recommended going right to IVF instead of IUI bc the chances of conceiving were much better.
Antagonist protocol - Egg retrieval & fresh transfer - resulted in MMC at 8w.
FET with estrogen priming - resulted in chemical pregnancy at 4w
Next FET will include lovenox - TBD

He let us decide to do one or two, but advised against transferring multiples due to higher risk pregnancy and delivery. We had high chances of conceiving with IVF so we agreed to do a single embryo with each transfer.
Dr Irani was very comprehensive in his explanations of treatment, risks, benefits...seemed to have a great sense of humor...When we brought up concerns based on our faith (long term embryo management, what to do with embryos that are mosaic) he was able to readjust his initial recommendations with a plan that would hopefully allow us to still only need one retrieval but have plenty of options if needed down the road.

They simply do not have enough time to see patients, while answering messages and taking/making phone calls. However, the Dr. is great and the nurses know what they are doing and what is needed. I'm about to go through the protocol with Dr. Irani. He's using a completely different protocol and different stim meds than I have used in the past with other doctors. First, I'm NOT going to start with birth control which I think my body did not react well with. I'm starting after ovulation before the baseline/stims start with estrogen. Then I will be using HCG rather than menopur, and Cetrotide rather than ganorelix. Lupron to trigger rather than ovidrel. The only stim that is staying the same as I have used in the past is Gonal-F. I have not yet started this cycle. Looking forward to seeing the difference, especially without using birthcontrol.

There seems to be very little organization at the clinic. You call the office and it's a 3rd party that answers and they transfer you to the clinic if someone is available and it's an emergency. They may or may not call you back if you do not get through. On my first visit, I was supposed to get an ultrasound and bloodwork, I only got my blood drawn. I think if they had more individuals working in the clinic this place would be great. I think they are understaffed, which makes it seem a bit chaotic.

Dr. Irani always treated me with respect and explained things both compassionately and scientifically. He was careful to go over the probabilities of success with my low AMH and antral follicle count. I am a complex patient, with premature ovarian failure and autoimmune issues. I felt that he was open to working with outside specialists to a degree; he never disputed other doctor's recommendations but he also did not fully endorse them or actively collaborate with them. He was willing to work with me using my own eggs, but he had a strict limit of antral follicles to proceed to an IVF retrieval. I consistently had only 1-2, so he would only do IUI. After 6 months, I felt that I was on a merry-go-round each month. He made adjustments each IUI cycle, but generally followed a high-stim protocol and did not deviate much from that, unless I specifically asked to try something.

I would recommend Dr. Irani to someone who has a more straightforward health profile without any reservations. For women with complex issues, such as DOR or POF/POI, particularly if their follicle count is low, I would recommend seeing someone with more expertise in those conditions.

He also prescribed progesterone suppositories after IUIs. He adjusted dosages and made tweaks to that general protocol each month.

The nursing staff in the Chicago office are excellent. The nursing team is a major strength for this clinic. I think I technically had a nurse assigned to me, but they all made phone calls, and did procedures and blood draws. Most of them remembered me, and they were all friendly and easy to work with.

I appreciate that they are open 7 days a week and 364 days a year. They do not batch patients, which was important to me. The clinic is clean and the waiting room is comfortable. The exam rooms in the Chicago office are spacious and they offer feminine hygiene items without having to ask.
The biggest disappointment to me was that they do not have a portal. I have a few doctors that I work with and that made coordinating care a bit challenging because I had to request copies of my lab results, which was no big deal but sometimes I felt like I had to explain why I wanted them. My husband expressed some dissatisfaction with the semen collection process. The receptionist would sometimes discuss financial matters with reference to specific procedures to patients while they were still in the waiting room. It would have been more respectful of patients' privacy to invite them to simply walk through the door next to her desk to have a conversation away from the other patients. That said, I found her to be friendly and otherwise respectful.

Overall this clinic is very well run. There was one time when one nurse gave conflicting information from another nurse because she couldn't find something in my chart. Nothing bad came of it but for a patient who is not savvy to the process, or to someone who is uncomfortable questioning doctors and nurses, it might have lead to confusion.
I had an IUI canceled once as a result of some unexplained clinical error or oversight. I had been in for monitoring leading up to ovulation, and was told to return again the next day for labs and monitoring. When I went in 24 hours later, I had ovulated already. No explanation was offered.
Dr. Irani fully reviewed my past treatment and medical records. He felt that I didn't need to be "over-medicated" to produce a lot eggs. He ensured we understood the process. He always called with the results. My fresh transfer ended in a ectopic and he called me directly each day to see how I was doing. He also coordinated care with my OB to ensure the ectopic was properly managed.

I was stimulating for longer but the second IVF cycle ended up with more viable eggs, and 4 made it to PGTA testing and then 2 came back PGTA normal.

The other nurses that called on behalf of Sue were excellent and left me detailed voicemails when I was unable to get to my phone which I appreciated. I only had to call back if I didn’t understand the directions given.

After a year long battle with my original fertility clinic to even SEE a doctor, I was extremely tentative about seeing another doctor, fearful of the same thing happening again. Dr. Irani made me feel welcomed and understood from the moment I sat down in his office. He even went over our scheduled time together and answered every single question I threw his way.

I am 29 and my husband is 32. We have been trying to conceive for almost 3 years now. My husbands semen analysis came back with a barely low count, just a point below "average" but nothing that Dr. Irani was concerned about due to all other aspects being normal with high volume.
Every test came back normal for me. My cycles are spot on 27-28 days long. I ovulate late in CD 10 or early CD 11 every month without fail. My follicle count, estrogen, AMH, progesterone... everything came back within normal or above normal range at the right time in my cycle. My saline infusion came back normal with no reported tube blockages.
Dr. Irani said that he was absolutely confident that he would be successful with IVF due to my age and regularity. Since we are self pay, we also had the option to skip IUI and move directly to IVF if we wanted (since we did not have insurance requirements to start with the smaller treatment). Since our diagnosis was generally unexplained with the only thing coming up as a barely low count from my husband, Dr. Irani was also confident that we were excellent candidates to achieve a success with IUI if we wanted to start with the most cost effective option. I stared Clomid on my first cycle (CD 3-7) and went in for my first monitoring appointment on CD 10. I measured with 2 mature follicles and my LH had already naturally risen, so we immediately did the trigger shot and scheduled my IUI for the next day. I returned on CD 18 for my progesterone test that came back at 29. I returned on CD 26 for my beta which came back negative and started my period the next day. We are moving right back in to another round of IUI and plan to attempt 3 rounds before considering IVF.

Describe Mohamad Irani's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Advanced Fertility Center of Chicago (Prelude Network clinic) - Gurnee.
Dr said with my age and situation he would recommend transferring one embryo only and that was almost always his preference.
Dr. Irani is a compassionate, genuine, and trustworthy man. I would highly recommend him. very caring and empathetic with the patient. He is willing to answer all my questions and emotionally support me during the treatment...I started the medications after my first ultrasound ( the second day of my cycle). Dr. Irani prescribed a high dosage from the hormone due to my problem with the number and size of the eggs.
I appreciate all Chicago office teams including the front desk [at Advanced Fertility Center of Chicago]. The nurse and doctor are very responsive to emails and phone calls. They are very caring. I recommend the Chicago office. My experience with the Gurnee clinic front desk is not the best. I think they need training on how to work with diverse patients.

I am self-pay. I can't remember exactly how much but it's worth it.
He [Dr. Irani] is straight forward but warm. He remembers details of my life (I have a son, my husband is military, etc)...He explained the fet procedure fully. I was confident with his skills. He also spent time with us earlier discussing our remaining embryos and whether we should pgs test them. He was extremely knowledgeable...Ask anything, he is really welcoming of questions...I was just doing an FET (we had 8 embryos from a previous donor egg cycle). My protocol was pill, then stopped pill and started estrogen patches and baby aspirin and prenatal, monitored via ultra sound in the 3 weeks prior to fet date. My lining was fine but estrogen a bit low so needed to add vaginal estrogen. The pio started 6 days prior to fet at night and antibiotics and medrol.
They [Advanced Fertility Center of Chicago] are great with out of town patients though I wish you got a discount on your cycles since I have to pay for monitoring out of pocket elsewhere. If I was local, that would all be covered in the cost of the cycle...the nursing staff was amazing! I always knew exactly what was going on, they were completely responsive. It was great!...FET 4000 + medications and for me I had additional of our town monitoring costs

I felt that he [Dr. Mohamed Irani] truly understood my situation and issues. I have pcos and was worried about ovarian hyper stimulation, but they were so careful -I had to go in everyday during stims for testing- felt very safe and trusted Dr Irani... responded well and had a great outcome on my first ivf treatment - I have 3 embryos post pgs testing. Thank you Dr Irani!!!

But trust the process and know that they know what they are doing.
