Dr. Tran is a thoughtful and compassionate doctor. He explained things very clearly and was straightforward about the challenges he anticipated in our case, which I really appreciated. He updated us at every step so I never felt confused about what to expect. Overall he gave us very realistic expectations...I completed an egg retrieval using an antagonist protocol (Lupron followed by follistim/menopur) and day 5 fresh embryo transfer with 2 embryos. I am currently pregnant with one (due next month).
The best thing about Spring was the care felt very personal. Everyone knew our names, and even the embryologist came to introduce himself before our egg retrieval. I felt very cared for throughout the entire process...Allie was friendly, caring, and compassionate. She always responded very promptly to questions and was very clear in her instructions regarding medication dosing, etc...I completed an egg retrieval using an antagonist protocol (Lupron followed by follistim/menopur) and day 5 fresh embryo transfer with 2 embryos. Dr. Tran and his staff always treated me like an individual. They all knew details of my case, and seeing a Dr at all of my monitoring visits made me feel like they took my cycle seriously and wanted to individualize the treatment course.

I stimmed for 11 days, then had an egg retrieval and had a fresh transfer on day 5. So far it was a classic straight forward IVF cycle.

She did a good job, was on top of things and called me back right away to answer questions. I get a little flustered that they called to tell me the dose change, I wish she just e-mailed me so I could double check later at night when I was given the shots. But I think this is more hospital protocol than her fault.

We had a complicated case of unexplained infertility and Dr. Tram talked us through so many potential options and things to try, always giving us a cost benefit analysis so we could make informed decisions. He was willing to try new things, experimental and things just being explored, and was open about them all, giving us the option to decide with his advise. We trusted him and maybe more importantly felt he trusted us.
[UCSF Fertility] is a big busy clinic and you see a lot of different practitioners. After years of care here I felt known by them all, but if you like a more top to bottom by the same small team kind of care, this isn't a fit for you...All the nurses we worked with were compassionate and caring. They also knew their stuff!...The only real issues I had were with billing. Billing Dept was terrible.

I loved Dr. Tran. He was always frank and we were an upsetting case for him...trying everything and finding no answers and no babies for so long. Our last few cycles, he did all our retrievals and transfers whether it was his week to do procedures or not. He was very invested in our care and our outcomes...cared about our mental health, our desires, our goals etc. I felt respected and understood as well as advised well.

I really delt that Dr. Tram was looking out for us but overall the whole experience still felt like we were part of a very structured, orderly system.... Dr. Tran was very professional and honest and helpful. He also checked in on us while he was on vacation (during our hormone injection cycle) which was not necessary but I truly appreciated at the time to know our unique case was being looked after. Forever grateful to this man for our beautiful, healthy child.

Weaknesses... overall it felt like we were one of thousand being treated at any given time and we HATED having to attend the crowded introduction seminar at the beginning. That felt really weird and impersonal to us.... Pretty sure we spent 30K when all was said and done. There were all the initial tests for both of us, then genetic counseling, IVF with ICSI plus TESE sperm retreval from an on-site urologist.

The strength of spring is the innovative treatment that Dr. Tran and his care team provided. I have reviewed hundreds of protocol on FertilityIQ and other sources shared by patients after my first IVF failed from another clinic. Most clinic just use 300/150 flat dose for patients, and many failed completely but the doctors will repeat the same treatment again and again with some other added medicines. If you have failed 300/150 GonalF/Menupur, I would recommend Spring which you can try a optimized protocol and might get a surprising good results.

Dr Tran is a busy man so have to take all my questions with me for the day. He does make time for me when I need to talk or have so much questions, but don’t get to see him at all my appointments. It would be nice to have him to all my appointment but I understand that he is a busy man and he does most of the clinic in SFO. ... Well for start he talks in a manner you understand very step on the treatment. Makes you fell like a person and not just another woman coming through.
Good nurse that is loving and caring , with an understanding. Making sure you get to know the doctors as dr Tran will not be there [Spring Fertility San Fran] all the time. Go prepared with all the questions and any other medical questions you may have ... She [nurse] is my go to person makes everything easy. once on the drugs, you'll care more about your doctor's communicativeness and kindness than you think you will now, so I suggest not seeing Dr Tran at all, unless you have a very strong patient advocate with you (e.g. a partner) who can provide reassurance and interface with him on your behalf if his manner becomes too much.
During treatment, were you treated like a number or a human with Nam Tran at Spring Fertility San Francisco?
1/ I presented as a very normal and "optimal" client (no infertility issues; treatment at age 31; 20+ eggs naturally available in each ovary), and yet was given an irregular hormonal treatment pathway that a second opinion (another Dr at the same clinic) found so confusing and inexplicable he had to call Dr. Tran about it. While I'm not against being part of a test data set, I was never told the theory behind my protocol and why it was what it was - so I felt a lot of anxiety about injecting mystery drugs into my body. 2/ Dr Tran seemed visibly bored and impatient in all of our conversations. 3/ My treatment, while optimized for egg retrieval, gave me brutal OHSS afterwards.

I felt very confident in my injection procedure throughout. And they gave me chocolates on retrieval day which felt really good!

Spring is trying to be modern/tech but hasn't quite reached it yet. So the systems are a little janky but have a veneer of niceness... I think it's an OK clinic, don't pick it if you are expecting a smoother process than at RMA or something. I went with them on friends' recs of Dr Klatsky and wasn't impressed with Dr Tran. But they did get OK results for me, and though my OHSS was terrible, I did survive it.

I think he’s [Dr. Nam Tran] very smart and hope we’ll eventually get good results. We see him so infrequently and even when I request to speak with him, I get an email from his care team instead. I think he’s very smart and hope we’ll eventually get good results. We see him so infrequently and even when I request to speak with him, I get an email from his care team instead. So it’s hard to assess. He knows the statistics and is honest with you. He is very focused on the facts.

when Dr Tran was at UCSF, he went out of his way to be the physician that handled my surgery, retrieval’s and transfers. Being at a teaching hospital, it is nearly impossible to expect your physician to do the actual procedures. But he’d rearrange his schedule and make sure he was there for all the major things that had to get done.

He paid close attention to my lab work, and I trusted that he'd review everything before a decision was made on what my dosage or next treatment steps would be. We did not, however, discuss these matters in depth, as he keeps appointments very brief. But I preferred a doctor committed to the science and data over a doctor focused on bedside manner, and I understand that the clinic is managing many patients. I liked that he performed all of my major procedures - retrievals and transfers. While I was able to see him for many ultrasound check-ins, I could not see him for all of them. It was difficult to schedule consultation appointments with him to discuss treatment - these often were scheduled weeks ahead of time, and were fairly short.
What's one piece of advice would you give a prospective patient of Nam Tran at Spring Fertility San Francisco?
Do not expect extensive or in-depth discussions, consultations, or bedside attention. He can make check-in appointments feel rushed, but he spends his time on the labs and data behind the scenes. He is decisive and confident in his treatment decisions, and I found him very trustworthy. I could moreover tell how much he cares about getting good results for his patients, and I did not feel anonymous to him -- and I liked that he was more focused on my labs than on making good small-talk with me.
During treatment, were you treated like a number or a human with Nam Tran at Spring Fertility San Francisco?
Dr. Tran paid close attention to my labs. I trusted that he was tailoring treatment to my particular indicators and would see me as frequently as needed to obtain the data he thought was scientifically necessary to make good decisions -- rather than putting me on a cookie-cutter protocol or trying the same things over and over. While he moves quickly, he is paying close attention to what matters medically and will not sugarcoat the information. The instances in which I felt more like a number were in the followups from staff. One time, clinic staff followed up with the wrong medication prescription, which was caught because I called to triple-check. Another time, Dr. Tran had stated I should borrow medication from the clinic to cover an extra unpredicted day of treatment, and clinic staff prevented me from doing so -- I had to drive in-person and wait hours at my pharmacy to make sure I'd have the needed medication that day. This added undue stress to an already complex medical process.

The clinic is fairly well organized but I did notice throughout the course of my time as a patient that it got busier and harder to reach with questions, scheduling issues, etc. The clinic's use of an app, rather than email or phone, as primary mode of communication was a plus in many ways -- easy to see documents, instruction videos, and medication notes -- but for certain questions, I would have preferred a conversation with my assigned nurse. I also had times when I waited a while for a response on something time-sensitive. In addition, I did not find the finance staff very proactively helpful in assisting me with billing and insurance questions. Some phlebotomists are highly experienced, but others seem very new on the job which made blood draws difficult at times. There seems to be increasing turnover/ addition of new and very junior staff. I did always receive medication instructions on time and liked having written confirmation via the app.

My only negative is that once you have your initial appointment with Dr. Tran, you don't see him again until your first monitoring appointment, so if you have questions or concerns that come up after the 1st appointment, you pretty much have to rely on talking to the nurses to answer those questions...While he wasn't what many would call overly sweet, he was respectful, concise, and thorough in his answers...On the day of retrieval, Dr. Tran answered my questions and was kind to me, holding my hand right before the procedure, as I started to cry from being scared/anxious.
They [nurses at Spring Fertility] also sent several follow up emails to check in on my pain and recovery. They also called to check on me. I felt like my concerns were addressed always in a timely manner, even if it wasn't my primary RN responding...she spent a lot of time explaining things in depth and I felt so much better after talking to her...

Dr. Tran is incredibly smart and advanced in his field. In fact, my friend's boyfriend who is an embryologist (not at spring) actually mentioned his name specifically as a good doctor to consider using. To me, this meant a lot and after researching and reading reviews I decided to use Dr. Tran and I'm glad I did. As a travel nurse for 13 years, I'm familiar with the wide array of physicians personalities and bedside manners. While he wasn't what many would call overly sweet, he was respectful, concise, and thorough in his answers. To me, knowing his experience level, and that he is the medical director, I am perfectly fine having a doctor be blunt and to the point about my medical care. I came to my appointment was a decent understanding of egg preservation after research and talking to other women who have undergone the procedure. I think because I was prepared, he didn't spend a lot of time explaining the process from beginning to end - but I think this is because he knew I had done my research. I think if I had not done this research, I would have needed a bit more of an in-depth explanation about the process than I received. I had read others tips to be prepared with questions for him, which I was and he answered candidly. My 1st 3 appointments for monitoring were with Audry, the NP who was also fantastic. Because I needed midday appointments I saw her, as Dr. Tran does AM monitoring. Later in my process, I had morning appointments with Dr. Tran. I believe he was training another Doctor who did a couple of my ultrasounds (I cannot remember this doctor's name) while Dr. Tran was in the room and answering my questions. This doctor that was training was not as gentle and communicative as I would have liked during my ultrasound. Perhaps she was distracted, but I think some work there can be done on bedside compassion and communication. On the day of retrieval, Dr. Tran answered my questions and was kind to me, holding my hand right before the procedure, as I started to cry from being scared/anxious. My only negative is that once you have your initial appointment with Dr. Tran, you don't see him again until your first monitoring appointment, so if you have questions or concerns that come up after the 1st appointment, you pretty much have to rely on talking to the nurses to answer those questions - I was told his phone appointments were booked well in advance when I asked to speak to him before my monitoring appt. Luckily, I was able to talk to one of the nurses who answered my questions before my 1st monitoring appointment. Dr. Tran managed my meds and care flawlessly. I was at high risk for hyperstimulation and we developed a plan to prevent this from happening. I'm not sure other doctors would have been so cautious with my care (as I've met other women who were also potentially high risk but their doctors at other clinics were not as cautious with their medications.) Because Dr. Tran managed my meds and procedure so well, I'm ever grateful I chose him, as he is confident and extremely experienced and did a fantastic job with my care. The only thing I think could have been done better, was a better explanation of the types of discomfort I would feel. I had expected "bloat and discomfort" but as I stimulated, I was having notable cramping - the left side was more pelvic and the right side radiated into my lower back and was quite intense. I had not expected this pain and wish someone had warned me. Also, after my retrieval, I had expected more pelvic cramping but the pain I experienced was high up in my abdomen under my ribs all the way down to my lower abdomen and my entire abdominal cavity was distended and hard. This pain was more gastric in nature - like cramps/gas discomfort you might experience with a bad gastrointestinal bug - only had pelvic/vaginal pain a few hours after the procedure.
What's one piece of advice would you give a prospective patient of Nam Tran at Spring Fertility San Francisco?
Research on your own before you come, so you are prepared with questions. Take your time asking what you want to know and speak up if you don't understand. Personally, as an ER nurse I knew I wouldn't be able to work during the stimulation/right after retrieval, as risk of ovarian torsion with high follicle count must be considered and work in the ER is very active. Also, even if that wasn't a risk, I was so uncomfortable mid stimulation- 1 week post retrieval that I know I couldn't have gone and worked 12 hours. So definitely ask Dr. Tran the true expectations of how you will feel and how it will affect you/and your work schedule. Everyone is different, but I'm really glad I was able to be off, as my experience was more uncomfortable than what I had been prepared for.

Describe the protocols Nam Tran used in your cycles at Spring Fertility San Francisco and their degree of success.
Knowing my follicle count was initially 39, I started with Menopur 200 iu/Gonal F 150iu for 3 days. After my first monitoring appointment, It changed to Menopur 150iu/Gonal F 150iu x 4 days. On stimulation day 7, I started the agonist, Cetrotide daily in the AM(this one burns/itches fyi - normal). On stimulation day 8, menopur 75iu/gonal f 150iu. On stimulation day 9 meopur 75iu/gonal f 75iu x 2 days. On day 11 of stimulation (saturday - luckily Dr Tran was on call so I saw him in office) I just did my agonist in the AM and only did ONE TRIGGER shot- LUPRON only at 8:30pm- this was the plan, as I was high risk for OHSS due to high number of follicles and the HCG trigger shot is notorious for making OHSS worse for high risk women. The next day I didn't do any more shots - just went to the Oakland clinic for a blood draw to make sure the Lupron trigger alone was enough to finalize maturation of the eggs (we had planned that if it wasnt, I would also trigger with HCG - but only if necessary). The Lupron worked, so I didn't have to do any more shots. My retrieval was at 0830 the following Monday. 42 eggs collected and 37 were frozen later. Dr. Tran also called in Norco for pain, which I did end up using for about 3 days as my abdominal discomfort post retrieval was pretty intense. He also called in 7 days of a birth control pill to help offset any symptoms of OHSS. About 3-4 days after my procedure the abdominal pain and swelling/bloating started to significantly decrease and I was able to control my discomfort with tylenol and motrin.
Describe your experience with your nurse at Spring Fertility San Francisco. (Assigned nurse: Sharon)
Sharon was my primary nurse, but during my stim she was on vacation some, so I did interact with other nurses. After my initial appt, but before my 1st monitoring appt/start of stimulation, I had some questions and was not able to speak to Dr. Tran. I was confused about the medication packages and options as well as had concerns about OHSS. I spoke with Nurse Jill and she spent a lot of time explaining things in depth and I felt so much better after talking to her that day. Also, I saw Audry the NP for several monitoring appts and she was great - very compassionate and gentle, as the ultrasounds can be uncomfortable as your ovaries grow. She was also very present with me, as I cried one day in the office (stupid hormones) and gave me a better understanding of the pain/discomfort I would feel as my ovaries grow, as I was surprised at how much back pain I was having. I also spoke to Ursula the NP on the phone one night after stimulation as my abdominal gas/cramping/distension had gotten pretty intense. She helped alleviate my concerns and called in more pain meds for me. They also sent several follow up emails to check in on my pain and recovery. They also called to check on me. I felt like my concerns were addressed always in a timely manner, even if it wasn't my primary RN responding.
Describe your experience with Spring Fertility San Francisco.
I really liked Spring and have already recommended it to friends. There's a lot of prep work - documents, financials, etc to complete and while my coordinator was helpful, It felt like a lot to complete while also working a lot. I would have preferred to speak to financial department sooner than I did. Perhaps somewhat my fault as I was working night shift a lot and was limited on daytime hours to talk to them.
