My experience with Dr. Klatsky in 2018 for the egg retrieval was flawless. He kept me updated and educated on why he was doing particular procedures or prescribing medications and was very empathetic to all of the side effects throughout the process. When I came back in for the embryo transfer in 2020, I felt more disconnected. He relies heavily on his IVF nurse lead (who was great) to keep track of medications and cycle stages. There was one appointment i particular that he wasn't aware of what medications I was currently taking and he made recommendations to change medication and move up the cycle. I then received an email from his nurse later that day stating to not make the changes the Dr. had recommended, because he was not aware of exactly what meds I was on and to keep things as originally scheduled. Dr. Klatsky did follow up after our failed embryo transfer, answered all of our questions and was very empathetic and apologetic about the miscommunication that had occured throughout the FET cycle. He discussed in depth the plan for our next FET and why we would take that approach and it has been scheduled to happen within the next two months.
Overall Spring is a good clinic. ... The billing department is very responsive, but it took many emails and hours over the phone with them to get the proper paperwork to try and submit to my out of network insurance. My Dr. was changed the day of my transfer to Dr. Tran (whom I have never met or interacted with) and I was not informed until he came into the room to do the transfer and introduced himself. This was very off putting to me and added a lot of stress on a day when you do not want your stress levels to be high. The clinic was very apologetic about it and I trust that they will have better communication with me for our next FET.

The advice that I would give to a prospective patient of Peter Klatsky is to make sure you ask a lot of questions and don't hesitate to request for a follow up phone call if more information is needed or questions need to be answered after an appointment or consultation.
During treatment, were you treated like a number or a human with Peter Klatsky at Spring Fertility San Francisco?
The first time I came into the clinic in 2018 I felt like I was treated like a human and not just a number. My Dr. and all of the nurses knew me by name and by the end of my egg retrieval the started to feel like family. When I came back in Feb 2020 to start the embryo transfer process, my experience was a bit different. It seemed like the waiting room was more crowded, procedure dates and times were changed 2-3 times due to scheduling issues (not due to my cycle) and there was some miscommunication happening. Thankfully, the missteps that were made were quickly corrected along the way and did not affect my treatment protocol.

My husband and I had been trying for over a year to get pregnant before we came into the clinic. Dr. Klatsky preformed a saline sonogram, and HSG, an ultrasound and ordered blood work for both my husband and I. He then diagnosed me with Unexplained Infertility saying that all of the tests came out clear. He also took into account the results of my husbands sperm sample that we had previously had done. He recommended IVF, stating that IUI has less chances of working to get us pregnant. We went with his recommendation and started IVF asap. We did a retrieval under his care and did pgs testing at his recommendation to prevent miscarriage or failed FETs. We then did a medicated FET that was not successful. Dr. Klatsky recommended a medicated FET (with intramuscular progesterone shots and estrogen patches), because during the retrieval he stated that my lining was pretty thin and that the medicated protocol would rule out the lining being a factor. Dr. Klatsky, as well as my husband and I were all pretty surprised when the first round didn't work, since everything went well, lining looked good and the embryo was a "perfect" 5AA. After following up after the transfer, Dr. Klatsky has changed his approach for our next FET to a natural transfer, as I reminded him of my history of pelvic tension issues, interstitial cystitis and spotting. He wants to make sure that endometriosis isn't a factor, although I don't have many of the symptoms and he has done the majority of non- invasive tests to rule it out. Just in case I do have it he does not want to give me estrogen for the next cycle, which causes endo to flare. It will be a risk because there is a chance my lining may not be thick enough on transfer day, but he thinks it is best to try something different than last time to rule out endometriosis or any other factors that may be preventing us from getting pregnant.

Laurel Maley is our nurse coordinator. She is a great communicator, responded promptly, is compassionate and always in the know of what medications I am taking and was stage of my cycle I was in. She was great and one of the best parts of my experience at Spring.

Overall Spring is a good clinic. From the waiting room to the Dr.'s offices it is very clean and welcoming. The nurses and staff are all very nice and greet you with a warm smile every time you are in the office. It seems like their client load has increased since we first came for our IVF retrieval in 2018, which I think was the cause to procedures times being altered multiple times and long waits for appointments. Dr. Klatsky recommended against putting in multiple embryos at my age (33) and with my diagnosis (unexplained infertility) as it could cause a high risk pregnancy (multiples). I don't even think the clinic allows people to ask for multiple transfers at my age or with my diagnosis, but I am sure there are certain exceptions that they may do so. We allowed the clinic to pick the best out of our 3 pgs tested embryos and asked for the gender to be confidential and not shared with us.

My tansfer procedure was rescheduled to 3 different times the day leading up to it, one of the times being only an hour and a half before the procedure was originally scheduled for. Dr. Tran did my transfer the day of and I was not informed that it would not be my primary doctor Dr. Klatsky until Dr. Tran walked in to do the procedure and introduced himself. The changing of appointment multiple times and not knowing that the Dr. for my transfer had changed until the moment of my transfer added a lot of stress to an already stressful process. It was apologized for by the nurse and Dr., but was definitely very upsetting and frustrating.

Dr. Klatsky from the moment on our first zoom call was the first doctor to listen to my medical concerns. He let me know what was happening was wrong and validated my feelings. He took a course of action to actually find out what was wrong and why after five years I could not get pregnant...Even though it didn't work for me I was do happy to finally have a diagnosis of why...He truly is an amazing doctor and I'd recommend anyone to him.
The [Spring Fertility New York] clinic is clean, well organized, the staff is very friendly and knowledgeable, everything from check in to checkout goes smoothly...I could call [nurse] Kris at anytime and she was always there. She would give me the best medical advice and treatment plans, if she didn't know the answer she called me back within an hour after asking the doctor. It felt like she was my nurse and counselor. When the transfers didn't work she was right there on the phone with you making sure that you were okay.

I felt like a human because they truly care. I traveled there from six hours away and saw him in the hallway and he knew who I was. You get your own nurse as well. I call and she answered. I felt cared for, listened to, and everything was not sugar coated it was explained in full detail.

For me mine was tricky because I was allergic to everything! We just used Letrozole and a steroid to get my lining thick.

Nothing negative to say, they are top notch. The clinic is clean, well organized, the staff is very friendly and knowledgeable, everything from check in to checkout goes smoothly. They are one of the best in egg quality I had a friend go here who had three failed egg retrieval. She came here and was able to get 4 viable embryos! They are just truly the best.

Dr. Klatsky has a lovely, calming presence. He’s obviously brilliant, given what he’s able to accomplish, but it’s his bedside manner and his ability to put you at ease that makes him wonderful. He is patient and soft spoken and thoroughly explains everything so you’re well informed...Dr. Klatsky provided facts and guidance which ultimately led to us deciding to do another retrieval, which was even more successful. I had been eager to transfer after the first retrieval but am so glad Dr. Klatsky explained how we might be more successful at my age with another retrieval - and we were! Dr. Klatsky never pressured us, but explained all options and offered us a choice, which we hadn’t always been given in the past.
Spring’s facility is clean and state of the art, but doesn’t feel cold. In fact, every detail was curated with the patient’s comfort in mind - from the chairs that patients sit in before procedure, to the bandages used after blood draws. Everything was well thought out to put the patient at ease. Appointments run on time, which is helpful when scheduling prior to work. The receptionists at the front desk are friendly and discreet. My only hope would be that Spring would accept more insurance providers. We were fortunate enough to be able to pay out of pocket, but supplements from insurance would have been helpful.

Dr. Klatsky provided facts and guidance which ultimately led to us deciding to do another retrieval, which was even more successful. I had been eager to transfer after the first retrieval but am so glad Dr. Klatsky explained how we might be more successful at my age with another retrieval - and we were! Dr. Klatsky never pressured us, but explained all options and offered us a choice, which we hadn’t always been given in the past.

Dr. Klatsky’s team always treated my husband and I with respect to identify the best go-forward solution. Having been to three prior fertility clinics, where we were told our unexplained infertility was just a “war of attrition”, Dr. Klatsky and his team sat with us, discussed our options and determined the best way forward. It was novel to be listened to, and not treated like we were desperate to have just any procedure performed. Dr. Klatsky used our data to try to determine the next, best steps for our family.

We had previously been to three fertility clinics, and had three rounds of IUI and four rounds of IVF, all of which were unsuccessful. Looking at the data, Dr. Klatsky recommended lower doses of medication for a day or two longer than I had previously taken. The strategy was to allow my slower growing follicles to become more mature. As a result, we retrieved high quality grade embryos, which were successfully transferred.

Maggie was our nurse coordinator. She was the most patient, empathetic and fast responding nurse we’ve ever dealt with. Maggie communicated instructions in a clear, calm manner and was patient when responding to questions. She eased a lot of worry and anxiety with her demeanor. I found that all of the nurses at Spring shared the same professional, compassionate and emotionally intelligent qualities. From morning blood draws to egg retrievals to transfers - everyone involved in the process helped to reduce the level of worry and anxiety you’re naturally feeling.

I was blown away the amazing treatment I received under Dr. Klatsky's care, and am so thrilled with the results of my egg freezing cycle with him. Dr. Klatsky understood that I have some irregular factors (low ovarian reserve, irregular cycle.) He also knew that I'd gone through a cycle 1 year prior at a different NYC clinic that did not go well. The biggest differentiating factor is that Dr. Klatsky customized my treatment from beginning to end, adjusting my meds throughout the cycle in response to every scan and blood test. His personalized approach made an incredible difference, and resulted exactly Double (!) the number of eggs frozen in a single cycle. He is also an incredibly personable doctor with excellent bedside manner, and I always feel like he is both the captain of my treatment team, and also rooting for me personally.

Never for a moment did I feel like a number or a run-of-the-mill case, which is exactly how I had felt after my experience at a previous practice. Dr Klatsky and the team did not rush me into starting a cycle, were patient with my irregular cycles, made a point of understanding what was going on with my body before going ahead with forming a plan, and generally treated my case with the level of attention and thought it deserved... And the result was a doubly successful cycle under his care vs my cycle at the other fertility clinic.

Mid-Luteal start, 12 days of injections (Cetrotide in AM days 6-12; Menopur and Gonal in the evenings Days 1-12.) Trigger shots were lupron (.8) and Novarel 2500. I began with 8-9 antral follicles, 9 were retreived, and 6 were mature/frozen.

While most of the process is managed through the portal and calls from the nurses, Dr Klatsky was always available to schedule calls to speak live. And the nurses always called directly and when they said they would.

They always called as promised. There were sometimes miscommunications, but broadly things were pretty smooth. I feel like it was not consistent who my nurse was, but they were all friendly and responsive.

Dr. Klatsky moved from SF to NY during our treatment, he finished the egg retrieval and put together the plan for embryo transfer, but another doctor did the actual transfer. This was still pretty smooth, since the Klatsky nursing team stayed with us. I found the billing department to be a bit unorganized, but not too bad. We did this during COVID (Sept/Oct 2020) and it felt safe and also it was not super hard to get appointments or attention.

Trust him. He is up to date on current recommendations and treatment plans. There were times when I wanted to try an add-on treatment if things weren't working but after we looked at the research together we decided against it. He was open to a discussion but was clear about the limitations of small studies and why he advised against these (cost, no clear difference in outcomes, etc.). In the end, the research based treatment protocol worked for us, we just needed to be patient and trust his instinct and years of experience.

He is very knowledgable and cares about making sure you achieve your plans. That said, if you are not sure about whether you want to do treatment, make sure you say that up front in a consult since he may assume you came and are planning procedures.

First retrieval - July 2019: did high stimulation protocol. I had 6 follicles so we debated doing high stim (a more typical cycle) vs low stim. He decided to go for high stim. Unfortunately, we ended up with nothing. 11 eggs retrieved, 5 were mature, 4 fertilized, 3 made it to day 3, 1 made it to embryo, did genetic testing and it was not genetically viable. 2nd retrieval - March 2020: Did low stim protocol. Primed with estrogen and then used Gonal-F and Menopur for about 2 weeks at a lower dose than before. Got 6 eggs. 5 were mature. 4 fertilized, all 4 made it to embryo stage. Did genetic testing and 3 viable embryos as a result. We were really happy with this outcome, given my number of follicles.

We did an IUI cycle with letrozole and trigger shot [with Dr. Peter Klatsky]. I asked for letrozole over clomid because I had researched that it had fewer side effects...We’ve decided to start with 3 cycles of IUI and if need be go to IVF next. My care coordinator Kelsey [at Spring Fertility] is fantastic! She’s always prompt in responding to my emails and helped me check off everything I needed to get done prior to starting out IUI cycle. I really like working with her. While she was gone on vacation, I still felt very taken care of by the other staff at Spring...Each IUI is about $1600. Letrozole and trigger shot about $100ish...Clean and modern facility. You get access to the Spring portal online that has everything you need for your cycle which is great. I'd advise them to come in with questions and a notepad when you have your initial consult. He will have a lot to share with you and you'll have an in depth conversation on what's next for you. This meeting is important and can feel overwhelming so it's helpful to have some questions in mind and a place to put your thoughts. That said, his care team is incredible and will always follow up with detailed notes and ensure you have everything you need.

Upon meeting Dr.Klatsky, he did not ask us why we were there or what we were looking for. He immediately launched into a pitch about IUI and IVF. I told him that we needed to back track the conversation because we were there for fertility assessments, understanding if our bodies had any issues, and weren’t yet ready for IUI/IVF. He got pretty defensive, which tanked the remainder of the discussion. We got a little bit of information from him, but not nearly the amount we would have gotten if he had met us where we wanted information instead of trying to shove us into the IVF/IUI sales pitch box.

Dr. Klatsky was there to sell us IUI/IVF. We had to push him to get off that track and discuss our bodies, where we are physically, and where we are on this journey (at the time, we weren’t ready for IUI/IVF). When I redirected the conversation, I accidentally referred to embryos as babies, prompting a long, condescending lecture on how embryos are a distinct protected class. As a patient, it is not my job to get all of the terms right. I do not have an MD. I should not be spoken to that way when I slip up on fertility vocab. It is my job to get what I need out of an appointment and with Dr. Klatsky, that was a struggle. He didn’t connect to what we wanted, he was focused from the get go on selling us IUI/IVF, and he became defensive and condescending when we tried to direct the conversation to where we needed it.

Upon meeting Dr.Klatsky, he did not ask us why we were there or what we were looking for. He immediately launched into a pitch about IUI and IVF. I told him that we needed to back track the conversation because we were there for fertility assessments, understanding if our bodies had any issues, and weren’t yet ready for IUI/IVF. He got pretty defensive, which tanked the remainder of the discussion. We got a little bit of information from him, but not nearly the amount we would have gotten if he had met us where we wanted information instead of trying to shove us into the IVF/IUI sales pitch box.

1 IUI with Clomid because I needed 3 IUIs in order to move onto IVF for my insurance. (I did 2 other IUIs through Kaiser.) No babies produced. Then I did 1 round of IVF resulting in 4 perfect embryos - 1 failed fresh transfer and 1 positive FET - currently 8 weeks. (I had NEVER had a positive pregnancy test, so it's possible!)

Dr. Klatsky's team was VERY kind and compassionate. There seems to be a lot of nurses, which made for a confusing experience to begin with, but everyone was friendly and professional!

I had no idea I needed to stay on top of my inbox for an email from them, even though I had already discussed everything in detail with the nurse at the clinic after my appointment. Halfway through my IVF meds, they simplified and only had 1 nurse contact me with next steps, which was more organized and thoughtful. I knew what to look for. I would suggest for the patient to set expectations on communication from day 1.

I’m not sure which specific protocol Dr. Klatsky put me on, but I had the saline sonogram and he saw that my uterus was healthy and smooth so I didn’t undergo a lot of testing. As I’m young, healthy and technically not infertile, I will be on menopur/Gonal-f/cetrotide/lupron. I have insulin injections as well. Dr. Klatsky said my injection amounts are a little more aggressive so they’re hoping I will produce more good quality eggs.

Helen is really friendly and very responsive to her emails. I had a lot of issues with getting my medications and Helen was very accommodating and quick with helping me call and send prescriptions to different pharmacies. I am extremely appreciative of her friendly can-do attitude and speedy responses!

IVF is covered through my work, but I realized their retrieval and transfer facility is out of my network so I had to pay for their facility fee which is not bad. Dr. Klatsky knows PGS/PGD testing is pricey so he works with a company that if they diagnosis you then they offer $99 PGS/PGD testing (not including shipping/storage fees). With all those fees included, the testing would cost $600 which is way better than $5k+.

He never pressured us to choose a course of action or even undergo subsequent fertility treatments, which he could have because it meant more revenue for him. For example, he even told us that we were likely to get pregnant on our own at some point and it was up to us whether we wanted to wait for that outcome or pursue treatments if unwilling to wait. We really appreciated that.

Furthermore, sometimes when she told us she would contact us with some update on X date, that date would pass with no contact requiring us to follow up. I think that much of this delay was due to the clinic being unexpectedly understaffed for the number of patients it treated so I think that Helen was just trying to stay afloat.

My treatment consisted of Gonal-f, Menopur, and Cetrotice. I triggered with Novarel and 4mg of Lupron. Aside from the bloating, which was to be expected, I didn’t have any significant issues. The doctor projected 10 - 12 eggs would be retrieved; however, to my surprise, 21 eggs were retrieved and all matured.

He initially suggested that I shouldn't move forward with egg freezing because I wasn't a good candidate and he didn't want me to waste my time and resources...to test my egg quality, I choose a sperm donor and had success freezing embryos. The first round of embryo freezing produced no embryos but the second time, Dr. Klatsky adjusted the treatment and I was able to freeze 5 embryos.

In the beginning they were personable, accessible, easy to book appointments and consistent with the assigned nurses. Over the course of a year as Spring's profile and patient list grew it became it lost the flexibility to accommodate schedules, assign the same nurses, and have that extra personal touch, but nonetheless the experience and success of my treatments remained the same.

[Spring Fertility-SF's] Strengths—doctors—you see your primary doctor most of the time for all appts (and they rotate on weekends)—all of the doctors are caring and talented. The front desk staff is well organized and helpful. All of the nurses are very knowledgeable and prompt with responses. Okay The medical assistants vary in their abilities and some struggle with blood draws which can be frustrating. Weaknesses The finance team is a nightmare to work with if you are using insurance.

Strengths—doctors—you see your primary doctor most of the time for all appts (and they rotate on weekends)—all of the doctors are caring and talented. The front desk staff is well organized and helpful. All of the nurses are very knowledgeable and prompt with responses. Okay The medical assistants vary in their abilities and some struggle with blood draws which can be frustrating. Weaknesses The finance team is a nightmare to work with if you are using insurance. Slow to respond if they respond at all and they struggle to effectively work with insurance companies on claims. I’ve learned a ton about medical billing during the process because of the errors and lack of proper communication with our insurance company. Be sure to understand the entire cost of treatment and check your bills and claims frequently.

Out of pocket costs were a bit more expensive than larger Bay Area clinics but they take insurance (big positive!) and you get more time with the doctor than in the larger clinics. They offer good deals for medication packages and genetic testing. Insurance costs have not been clearly described and it is extremely challenging to get clear answers (or any answers) from the spring fertility finance team.

Since we had pgs tested our embryos, he recommended single embryo transfer which was our choice too.

One time there was an issue with their message system and we did not get an update that the doctor wanted to see us earlier than our scheduled appointment. We worked through everything and got an appointment the next day and they fixed their message system. We been told conflicting financial information a few times.
