[Dr. Foulk] used the very latest medicine to help us conceive our child...plan of care was innovative...manner is reassuring...willing to communicate with me about setbacks or about my concerns...only drawback about such a great doctor is that he is super busy and I did not see him very often...Our protocol was rather long and also intense...PGS testing was my choice because my husband and I did not want multiple failed transfers. We had Dr. Foulk’s full support.
[Utah Fertility Center is] incredibly clean and everyone is so nice...loved the phlebotomist, the anesthetist, and the embryologist...front desk often struggles to connect you with someone who will return your calls...Billing is easy to work with and helpful. They were great at helping with payment plans...[nurse] was on top of my care, gave me her cell phone number to text anytime, called me to review all results.
How was your experience with Russell Foulk at Utah Fertility Center?
Dr. Foulk is an incredible doctor who used the very latest medicine to help us conceive our child. His plan of care was innovative. His manner is reassuring. And he always was willing to communicate with me about setbacks or about my concerns. The only drawback about such a great doctor is that he is super busy and I did not see him very often. I did feel that I could always communicate with him at any point, but, I do wish I had seen him at some other visits.
What's one piece of advice would you give a prospective patient of Russell Foulk at Utah Fertility Center?
Be ready for aggressive treatment, he is willing to move at your pace, but, if you want a baby he will move heaven and earth to get one in your arms. He’s optimistic but also realistic. You won’t see him or speak to him often, but I do know that he oversaw all my details and was always ready to answer questions.
During treatment, were you treated like a number or a human with Russell Foulk at Utah Fertility Center?
Dr. Foulk was kind, calm, compassionate, and knowledgeable. He knew the details of my case and was always willing to answer questions.
Describe the protocols Russell Foulk used in your cycles at Utah Fertility Center and their degree of success.
We used IVF with ICSI and PGS testing. Our protocol was rather long and also intense. I told him that I wanted everything that could increase my chances of conception. It included HGH, and an antagonist protocol. We knew my eggs were compromised with an endometrioma on my one ovary (the other had been removed many years prior) and tried to get as many eggs as possible. The PGS testing was my choice because my husband and I did not want multiple failed transfers. We had Dr. Foulk’s full support. He was a good balance of “here’s the facts here are my suggestions and you tell me what you want to do”. Prior to transfer we also did an endometrial scratch and embryo velcro.
Describe your experience with your nurse at Utah Fertility Center. (Assigned nurse: Chelsee)
Chelsee was AMAZING. She was on top of my care, gave me her cell phone number to text anytime, called me to review all results. I loved working with her.
Describe your experience with Utah Fertility Center.
They are incredibly clean and everyone is so nice. I loved the phlebotomist, the anesthetist, and the embryologist. Every person I interacted with was kind and helpful. The front desk often struggles to connect you with someone who will return your calls. This is a source of frustration and I found talking to my nurse coordinator about it helped. Billing is easy to work with and helpful. They were great at helping with payment plans.
Describe the costs associated with your care under Russell Foulk at Utah Fertility Center.
I believe our costs were around $30,000 including PGS testing, ICSI, and a procedure for polyp removal I needed.
Describe Russell Foulk's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Utah Fertility Center.
He encourages a single embryo transfer for us because we only had two normal embryos and I had a history of difficult pregnancy.
What specific things went wrong at Utah Fertility Center?
  • Failed to call with results
Describe the specific things that went wrong at Utah Fertility Center.
There was a mistake where my original nurse didn’t accurately call back with tests. I requested a new nurse coordinator when we did IVF and had a better experience.

Dr. Foulk allowed me to be me and talk with my true emotions...He never once dismissed any of my concerns and fears...During my 2nd round of hormone treatments, Dr. Foulk was determined to make sure I make it to egg retrieval. He increased my doses slowly & we saw results...he understood my religious approach with going forward with a transfer...He really takes the time to go over everything with you and is very open on trying different approaches...Due to the quality of my eggs always coming back as "abnormal" after PGS testing, Dr. Foulk was not against doing multiple embryo transfers
[Nevada Fertility Center nurse and staff] They were so compassionate and understanding that no question( regardless if I thought it was a little dumb ) went unanswered... I had additional fees to consider such as using a donor sperm...I was a participant who won 50% off a frozen IVF cycle and I was also part of the CLARA clinical trial so my out of pocket expenses weren't too much...my first round of Frozen IVF w/ the 50% discount came out to about $7000. Each egg retrieval was around $450...The only negative/weakness is the waiting for the call back for results
How was your experience with Russell Foulk at Nevada Fertility Center (FKA SIRM Las Vegas)?
Dr. Foulk is honest, patient, and amazing. The only thing I would say that is negative is that he travels from the clinic in Las Vegas and Utah so it is hard to schedule an appointment. But when I do have the chance to sit with him, no matter the outcome he is always there with a smile on his face and his tone of voice always gives me so much hope. He really takes the time to go over everything with you and is very open on trying different approaches. I have yet to hear him decline or laugh at my crazy ideas. As one of his patients, I cannot express how much I am grateful to put this dream of mine in his hands.
What's one piece of advice would you give a prospective patient of Russell Foulk at Nevada Fertility Center (FKA SIRM Las Vegas)?
Go into your first visit with an open mind. Don't expect everything to happen at once, but once the ball starts rolling Dr. Foulk & his staff will be by your side 1000%. Dr. Foulk is also a very patient & knowledgeable man. So if you have questions, I would ask away.
During treatment, were you treated like a number or a human with Russell Foulk at Nevada Fertility Center (FKA SIRM Las Vegas)?
Dr. Foulk allowed me to be me and talk with my true emotions. Even with tears running down my cheeks he and his staff has been amazing throughout my fertility journey. Unlike a doctor at a clinic, he never dismissed you when you walked passed. He always had a smile and greeted you even if he was rushing through the office between patients. Dr. Foulk also listened to everything I had to say. He never once dismissed any of my concerns and fears. I love his motto: "I was meant to be a mom.." and I am so happy that he is as determined as I am to make this dream for me come true.
Describe the protocols Russell Foulk used in your cycles at Nevada Fertility Center (FKA SIRM Las Vegas) and their degree of success.
My body always seemed to fail me when it came down to my hormones. It was never where it needed to be. During my 2nd round of hormone treatments, Dr. Foulk was determined to make sure I make it to egg retrieval. He increased my doses slowly & we saw results.
Describe your experience with your nurse at Nevada Fertility Center (FKA SIRM Las Vegas). (Assigned nurse: Kimberly Ealy & Chelsy Foulk)
The staff is so amazing! Since I was going there for over a year, I created this amazing bond with them. If I hit a rough patch they were there with me and when I cried they cried. When it was time to celebrate they were right there with me showering me with hugs! I couldn't ask for anything more from them. They were so compassionate and understanding that no question ( regardless if I thought it was a little dumb ) went unanswered. They were even there when I needed help with my shots! I was able to call the nurse ( Kim ) at 8 AM on the dot and she walked me through hands shaking and all!! Then when I struggled with my additional shots my other nurse ( Chelsy ) was right there to help me with the additional shots and re-assured me that I was doing everything correct. The staff is always filled with positive energy and they always made every visit no matter how nerve racking it was .. smooth.
Describe your experience with Nevada Fertility Center (FKA SIRM Las Vegas).
I think I have said so much about how much I love the staff at the clinic. The only negative/weakness is the waiting for the call back for results. That would only be the hardest thing for me. Other than that I have not had any negative experiences. Even the receptionist is amazing!
Describe your experience with your monitoring appointments at Nevada Fertility Center (FKA SIRM Las Vegas).
I never had issues with any of my appointments. They were always set based as close to my availability as possible and I never had to wait too long to be taken to the back. There were a few times when they were running behind, but it was never bad to where I noticed anything.
Describe the costs associated with your care under Russell Foulk at Nevada Fertility Center (FKA SIRM Las Vegas).
I was a participant who won 50% off a frozen IVF cycle and I was also part of the CLARA clinical trial so my out of pocket expenses weren't too much. I had additional fees to consider such as using a donor sperm, but I think they weren't too bad. Total for my first round of Frozen IVF w/ the 50% discount came out to about $7000. Each egg retrieval was around $450 for the anesthesia which had to be paid separate and on the day of the retrieval and the monthly cost for storage is $60.
Describe Russell Foulk's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Nevada Fertility Center (FKA SIRM Las Vegas).
Due to the quality of my eggs always coming back as "abnormal" after PGS testing, Dr. Foulk was not against doing multiple embryo transfers. I also appreciated the fact that he understood my religious approach with going forward with a transfer that contained "abnormal" embryos.

Dr. Foulk was warm, friendly and kind. He was professional and showed competence in our treatment plan. He told me the next steps and validated my concerns and took time to answer questions...recommended IVF for tubal blockage after undergoing two HSGs...appreciated that he didn’t attempt iui or timed intercourse
[Utah Fertility Center] is competent and I trust them to know how to provide accurate treatment and care. Communication and following up on test results isn’t always timely. Sometimes the clinic feels understaffed as we were sent to the hospital for the labs...Some nurses were patient and compassionate and I appreciated it. I also felt that some forgot how vulnerable infertility and testing is.
How was your experience with Russell Foulk at Utah Fertility Center?
Dr. Foulk was warm, friendly and kind. He was professional and showed competence in our treatment plan. He told me the next steps and validated my concerns and took time to answer questions. Despite his role at the clini, he still showed he had the time and would do whatever it takes to help us conceive.
What's one piece of advice would you give a prospective patient of Russell Foulk at Utah Fertility Center?
You will be in the hands of a renowned fertility expert in the field but also someone that has personally experienced infertility. I knew that he understood all sides of struggles and would take the time to make sure we are comfortable with the next steps.
During treatment, were you treated like a number or a human with Russell Foulk at Utah Fertility Center?
My story and experience was remembered by my provider. I was surprised that he remembered things my partner and I said and brought it up in the follow up. He instilled hope and optimism in the process. He was patient to answer my questions and didn’t act rushed.
Describe the protocols Russell Foulk used in your cycles at Utah Fertility Center and their degree of success.
Dr. Foulk recommended IVF for tubal blockage after undergoing two HSGs. The second with Valium to ensure there wasn’t false results. I felt that he was thorough in making sure IVF would be absolutely necessary before recommending it. I also appreciated that he didn’t attempt iui or timed intercourse when it was apparent it would be a waste of time and money.
Describe your experience with the nursing staff at Utah Fertility Center.
Some nurses were patient and compassionate and I appreciated it. I also felt that some forgot how vulnerable infertility and testing is.
Describe your experience with Utah Fertility Center.
The clinic is competent and I trust them to know how to provide accurate treatment and care. Communication and following up on test results isn’t always timely. Sometimes the clinic feels understaffed as we were sent to the hospital for the labs because there wasn’t anyone there able to draw them. When we received the results of our genetic testing, it felt a little casual when it was somewhat startling to be given that news, but it seemed like the nursing stuff was just delivering it in a way that was in their level of understanding.
Describe the costs associated with your care under Russell Foulk at Utah Fertility Center.
Diagnostic testing was mostly covered under insurance, but projected IVF is anticipated to cost around 17,000 not including medication’s or genetic testing
What specific things went wrong at Utah Fertility Center?
  • Failed to call in prescriptions to pharmacy
  • Failed to call with results
Describe the specific things that went wrong at Utah Fertility Center.
The clinic never called to provide labs for initial bloodwork, nor was it uploaded to the portal for us to view it was indicated that they could manually upload it, but this never happened we learned about the blood work from our provider which was a couple weeks out from labs I also had to remind them to call in my Valium for my HSG because they didn’t remember to

How was your experience with Russell Foulk at Nevada Fertility Center (FKA SIRM Las Vegas)?
Dr. Foulk is kind, patient, and honest. He has made us feel safe in his presence from the get go! He answers all my questions without rushing me or making me feel silly. He is honest about risks and treatment and still helps us feel hope for our situation. He has helped us feel calm and happy during each of our procedures! We are so hopeful that he will help us bring all of our children into the world. So far we have one baby girl and are awaiting our pregnancy test for our second child!
What's one piece of advice would you give a prospective patient of Russell Foulk at Nevada Fertility Center (FKA SIRM Las Vegas)?
Ask him all your questions! Tell him all the feedback given to you from other healthcare providers. He will respond to everything and help you figure things out.
During treatment, were you treated like a number or a human with Russell Foulk at Nevada Fertility Center (FKA SIRM Las Vegas)?
Dr. Foulk and his team are angels. They have helped me feel so safe and cared for every time I have been in their office or in contact with them! They respond quickly, never making me feel like an inconvenience and with sincere attention. Every person in the office makes me feel like I am their patient and they know me even if I can’t remember if they have treated me before. Having been to other offices this was very important for me when choosing our care provider. We love NFC and would go to them even if we lived out of state.
Describe the protocols Russell Foulk used in your cycles at Nevada Fertility Center (FKA SIRM Las Vegas) and their degree of success.
We did an egg retrieval. I took gonal-F, menopur, cetrotide, prenatals and a baby aspirin. I produce I think 30 or more eggs? 24 that were a healthy size for retrieval I think? And then 15 ended up being healthy and frozen for FET. I’ve done two FETs with them now. Both times using progesterone in oil, estradiol valerate, doxycycline, methylprednisolone, progesterone suppository, lovenox, a valium, prenatals and baby aspirin.
Describe your experience with your nurse at Nevada Fertility Center (FKA SIRM Las Vegas). (Assigned nurse: Chelsy Foulk)
ANGELS. Absolutely angelic, calm, caring, and funny. I can’t say enough of this team. The journey of infertility is heartbreaking, scary, and at first unknown. This practice was not the first fertility practice we went to and the difference was the reason we switched to get our treatment and care from NFC. We love them. They answer every question with detail, patience, and care. In sad or scary situations they help you feel calm and hopeful. We love them!
Describe your experience with Nevada Fertility Center (FKA SIRM Las Vegas).
Strengths: quick and thorough responses, kindness and warmth, cleanliness, calmness, quick appointments but never rush me out, and a family like environment. I can’t think of any weaknesses, only thing I can think of is I wish they had a Henderson location! They are a little far for us.
Describe the costs associated with your care under Russell Foulk at Nevada Fertility Center (FKA SIRM Las Vegas).
3,000-3,300 for FET, roughly 12,000 for retrieval, and there were more costs I can’t remember. Medications and embryo testing.
Describe Russell Foulk's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Nevada Fertility Center (FKA SIRM Las Vegas).
For my specific situation Dr. Foulk and his team recommended a single embryo transfer. They were very thorough in explaining risks of multiples and made recommendations when I asked! We transferred 1 embryo our first transfer we ended in a full term successful pregnancy and the second transfer we just did and elected to implant two embryos. My comment is that their team is incredible, caring and treated us with respect. Though they stated the risks of multiples they still allowed us as a family to choose and I think that shows how much they value their patients and respect the impact having to do this process has on their family. I had consulted with several different practices and was surprised to hear they wouldn’t even allow me to make that choice on my own. Dr. Foulk’s office giving clear and honest information and then letting us make our own choices is invaluable.

Dr Foulk welcomes you with a warm smile. Explains everything based on your life and experiences...We transferred our sperm and embryos over there so all [he] did was a water ultrasound and transfer for us. We were so hopeful and so was he but unfortunately we had a chemical/miscarriage...So respectfully to the point they never really addressed our same sex marriage besides when they had to mention the donor sperm
We did our egg retrieval at a different clinic so all we did was transfer our embryos to [Nevada Fertility Center] . So only thing we had to pay for was a transfer and it was so much cheaper than what we were used to paying at another clinic,,,They didn’t treat my wife and I any different being LGBTQ...Every nurse was so kind and compassionate. Especially our nurse Sage...I don’t know how she has a life outside of the office because she was there any hour of the day that I needed her

Success w/ Doc Too early to know
He [Dr. Russell Foulk] is understanding, he wants to try new things, he is determined to help me start my family, and that's exactly what I have needed in a doctor. I do know how in demand he is and sometimes that shows through with how quickly he is in and out of appointments, but I wouldn't trade that for the type of care I receive from him. He gave me more answers in one appointment from looking at my history than 2 other doctors gave me by performing tests themselves. I appreciate him and trust that he is doing everything he can to help me in my situation
UFC is amazing...I know they are busy with so many patients so their biggest weakness can be response time with some things, however, overall I have had such a good experience with them. They're friendly, they keep me informed with my plan of care, and they always follow up with me after major procedures...Dr. Foulk is very busy and sees a lot of patients because he is so good at what he does. I appreciate so much how he remembers my situation, he remembers me, and is constantly following up on what's going on.
How was your experience with Russell Foulk at Utah Fertility Center?
Dr. Foulk is so in tune and so educated and it shows. I have switched doctors so many times because of my doctors not listening, not knowing what to do next, and being so out of touch with my situation, Dr. Foulk is different. He is understanding, he wants to try new things, he is determined to help me start my family, and that's exactly what I have needed in a doctor.
What's one piece of advice would you give a prospective patient of Russell Foulk at Utah Fertility Center?
Just be honest about how you're feeling with everything. Tell him exactly what you need and ask him lots of questions. Sometimes doctors forget that you don't know the details of what you're talking about, but if you ask him questions, he's so good about explaining things exactly how you can understand them.
During treatment, were you treated like a number or a human with Russell Foulk at Utah Fertility Center?
Dr. Foulk is very busy and sees a lot of patients because he is so good at what he does. I appreciate so much how he remembers my situation, he remembers me, and is constantly following up on what's going on. Sometimes it feels a bit rushed and I wish I could spend more time getting his thoughts and opinions, but overall, he does an amazing job keeping me informed with what's going on.
Describe your experience with your nurse at Utah Fertility Center. (Assigned nurse: Morgan Chappuis)
Morgan has been my coordinator through all of this and she has been amazing. I have bugged her with so many questions and concerns and she responds so quickly and with such grace. I appreciate having her along the ride with me so that I always have someone I can reach out to! I always get different nurses when I go into the office but they have all been amazing too!
Describe your experience with Utah Fertility Center.
UFC is amazing! I originally was there, then switched to a different clinic, did not have a good experience, and then transferred back to UFC. I know they are busy with so many patients so their biggest weakness can be response time with some things, however, overall I have had such a good experience with them. They're friendly, they keep me informed with my plan of care, and they always follow up with me after major procedures.
Describe the costs associated with your care under Russell Foulk at Utah Fertility Center.
It seems like every time I have an appointment that I actually see Dr. Foulk, there is some kind of cost. I do have really good insurance which is helpful, however, I at least have a co-pay when I see him. IVF costs are different as those are excessive no matter what, but since he's a specialist, the cost is a bit higher when I see him directly.

He [Dr. Foulk] always gave me options and let me decide what was next. He shared his recommendations but ultimately left me in control. He was proactive in testing when I miscarried. He recently moved around his schedule so he could preform a few tests himself rather than having his NP complete them. I truly believe he’s on my side and wants what’s best for me. He answers questions, explains why he wants to do things and let’s me decide.
Rayanne [nurse at Nevada Center for Reproductive Medicine] is the very best! I’ve seen her from day one and have nothing but great things to say about her. She’s kind, caring, and understanding. She’s also smart and organized and always remembers specific details and makes sure to call when she says she’s going to. I also saw Jennifer Lassi and Allie during my egg retrieval, they’re both kind and organized and answered all of my questions. Kara helped during transfers and was also great. Their nursing staff is beyond wonderful.

Schedule with them before you think it’s necessary so you don’t have to wait not see them.
During treatment, were you treated like a number or a human with Russell Foulk at Nevada Center for Reproductive Medicine?
I see the same doctor, nurse and assistants every time I go there. They know me by name and know my history as well as what is occurring currently with my treatments. I genuinely feel like they care and to me, that is invaluable.
Describe the protocols Russell Foulk used in your cycles at Nevada Center for Reproductive Medicine and their degree of success.
IUI with clomid (150mg/day cycle days 3-7) and a trigger shot 24 hours prior to iui from the first one. Baseline ultrasound was cycle day 1-3, trigger shot around cycle day 13. I did 6 total, the 3rd and 6th successful but both ended in miscarriages. Progesterone suppositories following iui. I took cytotec to induce those miscarriages. He then did recurrent loss testing, all which came back normal and a hysteroscopy after a saline sonogram. At that point I moved to IVF. Egg retrieval ended with 4 healthy embryos after genetic testing. I did a mock cycle with ERA that showed I needed progesterone early (170 hours prior to transfer). Both transfers went well. I took Valium prior to transfer and was on strict bed rest for 4 days following my FET. They drew labs to check progesterone and estrogen 6 days post transfer. I did PIO and estrogen shots. Ny first transfer was successful but ended in a miscarriage at 7 weeks. My second transfer failed. He then did a saline sonogram and HSG and found a newly blocked Fallopian tube and new polyp in my uterus. Next up is surgery to address those findings and then another transfer.

Describe your experience with Nevada Center for Reproductive Medicine.
They’re good at communicating and scheduling. I have had a wonderful experience, even though I haven’t achieved the results I’m hoping for-yet. The nurses are a definite highlight as is Dr. Foulk. I appreciate that they know who I am and are compassionate.
Describe the costs associated with your care under Russell Foulk at Nevada Center for Reproductive Medicine.
IUI with medications we’re about $1,000 per attempt. Egg retieval/genetic testing $10,000. Each transfer is $3,000
Describe Russell Foulk's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Nevada Center for Reproductive Medicine.
I chose to transfer one. Given my miscarriage history I felt better having multiple attempts.

When doctor Foulk has met with us he is always is present. He explains complex science in an understandable way. He is never rushed. He has a calm demeanor that is calming for us...He acted with urgency and suggested 3 rounds of IUI before performing IVF. He remembers our life situation when he talks with us and asked how my husband's grad school was going
[the Utah Fertility Center nurses] are very efficient and we felt in great care. They always called back same day to answer questions. We felt like they had our back and did great at showing us next steps...They are organized and very on time. They know your name and where you're headed. They label specimens at bedside and always confirm patient name and date of birth. Very impressive. They have RN's perform the IUI. Nurse practitioners perform the ultrasounds. That was my experience. They had seamless scheduling between the Pleasant Grove and Murray locations.
How was your experience with Russell Foulk at Utah Fertility Center?
I'm 34 and no other doctor noticed my AMH levels or took concern until doctor Foulk. He understood the low AMH meant my egg reserve was as if I was in my late 30s. He acted with urgency and suggested 3 rounds of IUI before performing IVF. He remembers our life situation when he talks with us and asked how my husband's grad school was going. We feel very confident in him as our doctor.
What's one piece of advice would you give a prospective patient of Russell Foulk at Utah Fertility Center?
Ask all your questions and trust his process.

When doctor Foulk has met with us he is always is present. He explains complex science in an understandable way. He is never rushed. He has a calm demeanor that is calming for us. He is very sought-after as a doctor and his schedule is very full. Sometimes appointment times have been changed last minute (day-of) because of his schedule.
Describe the protocols Russell Foulk used in your cycles at Utah Fertility Center and their degree of success.
Doctor Foulk recommended only 3 rounds of IUI because the statistics of getting pregnant after 3-4 rounds doesn't go up. He performed a hysteroscopy to remove 5-6 polyps. Two were on my cervix and he said they may have acted as sentinels preventing sperm from entering the uterus. We had our first egg retrieval today. I took Menopour and Gonal-F for 10 days and had 3 ultrasounds to monitor follicle growth. I took a ovulation preventing drug for two days before the trigger shot. I took Pregnyl as the trigger shot because during my first IUI the trigger shot made me itchy at the injection site. We are awaiting results from the lab for fertilization of eggs and survival of embryos. It was recommended I eat a low carb diet and drink lots of electrolytes to get the ovarys back to the normal size.

While we have only interacted with Dr. Foulk during our consult and any surgeries or retrievals, he has always been very kind, doesn't rush us during our appointments and is very thorough in explaining everything to us. He has great bedside manner and is consistently positive while being honest...He was very positive in the consult and gave us the time of day after a urologist told my husband he would never be able to have kids. Every single appointment has been amazing with him and his whole staff.
The [Nevada Fertility Center] staff is amazing, the client portal to access everything you need is so nice to have, and I appreciate them being on top of everything. The only weakness is the finance side as I felt there were a few things not explained to us as well, which caused more money out of our pocket than we expected

What's one piece of advice would you give a prospective patient of Russell Foulk at Nevada Fertility Center (FKA SIRM Las Vegas)?
There are no dumb questions! Ask every single question you have - I had personal questions related to my husband and I, as well as questions from my own research via Google and Facebook Groups.
During treatment, were you treated like a number or a human with Russell Foulk at Nevada Fertility Center (FKA SIRM Las Vegas)?
From the moment we met Dr. Foulk, he treated us like we had been patients forever. He was very positive in the consult and gave us the time of day after a urologist told my husband he would never be able to have kids. Every single appointment has been amazing with him and his whole staff.
Describe the protocols Russell Foulk used in your cycles at Nevada Fertility Center (FKA SIRM Las Vegas) and their degree of success.
My husband has azoospermia, so he underwent surgery to extract sperm, then I went on 150IU Follistim and 75IU Menopur. Week of retrieval I also added in 250 MCG Ganirelix and 1500IU Pregnyl / 81IU Lupron a day or two before egg retrieval. In the midst of my FET, currently, and doing Estradial and Progesterone.
Describe your experience with your nurse at Nevada Fertility Center (FKA SIRM Las Vegas). (Assigned nurse: Melissa Jordan)
The entire staff at my clinic is amazing. They truly care, are friendly, happy to answer questions, and always available. Melissa is so wonderful and honestly with how emotional and financially straining this process can be, it has been so wonderful to have access to her.

Describe the costs associated with your care under Russell Foulk at Nevada Fertility Center (FKA SIRM Las Vegas).
We were self-pay - $12,300 for Egg Retrieval Cycle, $3,300 for FET, $1100 for Sperm Extraction Surgery, $500 for each anesthesia treatment (2x for us), $4,000 in medication so far (and not done with my FET cycle). We will be close to $25,000.

Dr Foulk is very soft spoken and kind. We asked a lot of questions about our failed transfer and he took the time to address each question and make sure we knew our options. He listened to our concerns and gave us the best next steps to looking into additional testing...It was more affordable for us to have him do my hysteroscopy and polyp removal than to go through our insurance. His transfer prices are also fair and reasonable.
[the Nevada Fertility Center nurse] was always available to speak with and ask questions. I would call and leave her a message and be contacted within the hour...The clinic runs smoothly. From making the first appointment, to finances and the insurance team. the front desk [staff] is always happy and greets us with a smile. The insurance verification team is amazing..

We switched to Dr Foulk after going through an egg retrieval, ERA and hysteroscopy/polyp removal with another clinic. Due to the ERA being done at the old clinic, we decided to keep the natural stimulated transfer cycle like the old clinic had me do. Unfortunately it resulted in a failed transfer

Dr. Foulk has awesome, warm bedside manners. He is easy to talk to and answered all my questions and addressed my concerns. He rotates between 2 clinics - Nevada and also Utah, so he is quite busy and not always available for all my visits...He will be there to perform the critical procedures such as embryo transfer and if I specifically ask for a phone consult with him, the nurse can arrange for him to call
Rayanne is the nurse coordinator with egg donor patients [at NCRM]. Overall, she did a good job but because she is so busy, my emails or phone calls may not always be answered in a timely fashion...Clinic's strengths are definitely its doctors...New patient should also expect it to be busy and not all the nurses are warm and personable, although most are....Quite reasonable rates
How was your experience with Russell Foulk at Nevada Center for Reproductive Medicine?
Dr. Foulk is quite confident from the start even though I was a bit skeptical. I was referred to him by my coworker and I am so glad I sought his care. Very nice and warm demeanor. I always felt at ease with him and just wish I can see him on every visit, although he is busy and not always available.
What's one piece of advice would you give a prospective patient of Russell Foulk at Nevada Center for Reproductive Medicine?
Don't expect to see Dr. Foulk too often. As mentioned before, he works between 2 clinics in 2 different states, so expect to see the nurses and/or nurse practitioner most of the time. He will be there to perform the critical procedures such as embryo transfer and if I specifically ask for a phone consult with him, the nurse can arrange for him to call.
During treatment, were you treated like a number or a human with Russell Foulk at Nevada Center for Reproductive Medicine?
Dr. Foulk has awesome, warm bedside manners. He is easy to talk to and answered all my questions and addressed my concerns. He rotates between 2 clinics - Nevada and also Utah, so he is quite busy and not always available for all my visits. The majority of the staff are friendly and nice, but sometimes I do wait a while to get a reply to my email.
Describe the protocols Russell Foulk used in your cycles at Nevada Center for Reproductive Medicine and their degree of success.
Since I used donor eggs, it was relatively simple for me. I had a saline sonogram done since I had a history of 2 uterine polyps removed the year before. Results showed my lining was clear without polyps. Then a mock cycle (ERA) performed to determine best time to transfer based on hours on progesterone, and it showed I was receptive. My male partner got genetic screening. Once sperm was collected from my partner and fertilized with the eggs, we got 4 viable embryos which were sent for PGT testing. Then did an embryo transfer cycle consisting of estradiol IM, progesterone IM, Endometrin (progesterone vaginal), doxycyline for few days as an anti-bacterial, prednisone for few days as anti-inflammatory and baby aspirin to prevent any blood clotting.
Describe your experience with your nurse at Nevada Center for Reproductive Medicine. (Assigned nurse: Rayanne Marshall)
Rayanne is the nurse coordinator with egg donor patients. Overall, she did a good job but because she is so busy, my emails or phone calls may not always be answered in a timely fashion. It's tough since I want to know certain things right away. When I call the office esp the front desk, it's often a long wait before I'm helped. So I feel the office can improve in terms of organization and time managment
Describe your experience with Nevada Center for Reproductive Medicine.
Clinic's strengths are definitely its doctors. I hear Dr. Whitten (the other doctor in the clinic) is also a great, compassionate doctor. The front office and waiting area looks warm and clean. However, my partner did tell me the room he went to for sperm collection does need improvement- it's a bit messy and not too inviting. New patient should also expect it to be busy and not all the nurses are warm and personable, although most are. Since it's in Nevada, the rates and fees are definitely an improvement over California but you still get the same high quality care if not better from Dr. Foulk.
Describe the costs associated with your care under Russell Foulk at Nevada Center for Reproductive Medicine.
Quite reasonable rates compared to the clinic I went to in California; cost less than half of what I paid in California for similar services. I used frozen donor eggs and went thru insemination with my male partner's sperm, along with PGT with the retrieved embryos. Then did an embryo transfer cycle which luckily was a success on 1st try.
Describe Russell Foulk's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Nevada Center for Reproductive Medicine.
Dr. Foulk is open to multiple embryo transfer but he advised me to do single transfer since i'ts less risky esp with my extensive medical history and advanced age. Having twins will carry more risks in pregnancy and it will complicate my history even more
What specific things went wrong at Nevada Center for Reproductive Medicine?
  • Lost paperwork
  • Failed to call with results
  • Provided conflicting information
Describe the specific things that went wrong at Nevada Center for Reproductive Medicine.
It was nothing major, but like I mentioned before the nurse sometimes did not email me results when I asked for it (ie- progesterone or estrogen levels). Not enough attention to detail is really the issue. I felt like I had to initiate a lot of things to move things along. Overall though, things were resolved in the end.

Dr. Foulk was warm yet informative. He told me what was happening with my body but it was never like it was a bad thing. He always wanted to find a solution...rather than focus on negative it was always here is what we should do to accomplish our goal...It's a scary process but he'll find your answers. My favorite thing he says if when we make your family not if. He's that confident... Every time it was genuine care, concern. Explain everything as we approached. It really eased my mind.
The [Nevada Fertility Center] clinics strengths is their compassion and commitment to their patients. Every person that comes in wants a family and they want to help them accomplish that. They want to figure out why it's not happening naturally and then come up with solutions. I honestly cannot think of a single weakness...Every staff member we encountered was so friendly and you could tell they wanted us to succeed in making a family. Kim specifically knew our case and became a friend during it all.

I had an hsg done and it determined I had one tube blocked. And I also have low egg egg count. So he put me on clomid and I would go in on the first day of my cycle to see where my follicles were and which side I would ovulate on. If my follicle count was low or I ovulated on the blocked side we would wait until the next month to try for the IUi. He never wanted me to go through with the procedure if it wasn't the best odds for my body.

[Dr. Russell Foulk] was so nice...had a good bedside manner...treated me well and addressed any questions I had with patience. If I was concerned he always would get back to me...worked with my husband with his sperm retrieval...used a low dose birth control in the beginning...Constant labs to measure where I was.
[At Utah Fertility Center] They would shut off phones right at 5pm or during lunch hour. I always had to make sure to make time during my work schedule to get a hold of them...would tell me they would call after 5 and often didn’t get back to me until way after hours...had to switch a coordinator because I was having a bad experience with the first one with her lack of communication
How was your experience with Russell Foulk at Utah Fertility Center?
He was so nice. I was very awkward and he had a good bedside manner. He treated me well and addressed any questions I had with patience. If I was concerned he always would get back to me. He also worked with my husband with his sperm retrieval.
What's one piece of advice would you give a prospective patient of Russell Foulk at Utah Fertility Center?
Ask questions if you have any and you’ll know everything of what to expect. If you don’t have that, ask for a different coordinator.
During treatment, were you treated like a number or a human with Russell Foulk at Utah Fertility Center?
He always treated me with respect and addressed my concerns.
Describe the protocols Russell Foulk used in your cycles at Utah Fertility Center and their degree of success.
He used a low dose birth control in the beginning, and told me why we were doing that. As an infertility patient I felt that was ironic but it made sense when he explained the reasoning. Constant labs to measure where I was.
Describe your experience with the nursing staff at Utah Fertility Center.
They would tell me they would call after 5 and often didn’t get back to me until way after hours and I didn’t have a way to contact them which was very frustrating. I actually had to switch a coordinator because I was having a bad experience with the first one with her lack of communication
Describe your experience with Utah Fertility Center.
They would shut off phones right at 5pm or during lunch hour. I always had to make sure to make time during my work schedule to get a hold of them. They did well with texting me when I could come in-this was during the pandemic (summer of 2020).
Describe the costs associated with your care under Russell Foulk at Utah Fertility Center.
I think all together was $17,000 for IVf and 1,500-1800 for sperm retrieval
Describe Russell Foulk's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Utah Fertility Center.
I chose to transfer 2 embryos and he warned me of the risks but was willing to try it out with our situation.
What specific things went wrong at Utah Fertility Center?
  • Lost appointments
  • Failed to call with results
  • Failed to inform you of changes in protocol
  • Failed to convey critical information
Describe the specific things that went wrong at Utah Fertility Center.
I had to have a scheduling coordinator change in the middle of doing an ivf round to retrieve eggs and it was stressful because it was my first time and I wasn’t getting the care I expected but once I got a new coordinator everything was much smoother and I was more at ease than stressed.

After a failed IUI with another provider, he [Dr. Foulk] met with me on a Sunday and reassured me he had a plan and I would be pregnant in the coming few months. He knew exactly what protocol to use and didn’t send me for unnecessary testing. He considered our willingness to be aggressive. We didn’t want a conservative approach and he was willing to comply. I loved that the most. He didn’t want to waste time and he understood my impatience after years of TTC.
Dr. Foulk and his staff [at Nevada Fertility Center] took to me quickly. By the 2nd appointment, everyone knew my name and my story and by month 3 of treatment, I considered them friends. I was tended to immediately after each concern and was given after hour contact info which I used very frequently to ease my mind...[the nurses] took care of me, gave me direction and motivation. They texted me after hours and celebrated each victory with me. The staff was never too busy for a question, a repeat blood draw (I had 12 in 20 days to check on levels) or adjusting meds. All my questions to Dr. Foulk’s NP, Kim Carter, we’re immediately directed to the doctor when he was out of state (he has a few clinics in the nation) and I had an answer in minutes in most cases. All lab results were provided within 4 hrs from blood draw (this is record timing). Just feel so lucky I experienced this with them.

Describe your experience with Nevada Fertility Center (FKA SIRM Las Vegas).
2 “weaknesses” 1. Front desk doesn’t always answer phone promptly (I think they’re phone system is being upgraded now). 2. Dr. Foulk is at practice only 1 week/month so if you HAVE to see the doc, it may be scheduled further out than you’d like. Strengths: Staff are compassionate and the most educated group of girls. Dr. Foulk is so up-to-date that I didn’t feel like “he could be doing more”. Availability from early morning hours which is convenient so you don’t have to take time from work or go after hours.

He [Dr. Foulk] is the most caring doctor. I have had some really hard times and he always delivered the hard news with so much empathy. I am proud to be one of his patients. He is very organized and made me feel like I was his only patient. I even remember one time he made sure he called me even though it was 8 pm. He wanted to make sure I was okay and that my questions were answered.
The nursing staff at Utah Fertility Center is amazing! They treat you with so much love and compassion. Lacey was our IVF coordinator and she was so helpful through everything. She made sure our meds were right and she spent countless hours helping us. Every time I see her she greets me with a smile and makes my husband and I feel at ease. The nurses are an extension of they doctors at UFC and they take such good care of their patients.

I would choose him over and over again. He is amazing and he will not give up on you. He is knowledgeable and he will take great care of you!!
During treatment, were you treated like a number or a human with Russell Foulk at Utah Fertility Center?
UFC and the doctors there are so caring! They want to help you reach your goals of having children. They do everything in their power to help you get there. They are there for the low of lows and I hope they will be there for the high of highs! They care so much about you and want to help you get pregnant. They have tremendous bedside manner.
Describe the protocols Russell Foulk used in your cycles at Utah Fertility Center and their degree of success.
I started off by doing shots and medicine for an egg retrieval. After a few weeks we realized that I was not producing follicles like I should so he cancelled my egg retrieval. The second time I did shots and I also did testosterone gel on my arm. I did produce follicles. We were hoping for 6-8 eggs and when I had my egg retrieval I had 3 eggs. 2 of them made it to day 4 and 1 made it. My embryo was tested and came back abnormal meaning it wouldn’t implant. I then decided to do some IUIs and I did 2 back to back with clomid and femara and a trigger shot and that has led me to right now. We are meeting with them tomorrow to figure out our next steps. To say this has been a long road would be an understatement but we will do anything to get our babies here safely.

Utah Fertility Center is amazing at communicating with their patients. They are organized and they get appointments scheduled quickly. They are always on schedule and you can count on them to not keep you waiting. The front desk is so inviting and they know you by name as you walk in. You feel loved there. I honestly cannot think of weaknesses. I have had nothing but amazing care at their clinic.
Describe the costs associated with your care under Russell Foulk at Utah Fertility Center.
IVF-$7500 in medicines and then paying for anesthesia for egg retrieval and money for testing of the embryos. IUI-we have had 2 of them and paid about 1,000 for each one. We have also paid for progesterone suppositories several times which cost a couple hundred each time.

Being a patient of Dr. Foulk I always knew what my treatment was going to entail, he explained everything in detail and to where we could understand. Dr. Foulk was there during the most difficult times of my life (my miscarriages), he would personally call to check up on me. Dr. Foulk is so caring and compassionate and I knew that he wasn't ready to give up on making my dream of becoming a mom true. He donated quite a bit of his time
The whole clinical staff [Utah Fertility Center] are always so helpful and thorough. They genuinely care about you and the reason you are there. It is a clean environment. They explain everything in detail. And I always know that they have your best interest at heart. They are amazing and I wouldn't want to go anywhere else.... Danica is just as amazing as Dr. Foulk. She is very compassionate, caring, and kind. I felt that I could ask her anything and she always help no matter what.

Dr. Foulk is so caring and compassionate and I knew that he wasn't ready to give up on making my dream of becoming a mom true. He donated quite a bit of his time as I couldn't always afford the treatments. I lost my last embryo after a negative pregnancy test so we're still trying to figure out how to move forward but I truly know that Dr. Foulk is not ready to give up.

In the beginning I had insurance that helped pay for my treatments so I'm unsure of the actual cost. After my husband switched companies we lost this benefits but after my first miscarriage Dr. Foulk and the UFC staff helped with the costs of my second egg retrieval and transfers.
Describe Russell Foulk's approach to eSET (elective single embryo transfer) vs. multiple embryo transfer at Utah Fertility Center.
My first transfer we transferred 2 embryos which ended up in a miscarriage after that Dr. Foulk strongly suggested only 1 embryo transfer in the following transfers.

I met with Dr Foulk once at the beginning for a consult. I found his explanations and answers to be straightforward and left feeling reassured and confident about the next steps to take. However, the following IUI cycles were lead by his PA and I didn’t hear or meet with him again until 3 rounds failed and they wanted to progress to IVF...I have confirmed tubal blockage on my left side and I feel like during my IUI treatments I received conflicting information...
[Nurse at Utah Fertility Center] pretty much runs the St George location while the Dr’s come down occasionally for consults and once a quarter for IVF treatments...extremely caring which I found very helpful during the stressful and often sad fertility process...biggest complaint is making payments is confusing and a little overly complicated. The St George office doesn’t accept payment and refers you to their main Pleasant Grove office.

The staff in the St George office are very caring and kind. They have been helpful in answering my questions and calming my nerves during the process.
Describe the protocols Russell Foulk used in your cycles at Utah Fertility Center and their degree of success.
I underwent three IUI cycles. I would call on the first day of my cycle and set up a baseline ultrasound. After the ultrasound I was on chlomid for 5 days. (1st round- 100mg; 2nd & 3rd round- 150 mg) I would have a monitoring ultrasound 7-8 days after the baseline. Depending upon the progress of the follicles I would either schedule a follow up monitoring ultrasound or be administered a trigger shot for an IUI 24 hours later. All three IUI cycles failed.
Describe your experience with your nurse at Utah Fertility Center. (Assigned nurse: Michelle Macfarlane)
Michelle Macfarlane pretty much runs the St George location while the Dr’s come down occasionally for consults and once a quarter for IVF treatments. She is extremely caring which I found very helpful during the stressful and often sad fertility process. She was great to answer any questions we had and helped us work through any confusion in the process. The rest of the St George location’s staff were caring and extremely professional as well.
Describe your experience with Utah Fertility Center.
My biggest complaint is making payments is confusing and a little overly complicated. The St George office doesn’t accept payment and refers you to their main Pleasant Grove office. Their policy is that payment is due at the beginning of a cycle and you are to call the Pleasant Grove office to coordinate that payment at the time of your baseline ultrasound. However, when I did this my first cycle I was transferred to the billing where I only reached voicemail. Upon finally connecting to billing they said they were unaware that I would be starting a cycle and they had yet to run the information through my insurance. They instructed me that if I was to owe anything I would be billed. Which seemed contradictory to their payment policy. The second cycle I called again at the beginning and upon being transferred to billing once again only reached voicemail and was never contacted so off of the previous instruction I just waiting to receive a bill from them. On my third cycle, the same day as my baseline ultrasound I received a call from the Pleasant Grove office requesting payment for the cycle. We are looking into starting IVF through the clinic and the financial advisor is out of the Pleasant Grove office and I hope it goes a little more smoothly then my previous experience.
Describe the costs associated with your care under Russell Foulk at Utah Fertility Center.
IUI was $875 a round not including medication costs. We have been told to estimate $20,000 for an IVF cycle. With the clinic costing $9-11k, meds costing $3-5k, and genetic testing costing $3-5k.
What specific things went wrong at Utah Fertility Center?
  • Provided conflicting information
Describe the specific things that went wrong at Utah Fertility Center.
I have confirmed tubal blockage on my left side and I feel like during my IUI treatments I received conflicting info on if the “better” follicles were on the left side if there was even a good chance of them making it through the blockage.

Dr. Foulk is always willing to give you a call after every treatment no matter the outcome. To touch bases again and to talk about next options or just give encouraging words...treats you like family. He is clear with his explanations, goals for your treatments, and diagnosis...He is extremely personable and understands the struggles that we are going through...We started the cycle with planning to do timed intercourse, but ended up wanting to do IUI. Since my follicles werent maturing, we cancelled the cycle and I then took provera to start my period and try again a different treatment plan. The next month I started taking metformin to help with my insulin resistant PCOS. We did letrozole and injectibles. My follicles finally grew, but my IUI was unsuccessful. Due to the pandemic, we took a month off and will go back when my next period starts.
I spent more on medication than I did on the actual IUI [at Nevada Fertility Center]. I spent $1,100 on my IUI. I spent almost $2,000 on the clomid, letrozole, and follistims...The staff is all so friendly and accommodating. They treat you like family. The are extremely understanding and will help with whatever you need help with. I am a teacher and I teach during the hours that they are open, they are always willing to move things around to help you get in for an appointment. They are thorough with what you need to do. They do step by step instructions for injectibles and give you a calendar so you know what to do and when to do it. They treat you like you matter.

Dr. Foulk and his staff took the the time I needed and followed my desires during this whole process...was kind and knowledgeable. He explains things clearly but doesn’t forget the huge emotional side of all these treatments and appointments. While he works at several offices, he was always able to fit me in or call me after my check up appointments to go over results or how I was. He was willing to listen to my suggestions and questions during the treatments and made adjustments based on what I wanted or needed. He is amazing.
The nursing staff at this office [Utah Fertility Center] became like family to me. They were always kind and understanding. I felt welcome and at ease asking any questions I had...The clinic was organized and friendly. It was easy to get appointments scheduled and I never felt rushed or like an annoyance. The billing department was always friendly and helpful as well. I particularly liked the phlebotomists. They were kind and easy going and made the sometimes daily blood draws easier.

During treatment, were you treated like a number or a human with Russell Foulk at Utah Fertility Center?
Dr. Foulk and his staff took the the time I needed and followed my desires during this whole process. I felt like family and even considered working there because I loved the office so much. They cared about my emotional and physical well-being.
Describe your experience with the nursing staff at Utah Fertility Center.
The nursing staff at this office became like family to me. They were always kind and understanding. I felt welcome and at ease asking any questions I had. They cried with me when we found no heartbeat, during my d and c they comforted me and helped me when I was super emotional. They would come out of their way to say hi when I had an appointment even if they weren’t seeing me that day. They knew me and not just my file or history.
Describe your experience with Utah Fertility Center.
The clinic was organized and friendly. It was easy to get appointments scheduled and I never felt rushed or like an annoyance. The billing department was always friendly and helpful as well. I particularly liked the phlebotomists. They were kind and easy going and made the sometimes daily blood draws easier.

I definitely felt he [Dr. Foulk] reviewed our specifics carefully and looked to see what he could do to make a difference this time ... He took the time to explain his thinking and didn't push us into anything. He showed empathy and tried to do what he could to work around our schedule & budget. On cycle day he sat and talked with me after the procedure longer than necessary.
Recently billing has been less transparent and hidden charges have been billed but generally they are great ... Everyone [at Sher Fertility of LV] is well-organized, friendly, & attentive. It's easy to find and while you're there for treatment there's plenty to do in LV to keep your mind occupied. Pricing is generally transparent which is helpful and they are just so easy to work with.
How was your experience with Russell Foulk at Nevada Fertility Center (FKA SIRM Las Vegas)?
Dr. Foulk's communication style & and the details and care given to us put us at ease even just over the phone before we met. In person he was just as caring and you didn't feel like he was rushing through his time with you. The only thing I didn't like is when we arrived for ERA biopsy it was with another doctor. But he did perform the egg retrieval and was great all along the way.oh also I ended up getting billed for extra charges outside cycle fee that we’re not previously discussed or disclosed to me. This didn’t happen with previous doctors at same clinic and was very frustrating. That still isn’t resolved.
What's one piece of advice would you give a prospective patient of Russell Foulk at Nevada Fertility Center (FKA SIRM Las Vegas)?
Ask if he will be hands-on to do your procedures himself Ask about ALL charges as you go

He definitely took the time to review our case thoroughly before getting on the phone. He took the time to explain his thinking and didn't push us into anything. He showed empathy and tried to do what he could to work around our schedule & budget. On cycle day he sat and talked with me after the procedure longer than necessary. I could feel he cared about my experience and my comfort. He really makes you feel he cares about YOU and you’re in good hands. I’m so glad we tried one more cycle with him!
Describe the protocols Russell Foulk used in your cycles at Nevada Fertility Center (FKA SIRM Las Vegas) and their degree of success.
Protocol would be same as I had previously (Lupron based IVF) but with ERA to adjust transfer window accordingly. For the transfer he suggested adding Neupogen to help the uterus be receptive to the embryo since we've had trouble with implantation in the past. I definitely felt he reviewed our specifics carefully and looked to see what he could do to make a difference this time
Describe your experience with your nurse at Nevada Fertility Center (FKA SIRM Las Vegas). (Assigned nurse: Kimberly for retrieval / Dayana for transfer)
Both nurses were very communicative, quick to respond, answered all of my questions, and just kind all along the way. They never got rattled when I got stressed out and asked for help out of stress. They were always available and helpful and compassionate. Both the nurses and Dr. Foulk were fantastic.

When you have an appointment with him [Dr. Foulk] ask a lot of questions, because you may not see him as often as you think. He’s also honest, so don’t expect sugar coating...We did a mock cycle, to make sure we had the best chance of sticking. It did turn out I needed more progesterone prior to transfer. Eventually we had our FET, unfortunately it also failed. After my failed IVF we met with dr Faulk again, and he explained that we unfortunately just had back luck with the embryo not sticking.
I always felt like I was in great hands with the PA and nurses [at Nevada Center for Reproductive Medicine]. They were always friendly, informative, helpful and compassionate...IVF without meds and PGS was about $15k...They’re really great at being present with patients, easy to get in touch with and always helpful. They always communicated what was happening or what would happen next, easy to talk to about concerns and just overall compassionate. I’m not sure what weaknesses they have, I don’t think there’s anything I can think of right now!
How was your experience with Russell Foulk at Nevada Center for Reproductive Medicine?
I love Dr. Faulk and think he’s overall a good Dr. for me. He was always honest and friendly. My only issue is that he also works in UT, so I didn’t see him as often. But I never worried about the care I was getting.

He was always honest about what my outcome could be, and was a nice, thoughtful doctor.
Describe the protocols Russell Foulk used in your cycles at Nevada Center for Reproductive Medicine and their degree of success.
The first 7 cycles I had were timed with clomid or Femera. I think I changed to Femara on the 4th month, it worked better with my body. After 7 cycles we moved to IUI, with femera. The 3rd cycle was cancelled due to having no follicles, or missing ovulation. I had 4 total. All my timed and IUI cycles failed, with no positive test. A few months later we started IVF, I don’t remember how much of each medication I took for any cycle. Our cycle only gave us one embryo. We did PGS testing, mostly because we decided to before the retrieval, thinking we’d have more than 1 day 6 embryo. It was normal. We did a mock cycle, to make sure we had the best chance of sticking. It did turn out I needed more progesterone prior to transfer. Eventually we had our FET, unfortunately it also failed. After my failed IVF we met with dr Faulk again, and he explained that we unfortunately just had back luck with the embryo not sticking. Which we understood. He also said I don’t have the best quality eggs, and the amount of medication we did was high for someone my age and size, should have had a better outcome. We did plan to do another round, with more medication and a different approach, but we have not yet done so.

For my initial visit with Dr. Foulk, be told us IVF as "anything else would be a waist of your time and money." It was completely overwhelming.... I wanted to put two embryos in and Dr. Foulk had no reservations about letting me do so, though he always stated that it was a high risk pregnancy. I am tall, so he did say that I had "plenty of space between my rib cage and hip bone and should be able to carry twins just fine."
I was told, the nurse would be my friend through the entire process. I went home and after 3 times trying to call the clinic, the nurse FINALLY got back to me. I had an open conversation with her about how that wasn't going to fly with me. After that, she was MUCH better. I was frustrated because in the time I didn't hear back from the clinic, I wasn't able to start and then the Dr. was out of town for an entire month. It ended up being perfect timing in the end. But, initially, I was overwhelmed and felt like a number, but during that actually IVF cycle, things were much better!
How was your experience with Russell Foulk at Utah Fertility Center?
Dr. Foulk met with my husband and I on our initial visit. I had all my records ready for him. He was very candid about our need to do IVF. That was pretty much our only option. He explained why and in terms we could understand. He sensed I was feeling overwhelmed (I'm guessing the tears gave it away) and he addressed that in our meeting. Looking back, I am so grateful he was candid and direct. When I had a multitude of questions, the nurse asked if I wanted to speak to Dr. Foulk on the phone. Seriously? Absolutely. I asked him all of my questions and he answered each one of them professionally and with clarity. During the procedures (egg retrieval and frozen embryo transfer) he was efficient and clear in explaining what was to happen which put me at ease.
What's one piece of advice would you give a prospective patient of Russell Foulk at Utah Fertility Center?
You will have to wait for him initially. He is very booked and very busy. I called in January and the earliest appointment I could get with him would be March. But it was worth the wait!!!!
During treatment, were you treated like a number or a human with Russell Foulk at Utah Fertility Center?
For my initial visit with Dr. Foulk, be told us IVF as "anything else would be a waist of your time and money." It was completely overwhelming. I'm thinking, "The most expensive and invasive approach?" And then HOW could we ever pay for this? I was then given a packet of information about IVF, a prescription (that would start my period) and the direction to set an appointment for a water ultrasound. Mind you, I had already paid over $1,000 for transvaginal ultrasound at the hospital. Couldn't they just use those results. I left with a serious of questions. So. Many. Questions. I was told, the nurse would be my friend through the entire process. I went home and after 3 times trying to call the clinic, the nurse FINALLY got back to me. I had an open conversation with her about how that wasn't going to fly with me. After that, she was MUCH better. Describe the protocols Russell Foulk used in your cycles at Utah Fertility Center and their degree of success.
I had to do baseline bloodwork and a water ultrasound. I found that my prolactin levels were high. The bromocriptine (parlodel) meds taken to lower that made me sick so the Dr. recommended I take them vaginally. I also needed a hysteroscopy to remove polyps on my uterus before I could start the IVF cycle. For the IVF cycle, after the baseline ultrasound, I took Gonal F and Menopur prior to the egg retrieval. Based on the growth of the leading follicle, my egg retrieval was two days later than what was on the initial calendar. I also took Cetrotide prior to the retrieval. For my partner, he took a z pack (as did I) a few weeks prior to the cycle and Cipro the day before egg retrieval. For the frozen embryo transfer, I took progesterone injection and suppositories daily and injections of estradiol twice a week until the 10 week mark when a graduated to a regular OB.
Describe your experience with your nurse at Utah Fertility Center. (Assigned nurse: Lacey)
Initially, I had a hard time getting anyone to call me back. I had to call three times and get upset before I had my questions answered prior to the IVF cycle. Once I was actually in motion to move towards IVF my phone calls and e-mails were returned promptly.
Describe your experience with Utah Fertility Center.
Strengths - they have a grant you can apply for if you have a combined income that is less than a certain amount; I could do blood work at a different location (in Murray) closer to my home. I didn't have to go to Pleasant Grove for every blood draw and ultrasound. Weaknesses - the meeting with the financial specialist really wasn't much help at all in that they didn't have many promising resources for help; They may encourage based on scheduling that another Dr. do certain procedures. I had to be VERY clear that I wanted Dr. Foulk to do the hysteroscopy, egg retrieval and frozen embryo transfer. If they aren't communicating with you in a timely manner, say so!
Describe the costs associated with your care under Russell Foulk at Utah Fertility Center.
IVF $8,715.00 Genetic Testing $1,050.00 IVF Meds $3,719.40 FET - Frozen Embryo Transfer $2,175.00 Hysteroscopy $491.90 Anesthesia $400.00

Dr Foulk is compassionate, empathetic, knowledgeable, and talked to us like he understands our pain but gave us so much hope even as we hit some speed bumps...Our first official IUI was a chemical pregnancy and he called to check in on me and schedule a follow up in a couple weeks. When we met he was genuinely concerned for my emotional state, gave me reassurance, changed our IUI plan slightly...Our frozen cycle was successful and he genuinely celebrated with us. He always kept in contact, seemed to recognize us and enioy us as patients.
Clean homey clinic [Utah Fertility Center], staff is all compassionate, hopeful, and understanding. I felt silly walking in at first and quickly realized we're all there for a reason and the staff is there to support you and knows what you're going through. Even in the hard times, they show so much love and understanding...IVF fresh cycle including retrieval was around $13,000 (before meds from an outside pharmacy). Frozen cycle before meds was less than $4,000
How was your experience with Russell Foulk at Utah Fertility Center?
Dr Foulk listened to our history, let me know we'd start tests right away (and we did). We met again soon after tests and he told us exactly what was found and our course of action. Our first IUI turned into timed intercourse and he told us about that right away. Our first official IUI was a chemical pregnancy and he called to check in on me and schedule a follow up in a couple weeks. When we met he was genuinely concerned for my emotional state, gave me reassurance, changed our IUI plan slightly. We had 3 unsuccessful IUI's so he met with us again telling us medically where we stood and that IVF would be the next option. He was so great at explaining the entire retrieval and transfer process for both fresh and frozen. Our frozen cycle was successful and he genuinely celebrated with us. He always kept in contact, seemed to recognize us and enioy us as patients.

Because of my previous miscarriages, I wanted to transfer 2 hoping at least one would make it. My doctor [Dr. Foulk] was happy to let me transfer 2...has always been so kind and patient. I feel like he knows us and shares in our excitement with a positive pregnancy test and then shared in our loss when I miscarried...After seeing another RE that didn’t listen or respect us, coming to Dr Foulk was a breath of fresh air. He immediately was able to diagnose me after 5 years of “unexplained infertility” and set a course of action to get us going in our treatments.
Scheduling the IUIs [at Utah Fertility Center] was always really hard. Once I started IVF, I felt like I had vip status and they were way more responsive. It was also helpful to have a dedicated coordinator at that point instead of having to leave a message with the nursing staff and waiting for them to return my calls... Certotide for 4 days before retrieval and Prognyl hcg trigger shot. Resulted in 33 eggs at retrieval, 19 fertilized, ended with 7 5-day blasts. Endometrin 3x day and then 2 ml Progesterin in Oil once pregnancy was confirmed.

He [Dr. Russell Foulk] took the time to explain everything in a way we understood. And he answered any and all questions we had...he has so many clinics to go to that we only saw him once...After that the nurses handled everything and we didn’t see or talk to him again...Don’t expect him to do any of your procedures...I took 50 units of gonal-f a day for about 2 weeks. That produced very little change in my ovaries. And my lining started to thin out so they canceled that cycle.
During both of my cycles [at Utah Fertility Center] I was going into their office 1-2 times a week for vaginal ultrasounds to check my follicles progress up until my iui. The nurse did everything and I never saw or talked to dr. Foulk after the consultation...She [care coordinator] double checked with dr. Foulk before continuing or changing my treatment. And the rest of the nursing staff was great they were very professional and friendly. And also very compassionate.

He is a great doctor but he has so many clinics to go to that we only saw him once when we first got our constellation. After that the nurses handled everything and we didn’t see or talk to him again.

Describe the protocols Russell Foulk used in your cycles at Utah Fertility Center and their degree of success.
He started me on progesterone to induce a period. And it went as planned. He then had me take clomid (100mg) for a few days, that produced zero results so they started me on injectables. I took 50 units of gonal-f a day for about 2 weeks. That produced very little change in my ovaries. And my lining started to thin out so they canceled that cycle. The second cycle we tried 2.5 mg of femara for 15 days that also produced nothing. So we started out at 75 units of gonad-f and it barely started to work so they upped my dosage to 150 units. That did start my ovaries producing follicles but very slowly so they again upped the dosage to 225 units. This produced 2 perfect follicles, and I was scheduled my iui. I took a trigger shot 36 hours before the iui, and they did 1 insemination. This resulted in a negative pregnancy test. During both of my cycles I was going into their office 1-2 times a week for vaginal ultrasounds to check my follicles progress up until my iui. The nurse did everything and I never saw or talked to dr. Foulk after the consultation.
Describe your experience with your nurse at Utah Fertility Center. (Assigned nurse: Michelle Mcfarland )
My care coordinator Michelle Mcfarland was very knowledgeable and friendly. She double checked with dr. Foulk before continuing or changing my treatment. And the rest of the nursing staff was great they were very professional and friendly. And also very compassionate.

The strengths of this clinic is that they get things done in a timely manner. The weaknesses would have to be getting back in contact with you over the phone, sometimes it took days to hear back from them.
Describe the costs associated with your care under Russell Foulk at Utah Fertility Center.
iui: 875.00 That included, ultrasounds and physician monitoring 525.00, sperm preparation 165.00, And insemination 185.00 My injectable meds were between $405.00-$900.00 depending on the dosage. So all in all 1 cycle was around $2,000.00 with everything.

I had test results that took way longer than was said to get the results back. And several times they forgot to call the pharmacy to get my fertility drugs sent to me. I had to call the pharmacy myself and have them call the fertility clinic to verify my prescription that I needed shipped to me the next day.
