How was your experience with Sara Vaughn at Spring Fertility Silicon Valley?
Dr. Vaughn was our assigned doctor when we started our IVF journey back in Oct/Nov 2021. Right from our very first visit she was very informational, friendly and empathetic. She got our requirements during our first meeting, which was immediate pregnancy. She was very attentive to our needs during every visit and gave us excellent care. We have a healthy, 2 week old, newborn now and it is all because of Dr. Vaughn's care, expertise and dexterity with embryo transfer earlier in 2022. Dr. Sara Vaughn is one of the best in the IVF field in Southbay.
What's one piece of advice would you give a prospective patient of Sara Vaughn at Spring Fertility Silicon Valley?
She is knowledgeable and thorough. Please trust in the process and Dr. Vaughn will deliver. :)

Dr. Vaughn was always educating us about the IVF procedure, also suggested us to go through another extraction cycle but when we insisted that we would like to get pregnant asap and think about next cycle in the future, she obliged with utmost care and empathy. She was always there to help us in every which way possible through the long IVF process.
Describe your experience with your nurse at Spring Fertility Silicon Valley. (Assigned nurse: Gigi)
The nursing staff were our point of contact for any of our questions. Spring Silicon Valley's nursing staff and especially Dr. Vaughn's team of nurses and care team were highly knowledgeable and always ready to help. They suggested us to go with Alto Pharmacy and it was the best decision that we made. Alto was a breeze to deal with; along with same day deliveries for critical injections that we need for IVF priming.

The clinic was in pristine condition with state-of-art equipment and facility. The front desk staff is very courteous and always available during business hours. The whole process of IVF (egg extraction) and e-SET was dealt with very carefully with utmost focus on cleanliness, sterility of the entire extraction+transfer regions of the clinic.

Dr. Vaughn was there for me ever step but also was open & honest with me. Over the many rounds she knew I always wanted to talk next steps plan b & she would be honest when we weren’t there yet & when it was time.

This would be a novel if I went through all my cycles. Long story short, Dr. Vaughn & I used each cycle as a learning opportunity to see what my body responded to & what didn’t work. What I appreciated is that each cycle was it’s own no cookie cutter cycle each was based on where my body was at at that time & would try something new like adding supplements to help. Dr Vaughn helped me through each round every step of the way.

For both transfers of my day 3 embryos we transferred 2 the first time & 3 the second time. Due to the fact I had bad egg quality the chances of even one taking were low enough it was worth transferring more than one. Dr. Vaughn & I talked a lot about this before making the decision & in the end she would share what she suggests but it was always my decision

She was hyper focused on our religious beliefs...After getting cleared by the hematologist and having done all the labs, we thought we were moving forward. So I was shocked to get a call from a patient rep (not even Dr. Vaughn herself) stating she wasn’t going to move forward working with us...There was no effort made to assist us in the next steps...
I only dealt with one [nurse at Spring Fertility]. She called me about my thyroid. She either didn’t read my chart or Dr. Vaughn didn’t. She said I need to take medication for my thyroid. I’ve literally been on thyroid medication for years and it’s in all my records. She didn’t even know what I was taking...$450 consult $250 semen analysis $200 Natera for each parent $ misc for lab work...
How was your experience with Sara Vaughn at Spring Fertility Silicon Valley?
After my (very short) intake appointment, Dr. Vaughn proceeded to have us go through all the labs to get started with treatment and required me to see a hematologist. I’m in my 30’s and have never had a bleeding disorder. This requirement to see a hematologist made no sense, but I went because we wanted to get moving with IVF. The hematologist laughed when I saw him and said he had no idea why Dr. Vaughn was wasting my time with the appointment. After getting cleared by the hematologist and having done all the labs, we thought we were moving forward. So I was shocked to get a call from a patient rep (not even Dr. Vaughn herself) stating she wasn’t going to move forward working with us. Basically she doesn’t want to work with couples of our religion. She clearly knew that she felt that way from the start. There was no effort made to assist us in the next steps. The day after that call from the patient rep, I saw a call come in from the Spring Clinic. I wasn’t able to answer as I was with my own patients, but assumed a message would be left. No message was left, but I continued to be called over and over between 4:07pm and 5:56pm. At that point I felt like I was being harassed. Turns out it was Dr. Vaughn calling over and over. The lack of professionalism in calling someone that many times in a row is appalling. Any provider with an ounce of common sense would leave a message and wait for a return call. I never did get to speak to her. I ended up spending with the founder of the clinic, Dr. Tran. I didn’t feel I got very far with him; he noted it would be hard for us to move to another doctor within the clinic because Dr. Vaughn is the medical director so she is over the other doctors at the clinic. So basically it wasn’t worth it to even see a different doctor at Spring because our case had already been tarnished by her opinion of us. He agreed we would be refunded for what we had paid for. I ended up working with the VP of Operations to get a refund. She had the nerve to send us a document saying we had to sign an agreement that included a confidentiality clause which would prohibit us from discussing our experience with anyone, including other doctors. We refused. If they want us to sign a confidentiality agreement it’s because they know she did something wrong and they don’t want anyone to know. We did end up getting a refund after a lot of back and forth. Shame on them for making it so stressful after what we had already gone through! We have since started treatment at another fertility clinic. My initial consult at the new clinic was very thorough and we were treated with respect. Now that I realize the level of care and respect I should have received by Dr. Vaughn, but clearly didn’t, I feel that she is a disgrace to the medical community.

Go anywhere else! This woman has zero respect for a patient’s individuality. She will only help those who fit her mold of what a patient should be. She has no remorse for wasting our time and money.

Brief one-time consult followed by an ultrasound. Blood work for me and my partner. Semen analysis and Natera screening. That’s it - she never met with us again.

We honestly couldn't have imagined a better experience. Dr. Vaughn is thoughtful, compassionate, professional, and always patiently and meticulously answered all of our questions. She was thoroughly prepared from the start, with slides to walk us through what to expect, presented it in an optimistic manner that was balanced by an honest appraisal, very data-driven but in a way even those without medical background can understand, and never missed the human element, even with the challenge of a virtual consultation amidst the first major wave of COVID-19. Added together this was very reassuring to us as the IVF journey can seem quite intimidating. Dr. Vaughn also planned the treatment according to our needs. At our first appointment, we told her if possible we'd like to plan having more than one child in the future. Based on the first egg retrieval results, she suggested a second one for a more comfortable margin of success given our family planning. Although we had been trying for more than a year and I wanted to be pregnant as soon as possible, we felt comfortable with her advice because she explained in detail the multitude of reasons why this was the smarter choice. After following her plan, we now should have enough healthy embryos.
What's one piece of advice would you give a prospective patient of Sara Vaughn at Spring Fertility Silicon Valley?
Don’t hesitate to ask questions. Dr. Vaughn is very patient and she explains everything very clearly. It certainly helps ease a lot of potential anxiety. I remember after my first transfer failed, I asked Dr. Vaughn if we should implant multiple embryos instead of one to increase the chance of success. She told me that not only it will increase the risk of multiple births, it is also possible I’ll lose multiple healthy embryos if my body isn’t well-prepared. My second transfer was successful. Hang in there and don't give up.

Spring has a very strong team. We had a great experience with other Spring fertility doctors who also help along the way, our anesthesiologists, and the whole nursing team from those who drew bloodwork to those who took care of me after egg retrievals and embryo transfers. And there's a pleasant little surprise a day or two after a transfer that goes to show their thoughtfulness from beginning to end and going the extra mile to make patients feel comfortable.

I lost a year of time after going to a different doctor and getting scared off by fertility treatments. Dr. Vaughn is a great doctor and I wish I had started seeing her sooner...If something was too hard (shots, lots of appointments can be hard on top of the stress or work), we talked about how to adjust the plan to work for me and my needs. The appointments never felt rushed. I feel like I got to know her as a person and not just my doctor and that she was genuinely really happy for us when we got pregnant.

How was your experience with Sara Vaughn at Stanford University?
We came to Dr. Vaughn after a discouraging attempt with a different doctor another practice a year earlier. Immediately, the difference in her approach was clear. Our first appointment revolved around setting up a plan for treatment. She came in with an immense knowledge of my entire health history, better than even I knew it. A clear plan of action was developed, taking our input as needed. We left the initial appointment feeling confident, with the right level of cautious optimism and understanding of the chances and risks. It was relieving to know that we were (finally) in good hands. One thing I really think sets Dr. Vaughn apart is her ability to clearly convey medical information. Throughout my treatment, I had lots of questions, and she would always answer every one with the perfect level of detail. It never felt condescending or rushed, and I would joke with my friends that I was getting a mini-fertility lesson each time I saw her. My treatment involved lots of monitoring through ultrasounds, which when I've had them at other locations were incredibly awkward/uncomfortable. While still not a 'fun' experience, Dr. Vaughn recognized I was nervous and would talk throughout the procedure with me and calm me down by explaining what we were looking at and what we were looking for. As an engineer and scientist, the learning and conversation was something that helped me persevere on and reach the goal of having a baby. Dr. Vaughn always had excellent bed side manner. She would always smile when she came into the room and take the time to ask how I was doing. Struggling with infertility can be emotionally hard sometimes, but whenever I had an appointment with her I felt strengthened and encouraged, not by false promises but by a confidence that there was a plan and she would be there with me to go through it. I really wish all my doctors could be as good as Dr. Vaughn. My husband and I agree that she is by far the best doctor either of us have ever had and that she was so much more than we could have asked for. Our baby boy is due in 5 more weeks, and I really do believe we owe it all to Dr. Vaughn.

From the first diagnosis appointment where we came up with a plan of action to the final appointment where we saw the baby's heart beat, Dr. Vaughn consistently treated me as a whole person. She consistently listened to our concerns and took the time to address all our questions. If something was too hard (shots, lots of appointments can be hard on top of the stress or work), we talked about how to adjust the plan to work for me and my needs. The appointments never felt rushed. I feel like I got to know her as a person and not just my doctor and that she was genuinely really happy for us when we got pregnant.

*Understanding of source of infertility (anovultion due to PCOS) through blood test, MRI, ultrasounds, saline sonogram, analysis of husband's sperm *5 rounds of letrozole to induce ovulation. She got the right dose on the first time - Two IUIs - Three natural approaches (the shots/number of appointments stressed me out due to a busy time at my work, so she explained the risks and modified my plan for a few cycles.) Result: baby boy.

My advice would not be about her specifically because she was great, but I would let the prospective patient know to ask about medication that could potentially increase the number of eggs for a retrieval. I only learned about this right before our last round of retrieval following Dr. Vaughn getting input from other doctors at my case review. If I would have known this "everything and the kitchen sink" medication was available, we would have done it the first or second time around.

I had an excellent experience with Dr Vaughn during my two cycles of egg freezing at Spring Fertility. She was warm and always put me at ease when I had any concerns. She was very specific in her thoughts and suggestions on my treatment, which made me feel well cared for. I definitely felt that my treatment was tailored for me. She was knowledgable, trustworthy, patient, and excellent at explaining exactly how everything worked. After my first cycle, I considered waiting a year for another cycle, but Dr. Vaughn was honest with me about her recommendations on waiting too long. After the retrieval, she was excellent with following up with me and checking that everything went well and that I was feeling ok physically. In the end, I had 2 cycles (1 month apart), everything went excellently. I was able to save 64 eggs (an astounding number), and feel good about my prospect for having multiple children in the future. I loved Dr. Vaughn, and highly recommend her!! Due to covid, I was often not in the same location so I did not see her as often as I would have liked, but at no fault of hers.

Starting the process might feel daunting, but if you know you want kids in the future, stop putting it off and just make an appointment. Prior to the consultation, I had no idea how anything worked, but Dr. Vaughn was able to easily educate me on the whole process. I'm so glad I did it when I did and didn't waste more years putting it off.

After the initial consultation, I was told that I had an outstanding number of follicles and was in the 99th percentile of women my age. It was also suggested to me that I might not need as much medication as most patients and so I forewent the medication package. I bought all my meds through Alto, and it ended up costing me about 2K less than the package, so I was thankful for the recommendation. I started with 2 weeks of pills, and then followed by a few days before about 10 days of injections. Everything was monitored closely with frequent blood draws and dosing was decided very carefully and adjusted as necessary. I also did not want to purchase more than I needed since the meds were quite pricey. In the end, I feel that the nurses and Dr. Vaughn did an excellent job of predicting exactly what I needed. Alto was also excellent and I could order something and have it delivered the same day. I also accidentally thought I could use my trigger med from the previous cycle and so I did not order it in time and Spring was able to help me out with that so I wouldn't miss my retrieval. I thought that was super accommodating, given that it was my mistake!

I appreciated that I was able to finance one of my cycles. I felt for the most part that everything was ran very smoothly. They were accommodating with where was getting seen, as I was living in SF at the time but working in Mountain View, and then during the pandemic I was also between places. I appreciated that I was able to get seen at whichever clinic was close to me. Throughout the course of the treatment, I was seen in Redwood City, Oakland, SF, and Mountain View (now Sunnyvale office). I felt that Spring was very flexible. The day of the operation, I felt in extremely good hands! All the doctors and nurses had excellent bedside manner and I never felt any pain or confusion about what was happening. All in all, I couldn't have asked for a better experience.

I read a lot of “fertility diaries” about other people’s experiences in fertility treatments, and based on those I can say Spring was top notch. I did two egg retrievals and one transfer, and they were on time to the minute for all except for one of my appointments - which was only 15 minutes late. All of my treatments took place during COVID and while the facility was opening, which makes it even more impressive. I will always remember Dr. Vaughn telling me at our first meeting, "You are not broken." I didn't know how badly I needed to hear that until I heard it. Dr. Vaughn was warm, caring, communicative, and unbelievably professional. She worked with us to explain each step of treatment and set realistic expectations, and then celebrated with us when our results exceeded expectations. She always seemed glad to see me, provided helpful input, and was just generally a comforting presence. I felt very safe in her care.

I believe it was an antagonist protocol. I was on Menopur, Gonal-F and Cetrotide before my retrieval, and the injection period lasted 12 or 13 days. Dr. Vaughn monitored me often and adjusted my dosage as necessary. Her excellent judgment led to a highly successful egg retrieval (21 eggs) which resulted in 11 5-day blasts, 7 of which were PGS normal.

Strengths--Everyone we worked with treated us kindly. Although I received treatment at a total of 4 locations (Mountain View, San Francisco, Sunnyvale, and Redwood City) the consistency in treatment and the excellent communication amongst staff created a feeling of continuity over the course of our care. The clinics themselves are comfortable, very clean, and have a friendly and not overly clinical feeling. It was especially interesting (and reassuring) to be able to see into the lab at the SF location from the treatment room. The staff are consummate professionals and always made us feel valued and cared for. The sweet flowers and card they sent after my transfer had me in tears--I just can't imagine a better team to guide us on this very long road. Weaknesses--honestly, almost nothing. Our treatment happened to coincide with the move from Mountain View to Sunnyvale, as well as the holidays, which meant we occasionally bounced around locations--but the fact that someone showed up on Thanksgiving to give me an ultrasound was remarkable enough to make it worth the drive!

Since it was a midst the Pandemic that I started visiting Spring Fertility I only saw the team and staff there a few time. They were helpful and responsive through email and phone calls for every need or question, however...Clean center, small but I liked that. I was able to see Dr. Vaughn at this clinic which was closer to home for me when normally I think she works up in San Francisco.

I chose Dr. Vaughn because of her experience with PCOS. It was clear from our first meeting that she knew what she was talking about and didn't just have experience but sincere expertise. I appreciated that fact that she had understanding of PCOS and was really willing to listen to my specific situation and journey. I had been working with more holistic care professionals up until meeting her and was looking for a second opinion mostly when I set my appointment. She was able to set me up with some more detailed procedures to make sure that all was working as needed and wasn't the cause of my fertility issues so far. While she ignored a lot of what I shared with her from my work with my Functional Medicine Fertility Specialist, she suggested that I have a saline sonogram and hysteroscopy which my FMD hadn't suggested. These procedures ultimately led to as close as we could get to real answers and a better larger picture understanding of how my system was functioning. I don't that this is necessarily a red flag or an issue it's just how she was trained and I appreciated her accepting and working in parallel to the holistic work I was doing without negating it or actively trying to contradict. The only other thing I would add for others trying to learn if Dr. Vaughn is a good fit is that one of the first things she looked at when I mentioned PCOS was my weight, I did not appreciate this and she eventually worked through that my weight was not the issue but as most main stream medicine professionals have a focus on weight as a first answer, especially with PCOS, Dr. Vaughn does seem to be right in line with that.
What's one piece of advice would you give a prospective patient of Sara Vaughn at Spring Fertility San Francisco?
Be ready with questions, she will take the time to explain everything but knowing the questions to ask to get the answers and explanations you need is useful. If you are someone like me and wants all the information as deep as it will go then do your research ahead of time. If you are happy to let your doctors be your fully trusted guide I don't think you can go wrong with Dr. Vaughn either, though.

We did some testing so she could confirm my PCOS. The issue that seemed to present itself was that my endometrial lining was thin. I had an inkling this was the case as my periods were not full periods, they were very light (practically just spotting) and not fresh red blood. She first suggested a saline sonogram which she performed in order to make sure there were no blockages. She mentioned that sometimes the cervix can become blocked and simply performing the sonogram would clear that blockage in the most simplest of cases, otherwise it would give us more insight into my uterine lining and opening. After the sonogram, my flow did not improve by my cervical mucus did, I was seeing more fertile mucus than I had been seeing before so I saw this as an improvement. She also thought that there maybe there was something blocking the uterine opening and mentioned that the saline sonogram couldn't tell us much more then there was something there and suggested a hysteroscopy to get more information. My endometrial lining was still seeming thin but it was hard to tell based on the timing in my cycle. Dr. Vaughn sent me to a wonderful OB to do the hysteroscopy due to my insurance requirements. The hysteroscopy clarified that I had some polyp like tissue in my uterus as well as other fluffy tissue. Non of this was of a real concern but they cleaned up my uterus anyway. I believe this was a significant part of us being able to conceive naturally in the end.

When I came to Dr. Vaughn, I expected to be treated like a number but ended up being treated like a human. She is warm, friendly--yet direct and confident. And she is reassuring without promising false hope. You get the sense she really cares because she spends time with you addressing your concerns and remembers details about your life. She is the perfect mix between relational and professional.

For our treatment, we of course first did all of the necessary testing and landed in the undiagnosed fertility category. Once treatment began, I started with Clomid, then triggered Ovidrel--having the IUI procedure two days later. The plan was to try that three times and then discuss other options if that didn't work. The first round was not successful but the second round was. Dr. Vaughns's rationale for using IUI as a first treatment was mostly my age (and I'm sure lots of other really logical concepts I don't remember). I remember she felt really confident about the treatment plan and so did we.

Spring Fertility seems to have a really well-organized system. There is always someone there to talk to, answer questions, or help you stay informed. You have a patient navigator, a nurse, a financial coordinator, a doctor, and then other people that call you to check on you periodically. For me, I think the doctor you get really makes a difference and I can't speak to all of the doctors but the ones I interacted with in addition to my primary doctor were really professional, competent, and friendly. They were also extremely accommodating about scheduling as I work and live two hours away from Mountain View where I was seen (I also went to SF occasioanlly). I never had trouble scheduling, and ultimately that is to the credit of my amazing Patient Navigator, Zunaira. She worked really hard to get me scheduled at times that were convenient for me.

I would highly recommend her to anyone undergoing IVF, especially those with diminished ovarian reserve.

Spring Fertility has a great success rate, which is why I chose them. Their equipment and methodologies are very up-to-date, and their systems are seamless. The only downside to working with them at their Mountain View office is that it is only open M/W/F, so for appointments on other days, I had to go to Redwood City (Tu/Th) or San Francisco (weekends). Only the San Francisco office does retrievals. Therefore, there was some logistical hassles, but the doctors at the different offices exchanged information seamlessly and there were no issues with my treatment due to this.

My egg retrieval treatment included regular appointments for ultrasounds to monitor ovarian follicle growth and bloodwork to monitor hormone levels. I began with 10 days of birth control and then moved on to 12 total days of twice daily injections before my retrieval.

Dr. Vaughn cared deeply for me as a patient and for the success of my treatment. During my second egg freezing cycle I had two groups of eggs growing at different rates. Dr. Vaughn made the excellent call to let the first set go as she believed (and was correct) that the second group would produce more viable eggs to freeze. I believe this was a tricky dicision and she nailed it. Every visit I felt welcomed and relaxed in the office and in the presence of Dr. Vaugn and her nurses. The were always available for question and Dr. Vaughn talked to me at any time of day/night to answer concerns I had. The only thing I did not cover too much up front or perhaps the information did not stick with me were if there would be any adverse effects. Understanding that in more detail may have been good.

Dr. Vaughn proposed running a limited number of tests to determine my egg count, hormone levels, etc. And upon looking at the results, Dr. Vaughn figured out that I likely had a borderline case of "lean" PCOS, and that using clomid and ovidrel would likely be successful. And because of my egg reserve and age, she saw no reason why we would need to move to IVF immediately unless I wanted to. I was so relieved I cried tears of joy. And Dr. Vaughn was absolutely right: we got pregnant on our very first cycle of clomid/ovidrel and I had a beautiful, healthy baby girl.
The Stanford clinic was generally very good - the staff were always on time for appointments, the clinic had blood work that could be done easily on site, and the facilities were clean and nice. There were one or two times where billing was handled inaccurately and that led to some insurance headaches. But the clinic worked with me to get the errors fixed...I did not actually engage with the nursing staff very much...Dr. Vaughn managed most of my care directly,
How was your experience with Sara Vaughn at Stanford University?
Dr. Vaughn is one of the best doctors I've ever had. Before finding her, I had met with half a dozen fertility doctors over the course of a year and half. During that time, these other doctors were unable to determine why I wasn't getting my period and/or wasn't ovulating. And all but one told me I had very little hope of conceiving and that I should immediately start IVF if I wanted any chance of having a baby. By contrast, Dr. Vaughn proposed running a limited number of tests to determine my egg count, hormone levels, etc. And upon looking at the results, Dr. Vaughn figured out that I likely had a borderline case of "lean" PCOS, and that using clomid and ovidrel would likely be successful. And because of my egg reserve and age, she saw no reason why we would need to move to IVF immediately unless I wanted to. I was so relieved I cried tears of joy. And Dr. Vaughn was absolutely right: we got pregnant on our very first cycle of clomid/ovidrel and I had a beautiful, healthy baby girl. After that, Dr. Vaughn kept in touch and helped me make some critical decisions when I needed to seek fertility treatment again but no longer had insurance coverage that enabled me to be treated by her. And even before that, Dr. Vaughn was very understanding and sensitive to what tests and treatment were covered by insurance so that I would not end up with huge out of pocket expenses if at all possible--I've yet to see a doctor before or since who was so proactive and conscientious about costs when I expressed concern. She also worked hard to make sure she was available for all of my appointments - including ultrasounds - so that I wasn't simply scheduled with whoever was in the office at the time. In short, Dr. Vaughn is exceedingly smart, kind, thoughtful, and attuned to her patients' wants and needs. I could not endorse her more strongly.

If you have any doubt, just meet with Dr. Vaughn to see what you think. I'm confident her smarts and kindness will persuade you that she is a fantastic and caring doctor.

Dr. Vaughn was the first of far too many fertility doctors I had seen who immediately understood the stress and desperation I felt, discussed my options in a candid but supportive way, and from the beginning sought to understand exactly what I had gone through and how she could help me to accomplish my goals--without needing to unnecessarily repeat tests or put my body through additional stress. Not only is she extraordinarily smart, but she's also kind, thoughtful, and caring, all of which comes across from the first time you meet her.
Describe the protocols Sara Vaughn used in your cycles at Stanford University and their degree of success.
I used a low dose of clomid and ovidrel. So far as I understand it and can recall, the rationale was that I ovulate irregularly, which makes it very hard to use timed-intercourse effectively through natural means. We started with a low dose of clomid to see if that would be sufficient to grow a follicle and stimulate ovulation - and it was. I then used the ovidrel to trigger ovulation and had timed intercourse. It all worked perfectly.

Dr. Vaughn managed most of my care directly, so I did not actually engage with the nursing staff very much.

For example in my case doctor decided to start with IUI because it looks to be apt process for me but in middle my body didn't responded well to the decided plan and so with mutual discussion we decided to go for IVF without any further delay which saved my time and energy. She will make her decisions based on blood work and ultrasound. She follows all the guidelines and protocols stated by state of California and their clinic.
