Since we were freezing embryos for fertility preservation, and are not choosing to get pregnant at this time, I can only speak for that portion of treatment - but we got great results and I didn't have any significant setbacks during the priming/ injection period.

Overall, the nursing staff was great - although some were more approachable than others (I enjoyed working with Kali and Rae for the regular correspondence). I never had a message or call go unreturned, and they were often quick with their responses. Since there wasn't much face time with the Dr., I think it would have been nice to have one main designated nurse, and one backup nurse, to allow for some form of consistency. Throughout the procedure, I never got to see them in the office, and at times would end up speaking with other nursing staff that I was unfamiliar with. That being said, everyone I saw for my in person check-ins (blood draws, ultrasounds) were always incredibly kind and helpful.

I went with UCSF as they were the best fit for our insurance, and a couple friends had also recommended them. They both had the same forewarnings: you won't have one consistent Dr, and you have to be your own advocate when it comes to billing and the overall logistics. That being said, I would definitely return to UCSF for any future treatments, as I felt that everyone there was very knowledgeable and I trusted turning my body over to them (which I can have a lot of barriers around).
One thing to note is that their billing department is a real pain to work with. I kept receiving bills to pay hundreds of dollars, but when I called to question why I was receiving the bill (as it should have been covered by Progyny, they told me I didn't have to actually pay it.) I'm still not 100% confident that we're in the clear in terms of money owed.

Estrace priming for low AMH. for IVF. Second one worked when she upped the dose of estrace and we have a gorgeous baby girl from that.
She started off with the standard 3 IUIs, which were a complete waste of time in hindsight, but at that stage I was also not ready to go to IVF. If I did it again I would skip the IUIs.

Very sweet but not the brightest bulb in the room. She couldn't figure out my period days and almost had me start on a wrong day until I kicked it up to the doctor. Also when I did not conceive with my first IVF, she was robotic in her response ' these are questions for your doctor. Luckily I called the clinic and the nurse on call was so compassionate and helpful about not conceiving the first time, it really helped me at that time.

For instance, there are multiple forms that you must sign with a witness and no one makes sure that you have all of them at once and gets every single item signed at the same time. They are very disorganized and seemingly quite unaware of the issues. They are supposed to be fixing it and I hope they will so other people don't waste time and money. Make sure to advocate for yourself. Try your best to get an understanding of the bigger process and deadlines if you're on a tight timeline. Self pay medications with them are extremely affordable. I think friends that went to private clinics paid a lot more.

In person and on the phone, Dr. Segal (and every other person at UCSF CRH that I've been in contact with) is kind, informative and very helpful. While our diagnosis is unexplained infertility, Dr. Segal seemed to create a plan that made sense and explained anything I had questions about.
The process at UCSF, however, is very flawed. It's not clear where you are in the process, who you are supposed to be in touch with, it takes on average 3 calls to get a phone call/email back. Expect for this to take longer than you think it will. I had my first appointment early April and was planning to do my first egg retrieval starting end of May and one day before cycle day 1, they called to tell me that they had to push to the next cycle. Needless to say I was quite unhappy about this as we were trying to get all this done as my insurance plan year is coming to a close. Apparently UCSF is working to fix some serious issues, but be warned - you must be an advocate for yourself and push them along. Maybe other teams are more organized but it really sounds like it's a UCSF thing not a team/Dr. specific issue.

Skipped estrace priming, 4 days Menopur 150 IU/ Gonal F 150 IU, 5 days Ganirelix 250 mcg and Menopur 150 IU / Gonal F 75, cotrigger with 4mg Lupron/2500 units HCG.
I had 2 lead follicles early in the process so skipping the estrace priming was probably a mistake and will be added to the next egg retrieval cycle. My estrogen level was a bit high so changed to cotrigger Lupron with HCG instead of only HCG.

I spoke to the nurse very rarely. Two issues happened - the nursing team couldn't call in my prescriptions in time for the first scheduled egg retrieval in early June. This was the reason my cycle was pushed out.
Second issue, is that no one called with my 2nd blood draw numbers and so I didn't change my dosage. It's unknown whether a dosage change would have been needed but I was instructed after my 3rd blood draw that if I hadn't heard from anyone that day to call (even to ER line after hours) the day of a blood draw to confirm the dosage amounts.

because UCSF is a large organization with its own silos there can be misses in the experience...we had to delay one procedure because someone in the back-office did not do what they said they would do to send an authorization request to my health insurance. It took active program management on my end keeping things on track and moving forward.

Dr. Segal helped us through one of the hardest experiences of our lives -- trying to get and stay pregnant. After we miscarried, we were set for the next transfer as I had just received my medications, but then Covid hit, and everything was put on hold, indefinitely. She called and emailed to check-in with us as time went by. She also helped navigate some of the challenges within the UCSF system. We now have an amazing daughter that I am so thankful for. I could not imagine going through this process without a doctor like Dr. Segal.

We attempted two rounds of IUI. Both were not successful. We advanced to IVF. For egg retrieval, stimulation phase we used Estrace, Gonal F, Menopur and Ganirelix. The trigger was HCG. After live transfer, I was on Medrol, Endometrin, Estrace and then a Valium (one-time). For our second transfer (FET), originally, we were going to do the "gold standard" which included progesterone in oil. But then Covid hit, and everything was put on hold. When we re-engaged with Dr. Segal a few months later as things were opening back up, we decided to forgo the progesterone protocol as we had two embryos frozen in the same tube and opted to transfer both. I cannot recall the specifics of the protocol the second round, but it was much lighter. Thankfully, one embryo took.

I think because Dr. Segal was a newer doctor, in the beginning she did not have a dedicated nurse, just a coordinator. So, it was a rocky start. Things improved once she had a dedicated nurse, although there were two nurses helping at times, which sometimes complicated things.

UCSF does feel as though it's at the forefront of medical advancement. But because UCSF is a large organization with its own silos there can be misses in the experience. UCSF was the only clinic that was covered by my work's fertility benefit, so we made it work. But we had to delay one procedure because someone in the back-office did not do what they said they would do to send an authorization request to my health insurance. It took active program management on my end keeping things on track and moving forward. I imagine if you are self-paying, it is a much easier experience. But being covered by a fertility benefit made the experience at times extremely hard. The unseen benefit was my insurance provider (Aetna) had a nurse that I eventually discovered who helped me navigate the whole insurance filing process for both infertility and then for pregnancy.

Unfortunately, I do not. My work provided an infertility benefit that we were able to leverage. We did have to pay our deductible and then 10% of the costs. I believe we paid somewhere between $5k and $10k but cannot be more specific at this time.

It took an escalation to the managing staff to get things back on track but by then, we had to delay the process a month. At the time, I was devastated.

UCSF has top-quality doctors and extremely high-variance support from the coordinating team. I felt confident in the answers to my medical questions.
UCSF also has a lot of supporting materials and resources if you know how to find them. For examples, when we moved to PIO, we were assigned an "injection coach" nurse who was great.

Dr. Segal and her team personally reached out to me on every occasion necessary. I had 2 procedures and they called me each day and the day after to check on me and how I was doing. I was able to speak with someone on her direct team each time I asked.
Describe the protocols Thalia Segal used in your cycles at UCSF and their degree of success.
First I had a hysterosalpingography (HSG) to assess the status of my uterus and tubes after 2 years of infertility and an ectopic pregnancy. Then there was some irregularities on my initial ultrasound and I underwent a hysteroscopy to remove some tissue and scarring in my uterus. Next, together, we opted to attempt 1 IUI cycle before proceeding with IVF. I did 5 days on Clomid 100mg, had an ultrasound to count follicles. on Day 10 I did 1 trigger injection of Ovidrel 250mcg. Had the IUI on day 12. On day 14 I began using Endometrin 100mg vaginal inserts. 2 weeks after my IUI I received a positive pregnancy test and used Endometrin until my 10th week.

Anne is the best! She contacted me personally on many occasions to check on me after procedures, after a little bleeding scare I had. She even called me on one of her days off to make sure everything was ok.

Saline sonogram revealed a polyp, which was removed via out patient procedure. Elevated hormones revealed potential thyroid issue now managed with medication. My husband and I naturally conceived the cycle after polypectomy, so we assume that was a major contributor. Unfortunately that pregnancy ended in a missed miscarriage. Because of the time sensitive nature (fear of ectopic), another on call doctor performed the procedure, but Dr Segal called me multiple times that weekend to check in. I then conceived again the next cycle. Between the thyroid and the polyp, I believe Dr. Segal has helped me resolve my fertility issues.

Kali was an amazing support during my two back to back pregnancies (first one non viable). She and I checked in nearly every day while going through the confusion of a possible ectopic and ultimate MMC.

I had consulted with 3 not-so-nice doctors/clinics before meeting with Dr.Segal at UCSF. The 3 doctors/clinics either talked to me like a sales pitch or brushed me off insinuating that I would lower their success rate. Therefore by that point, aside from seeking for help, I also just wanted someone to be kind to us. Dr. Segal treated my husband and I as her patients the moment we met her. She was so compassionate, explained the process to us clearly, was easy to talk to, and most importantly— I can tell that she wanted to help us achieve our baby goals. She got the ball rolling the day of consultation. Nurses drew blood and listed out the medication I’ll need. Dr. Segal gave us the cliff notes on how to navigate and follow up with their clinic. Essentially, she also told us to be patient but yet follow up with the staff. Additionally, she told us that it was usually the people who stayed persistent who saw success. That really helped set expectations for us — and when things got tough, we reminded each other to stay persistent.

Be mindful that everyone at the clinic is very busy. Be patient with the staff but also stay organized. I was constantly on top of scheduling my appointments, making sure I don’t have any payments outstanding with finance dept (it can delay your treatment), and make sure the nurses have sent the medication authorization to your pharmacy.
Also, Dr.Segal is busy. When I don’t hear back in 1-2 days (pending the urgency), I’ve had to follow up and ask someone to check if Dr. Segal has seen my request.

Anne would take the time out to chat with me when she calls to schedule my next appointment. Going through fertility treatment can get emotionally exhausting. Our insurance allowed up to 6 IUI treatments but didn’t cover IVF and medication. Unfortunately I don’t remember the exact cost but I do remember that UCSF IVF were the lowest compared to the other clinics. If the cycle stops at a certain point before egg retrieval, you can have the remainder applied to the next cycle. Fertility medication is in addition to the cycle cost. My medication for IVF and stimulated IUI cost around $7-9k each cycle. We did 2 IVF cycles and 5 IUI cycles.

She never dismissed our concerns, and promptly as well. We felt heard...She gave us hope, but also explained the risks in the process...Our doc recommended single embryo transfer because of the risks involved with multiple embryo transfer. I loved UCSF. They were (relatively) inexpensive, doctors were great, staff was friendly, the clinic was neat. They took us in as patients immediately, and didn't delay the process unnecessarily. Sorry, I cannot remember thoroughly all the specifics of my treatments that were done. The first egg retrieval took longer than expected since most of my eggs were behind. 9 eggs were retrieved, 2 were tested but were not
viable. Second egg retrieval had to be halted midway, since my eggs were not responding and I did not produce much eggs. The 3rd egg retrieval resulted to 9 eggs, 4 tested and 3 were viable.

IVF is not easy, it takes time, patience, energy and resources - take it one day at a time and keep a positive attitude. Do your research. Be transparent with your provider about your expectations and ask a lot of questions.

IVF patient already have a lot going on and this mistake on their end was a nuisance

I even remember a time she drew a diagram for us, which I loved!...collaborative and respectful.

The people drawing blood were great....I liked that the waiting area had little books and notes from other women struggling with infertility...

What's one piece of advice would you give a prospective patient of Thalia Segal at UCSF?
To not be afraid to talk openly about concerns. She is very collaborative. For example - I didn't want to use Clomid bs I was worried about the risk for multiples, and we talked in depth about the risks and benefits.

UCSF has a deep bench of doctors. They rotate care for procedures. Everyone I interacted with was kind.
