How was your experience with Thomas Fisher at Spring Fertility East Bay?
I love that the doctors at Spring are forward thinking and progressive, ready to try anything that could work to help an over 40 mom like me. At UCSF the doctors were cynical, so when I brought up things like a growth hormone they sighed and said that it probably wouldn’t do anything to help…. But Dr. Fisher made these things a part of his plan from the beginning and the results were clear. I did my first treatment at UCSF at age 40 and got 2 euploid embryos, one of which resulted in a miscarriage and the other which did not take. When I came to Spring at 42 after that series of disasters at UCSF we were really hoping to get even one, but with Dr. Fisher’s protocol we were able to get 2 despite my age, and I am now listening to my healthy baby daughter cooing on the couch next to me. Dr. Fisher and his team did everything in their power to help me and it worked. Dr. Fisher is a realist, so he did not bloat my expectations and set me up for disappointment- he kept it real and warned me that we might not be successful. But he worked SO hard for us, did everything in his power, and didn’t ignore possibilities that could help just because the data was not as vast as it could be. If there is data that something could help, why not try it? I have no doubt that Spring’s willingness to try things (within reason- nothing that could harm me) is the reason why my daughter is here today. I cannot wait for him to meet her! As soon as we emerge from the newborn bubble I am taking her to there and I know he will be so overjoyed. He felt like family to us and we know he really cared. He shared tender experiences from his own life to help make me feel better when emotions overwhelmed me. He was a true ally.
What's one piece of advice would you give a prospective patient of Thomas Fisher at Spring Fertility East Bay?
Don't be put off by his blunt nature. He is just being honest and that is so important in a doctor. He is razor sharp and so very smart, but also very human and enthusiastic. He is the best!

I have actually worked with 2 different clinics, UCSF and Spring Fertility, so I feel like I have a pretty decent perspective on why Spring is the absolute best. I worked with Dr. Fisher and had a wonderful experience after being somewhat traumatized at UCSF. The first thing that really stands out is how much time you get at Spring with your actual doctor. At UCSF the doctors are like these celebrities that you only communicate with via video chat and at the actual retrieval, and nurses are all you see in the many monitoring appointments. At Spring you see your doc frequently and he/she does all the monitoring and ultrasounds to make sure that things are going smoothly. This meant the world to me. I also loved the way that Dr. Fisher communicated. He was blunt but kind. I loved his realist perspective.
Describe the protocols Thomas Fisher used in your cycles at Spring Fertility East Bay and their degree of success.
He used ICSI and omnitrope which I really think made the difference for us. With aging eggs like mine both of these things are really helpful. He was able to adjust the protocol as needed since he was the one doing all of the monitoring sessions.

At UCSF I felt like I was using my health insurance and getting very basic care, not spending tens of thousands of dollars out of pocket, but at Spring my husband and I felt the benefits of a boutique health care experience. Things like snacks in the waiting room etc are nothing in comparison to the effectiveness of the treatment, but honestly since we didn’t have insurance that would cover the cost we really appreciated being spoiled a little. At UCSF we felt like a number, not valued at all. At Spring we felt that human connection and really appreciated the care that we received. The fact that they were also more effective is the most important thing of course. I would recommend Spring to anyone, and in fact my very best friend is starting her care there right now. That is how good they are. They have forever touched my family’s life.

I know I want kids in the future and therefore was contemplating egg freezing. Dr. Fisher never made me feel like he was selling the idea of egg freezing and rather me provided with facts and data...always explained what was happening and provided next steps so I knew what to expect...created a sense of trust and removed much anxiety which is important as a patient.
[Spring Fertility was] Extremely clean and comfortable...[nurse] was very quick to respond, clearly communicated all instructions and would answer (or get answers) to all my questions. Her responsiveness ensured I felt like I could rely on her... the billing team was not as organized or responsive...Right now I am resolving an issue with them where a third party billed me for more than what I was told my maximum bill would be.

Dr. Fisher is incredibly knowledgable and patient making him very easy and great to work with. I saw Dr. Fisher about freezing eggs and embryos and from the onset always felt like he shared his knowledge of fertility which allowed me to make educated decisions. I was 32 turning 33 when I first saw Dr. Fisher about egg freezing. I know I want kids in the future and therefore was contemplating egg freezing. Dr. Fisher never made me feel like he was selling the idea of egg freezing and rather me provided with facts and data. It was because of this that I chose to do egg freezing with Dr. Fisher. There are some Drs. who gave me the impression that were selling me the service as it was must do however Dr. Fisher never did that. He kept to facts and when asked his opinion to stuck to facts such as 'typically egg quality is like x during y, and then will be z during a". This made me feel like I wasn't getting sold something but rather had a Dr. who kept to the science which I greatly appreciate. I could also get a sense that he cared for his patients and he always took the time to answer questions. I left our initial consultation feeling like I had a better idea of female fertility and reasons why but did not feel like I had to freeze my eggs at that exact moment. I chose to however for personal reasons of knowing that if I was going to in the future anyway why not now especially having found a Dr. i liked working with. During my egg freezing Dr. Fisher always explained what was happening and provided next steps so I knew what to expect, what was happening and why, and what was to come which created a sense of trust and removed much anxiety which is important as a patient. Dr. Fisher monitored my progress throughout my egg freezing process.

During treatment, were you treated like a number or a human with Thomas Fisher at Spring Fertility East Bay?
Walking through the process and the procedure and though data was shared it was provided to give me insight as a patient and to understand the data on egg freezing and embryo freezing. They also walked me through what it is my body is doing throughout the process which felt very human to me and I found it extremely educational. Dr Fisher also walked me through female fertility during your thirties and forties and I felt like a human as he explained what happens to a human body and used this information to help me understand my body and the process. He would separate presenting data to where I am and therefore I felt a lot like a human.
Describe the protocols Thomas Fisher used in your cycles at Spring Fertility East Bay and their degree of success.
My treatment strategies and protocol was to track my ovulation as I had gone off birth control (my own choice) ahead of egg freezing. Since my amh levels came back normal (higher than average) and my initial consultation showed regular follicle count I was to use gonal f and menopur during the monitoring phase as we began the egg freezing process. Right from the beginning I was told at what time of day to test for ovulation and to immediately call/advise the clinic when I got a positive. Since this was during Covid the clinic also did covid tests ahead of treatment which I greatly appreciated given I was paying for this myself. Once I got a positive they booked me an appointment for a blood draw (I think the day after possibly) and since blood draws were in the morning they could get results by afternoon and provide us with our protocol. I was then given clear instructions both via a calendar, a web portal and a phone call by a nurse of what to do for the next few days. I started off with gonal f and menopur and would go in every 2-3 days for monitoring to begin with. Closer to my retrieval I went in every day and my dosage changed a little which again was communicated via a shared calendar, a web portal, and a phone call. They also did injection trainings ahead of this so I felt as well prepared as i could have been having never injected myself before. On the day of my trigger, because one of hormones was high I did not have to use two medications to trigger but one. The nurse had called me the day before to give me the exact time I was going to trigger and to walk me through exactly which medications to use, how to do the dosage and was very clear on the importance of timing. They recommended getting the injections prepped a little before the actual trigger keeping in mind the need to refrigerate one of the medications. The anesthesiologist also called me the early evening before the retrieval so I knew waht to do in terms of food and drink.

Mel was my primary nurse and she was very quick to respond, clearly communicated all instructions and would answer (or get answers) to all my questions. Her responsiveness ensured I felt like I could rely on her. She was very clear with instructions and was patient with me as I would repeat them back to her as I this was my first time. The nursing staff at Spring overall were all very friendly and very caring. My care coordinator was also very responsive and also always answered (or found answers to my questions). They were great to work with. During the day of my egg retrieval the nurses (and entire team) were all very caring, patient, and it was comforting to be under their care.

Strengths - definitely felt like state of the art equipment especially during the egg retrieval and also seeing the embryologists work in SF made I was visually able to see the state of the art facilities. Extremely clean and comfortable and I could tell all the nurses and drs were extremely professional while also kind and caring. The extra touches such as the sparkling water and snacks/chocolates was definitely a plus. Though for me what is most important is the knowledge and professional of the team which was a 10/10 and the healthcare equipment which was also a 10/10. Weaknesses - the billing team was not as organized or responsive. Sometimes I had to follow up with the billing team on certain questions and one time I was told I had an outstanding payment when I did not. Right now I am resolving an issue with them where a third party billed me for more than what I was told my maximum bill would be.

Egg freezing I believe was $8750 for my first round and then I think my second round I got a 10% discount. Embryo freezing (I did one round) was approximately $16,000-$17,000 for one round. In addition, each round required medication which cost around $3000-$4000

He was patient when I had ten million questions and wanted to know why for everything. One of the things that made me respect him the most, however, was how highly his staff talked about him. They would tell me how they learned so much from him from his in-service classes he taught weekly. How he really wanted them to have the knowledge and love what they did, and he did so in a fun and interesting way. All of that could be seen as a patient as well. We could not have been in better hands!

Him and his team made me want to come in because it felt so warm and inviting.

I loved that I received a call from a nurse after every appointment and that they checked in on me after my procedures. I spoke to random nurses and rarely my own nurse. However, my Care Coordinator was always available, always coming in to cheer me on, and even held my hand when I got sick after my saline sonogram. Anything I needed she was always on top of it!

I was seen at both Danville and Oakland and I was impressed with every single person I came in contact with. Everyone was always friendly, upbeat, and generally happy to see you/be at work. I always received prompt professional answers, and always felt like the only person they were treating. There is not one single negative to report.
Describe your experience with your monitoring appointments at Spring Fertility East Bay.
I’m not sure if it was due to the pandemic and trying to keep people from waiting in the waiting room together, but I never saw any other patients in the Danville or Oakland office for monitoring. I was always 5-10 minutes early and in my car about 5-10 minutes after my appointment. I was seen quickly with all my questions answered. And not once did I feel rushed to get in or to get out. I was monitored twice by other doctors because it was Dr Fisher’s day off. I must say, his way of explaining the monitoring process and giving exact detail was much better than the other two. The other two told me to ask the nurse when they called in the evening. It wasn’t rude, just that it was the nurses job.

Like many patients, I was very anxious heading into IVF treatments. I am an OB/Gyn and I knew that the odds were stacked against me given my very low ovarian reserve. Dr. Fisher laid out a plan and explained what I can expect and the rationale behind everything. He was always available for a phone call or message to answer the multitude of questions I had, was always patient, and never made me feel like I was asking a stupid question. He kept my expectations realistic while genuinely cheering me on through the process. In the end, I ended up having to do fewer egg retrievals than predicted, and got pregnant after the first transfer (and just had my baby!). I am so grateful for the guidance and compassion he showed me through my fertility journey, and feel so lucky to have had his care.

Some patient are hesitant about seeing a male doctor, but I found Dr. Fisher to be gentle, caring, and professional, and provided the best care I could have hoped for.
Describe the protocols Thomas Fisher used in your cycles at Spring Fertility East Bay and their degree of success.
Given my low reserve, we did mini-stim cycles. Initial plan was to do 3 retrievals with the goal of getting one embryo per retrieval, and transfer 3 day 3 embryos in the end; day 3 embryos because of the attrition of embryos from day 3 to 5. I ended up getting 3 eggs from 3 follicles on the first retrieval, all of which fertilized and became day 3 embryos. We did a second stim cycle but didn’t retrieve because there was maybe just one medium-sized follicle. Instead, we transferred the 3 embryos that cycle and I got pregnant with a singleton pregnancy.

The nurses were thorough and very responsive.

Dr.Fisher has a ton of knowledge that doesn’t come off pretentious which is great. He isn’t scared of patients doing their own research and has a HUGe sense of understanding of what infertility does mentally and how that effects the amount of information we need to feel better about our situation . He’s so compassionate and is always listening intently without being dismissive. He’s also very kind and asks us about how our day is going/how our weekend was and also includes my partner in that conversation.

Spring is a great experience. If only hospitals and primary care care providers are as on top of things as my fertility clinic, life would be so much easier. You don't have to know anything at the start of the IVF process but by the end, you will know a lot and it will have been pretty efficiently communicated by their team...Mel is very on top of things. Dr. Fisher is warm, attentive and kind. He is clear with communication and is empathetic in making sure we understand what could otherwise be confusing information for first time undergoing IVF. We are super happy we chose him as our doctor.
Describe the protocols Thomas Fisher used in your cycles at Spring Fertility East Bay and their degree of success.
We used mid-luteal for the first three retrieval rounds and Aygestin protocol + calcium ionophore for the fourth retrieval after a failed third round. Rationale for mid-luteal being more natural method of getting cohesive set of egg growth for each cohort. This worked well for the first and second rounds. So far we have 5 that made it to blast at fourth round with different protocol and are waiting for results of genetic testing.

In fact he is warm and very friendly and genuinely cares about his patients. He sets realistic expectations so that nothing comes as a shock.
Spring has the best of both worlds. It’s equipped with latest and greatest of technology and competent providers. Yet it provides care with the delicacy of a small fertility clinic who care about their patients and don’t make you feel like you are just a paying “customer”. I had a wonderful experience dealing with staff and doctors and would recommend it to my friends and family.

Dr. Fisher was invested in my treatment and care and made sure he utilized his professional knowledge to get optimal results for me. When discussing stats, I sometimes felt like I was going to be part of a statistic too, however, this can’t be avoided since Dr. Fisher has to set realistic expectations for me.

Staff is very friendly and put you at ease. They empathize with what you’re going through. Communication is great. However due to covid everything was over video and phone calls and that made me a little overwhelmed in the beginning. Once I had an understanding of what to do and how to do, I thought staff did an excellent job at keeping everything on track.

Candidly, when I first approached Spring, I wanted a female doctor but the earliest appointment was with Dr.Fisher. Although initially hesitant, since day 1 of seeing him, I felt very comfortable and at ease. He opened up and shared his own story of how him and his wife also went through their own fertility journey. I think that makes him a lot more relatable and empathetic to his patient's journey. He was polite, respectful, always greeted me with a smile, was patient with all my questions and never made the appointments feel rushed. He was always thorough in explaining procedures and next steps, comprehensive with his exams of follicle sizes, concerns and what to expect. I really appreciated the attentive care provided whether it was via email, in person or follow up calls after certain procedures to update me on progress. I remember the day of my transfer, he rushed over to the SF clinic all the way from Danville just to make sure he was the one doing the transfer himself. That definitely re-assured me, made me more confident that the chance of success would be high and overall just put me in a more positive state of mind. We are now a month away from meeting our miracle baby and I am immensely grateful to Dr.Fisher and the Spring team. Dr. Fisher and staff were always friendly, on-time, patient with my many questions, understanding when I got emotional due to any results or drop offs from retrieval, day 3, day 5 survival and embryo quality. Dr.Fisher was responsive and personable on calls and always made me feel heard.
Describe the protocols Thomas Fisher used in your cycles at Spring Fertility East Bay and their degree of success.
I went through 2 IVF cycles and 1 frozen embryo transfer with Dr. Fisher. For both retrievals, I was taking the following supplements: prenatal, Vitamin D, Fish Oil, and Q10. My diagnosis was unexplained infertility as I had done prior exams and blood work at another clinic before Spring. In my first cycle, my antral follicle count was only 9-10. I started off with taking Estrace, got a bleed, went in for a baseline ultrasound and then started ​225IU of Gonal F and then 150IU for Menopur. The dosage of Gonal and Menopur were dropped as they doctor tracked my bloodwork and the size and growth of my follicles every few days to try to get them to all grow at the same pace. I then started Cetrotide in conjuction with Menopur and Gonal F and eventually triggered with Novarel and Lupron. For my first retrieval, my body only yielded 8 eggs, 1 follicle was empty and only 1 egg was mature. That was devastating news and I remember crying for a long time thinking the odds of a healthy embryo was likely going to be none after the drop off from Day 3 to Day 5 to genetic testing. Dr.Fisher and the care team definitely set proper expectations and were also quite comforting in talking through alternatives for a second round. Miraculously, that one mature egg actually resulted in a 6AA Day5 Euploid embryo. Given that my husband and I wanted at least 2 kids, Dr. Fisher also recommended having at least 2 healthy embryos per potentially viable pregnancy so we decided to pursue a second round of IVF egg retrieval before moving forward with a transfer. In my second IVF cycle: I started off with Aygestin, had around 11 antral follicles, started same dosage of ​225IU of Gonal F and then 150IU for Menopur which was adjusted slightly after every ultrasound and blood test appointment. Then started Cetrotide and triggered again with Lupron and Novarel. This time, 11 eggs were retrieved, 8 mature. I ended up with 4 that survived until Day 5 and then 3 that turned out to be Euploids. For my frozen embryo transfer, I started off with Aygestin pills (this was mostly because we were thinking of potentially doing a 3rd cycle of IVF depending on results of the embryo genetic testing). However, once we confirmed we were moving forward with FET, I was put on Letrozole 5mg for 5 days. After a few monitoring visits, the doctor asked me to start Gonal F at 150IU. As monitoring progressed, I was then instructed to take Ovidrel at 250mcg and take Medrol 16mg pills for 5 days. I went back in for a progesterone level check and then was confirmed for transfer. Transfer was successful and I am now 8 months pregnant!

Although most people including myself go in thinking we will perhaps only need one IVF cycle, it's always good to know your options and what pricing will look like if you needed subsequent cycles. Accounting for price of medication is also something that needs to be factored in. Alto Pharmacy was superb in terms of convenience of delivering the medication to my doorstep. Our cost was ~$50K for 2 IVF cycles and 1 FET.

During my embryo transfer, at one of my ultrasound visits, I was seen by another doctor. The reaction of the doctor made it seem like there was a strong possibility the transfer would need to be cancelled as I might have ovulated already. I panicked and started crying frantically. That said, Dr. Fisher called me shortly after my appointment to reassure me that he was not concerned and that I would be monitored closely for the next couple of days to confirm next steps. In the end, I was able to successfully complete the FET so although alarming at first, it all worked out.

We went with the mid-luteal protocol. For medications, was as on the typical injections, FSH, Menopur, Cetrotide and I can't remember what I used for trigger. For the transfer, we went with natural/letrozole protocol as I was able to ovulate on my own and my progesterone levels were normal.

For my egg retrieval: I was on Menopur, Follistim, Ganirelix, leuprolide and triggered with pregnyl (cannot remember the doses as these were adjusted) For my transfer: medrol and ovidrel. My FET was successful and I am now pregnant at 42-years-old.

The care team at Spring Danville was wonderful to work with. Very professional, organized, knowledgeable and friendly. For some of my procedures I went to their Oakland office, where I also got excellent care. Everyone including receptionists, nurses, medical assistants, and every person I ever interacted with at Spring offered me the best care.

Spring has a very organized system: billing is straightforward, their patient portal where we get all communications and updates about our treatment is quite helpful and user friendly, the facilities are very clean and modern and since they have different locations you could use for your monitoring, you never have to be commuting too far to get your care. Parking in the Danville office was always good, but in the Oakland office it was a bit difficult so it was good to show up with enough time to be able to find a spot and still make it on time for my appointment.

I enjoyed going to Spring. The doctors, nurses and front desk staff were always kind. Dr. Fisher was very kind, professional and knowledgeable. I chose to transfer to Dr. Fisher when my initial Dr left the clinic. Dr. Fisher came highly recommended. We built a nice rapport due to the personal nature of our visits, not to mention the frequency. He always patiently answered my questions. He even shared he and his wife’s own journey. We started on a new, slightly tweaked regimen for my next retrieval to see if we could get better results. We used the HGH, but we did not see better results. I’d had my fibroids removed a few years prior, but they’d starting growing back likely die to the hormone treatments. So, I went and had a hysteroscopy. The, did my final retrieval. For this round, I wanted to do a transfer as I’d only done transfers, then froze and tested my embryos. Dr. Fisher really took time to discuss my treatment options and he listened to what I wanted to do. He shared his clinics stats with me as well so that I could make a more informed decision. I decided to do a retrieval and fresh Day 5 transfer. We did and thankfully it worked!

The office ran smoothly. The nurses were personable and well trained. They greeted you as soon as you walked in. The nurses that drew my blood were always talkative and would ask about my day or plans which I greatly appreciated. We’d frequently exchange movie ideas. They are a big part of the reason I’d highly recommend this office!

Describe the protocols Thomas Fisher used in your cycles at Spring Fertility East Bay and their degree of success.
I did a luteal phase agonist protocol for egg retrieval - my AFC was 8, we ended up retrieving 11 eggs, 10 fertilized and 6 made it to blast. Dr. Fisher really paid close attention to how I was responding and changed the doses to get me to our goal.

I never met Nurse Mel in person, but during our phone interactions she exuded care, competence, and patience - she is truly an exceptional provider. I really enjoyed all of conversations and she was always willing to answer my questions.
Describe your experience with Spring Fertility East Bay.
My overall experience was truly wonderful. With the exception of a staff person who is no longer at the company, every person I interacted with was kind, professional, and courteous.  Dr. Fisher is truly a gem- from our first interaction to our transfer, he treated us with respect, consideration, and warmth.  I really appreciated the time he took to answer questions and his excitement at our promising results.   And although I never met Nurse Mel in person, during our phone interactions she exuded care, competence, and patience - she is truly an exceptional provider.     2. The guidance throughout the experience was, for the most part, excellent.  I really appreciated having access to the portal, and the portal is well-organized and information is easy to access.  While sometimes it felt like I was getting the same information explained to me multiple times, I appreciated the reassurance that I was really clear about what to expect.    Things that could be better: 1. The financial documents seemed excessively confusing.  The fact that the same information would appear in multiple places (see attached document) made it hard to know what we were actually responsible for. On the attached cost letter, there are things on it (like the medication) with dollar amounts listed that we didn't actually have to pay for, and other things, like the copay, which were much more than what was listed.  I would strongly recommend making the financial documents clearer and easier to understand.   Relatedly, the costs we paid for semen cryopreservation weren't included on this letter, but should have been.  If you're going to present a cost letter it should include all the charges.  2. The requirement that a known donor submit both a negative covid test and a negative antibody test conducted by Spring is both not medically indicated and an onerous requirement that places a further burden on people who are using donor gametes.  In my (totally unscientific) survey of other clinics, there aren't any that are requiring this.  3.  There were several instances in which my gender-non confirming spouse couldn't choose a box that reflected their reality, and I would encourage you to review your forms with an eye towards inclusivity of queer clients and gender non-conforming clients.(These forms were updated and resent to us).   4. The requirement for psychological counseling is both discriminatory towards people who are using donor gametes and not actually required by any accrediting body as far as i can tell. In the context of the family building process, there are certainly some people who might benefit from counseling, but I would strongly urge you to consider changing the counseling requirement to a recommendation.  Supporting the mental health of your clients who are undergoing fertility treatments is commendable, creating burdensome hoops that people who need donor gametes must jump through is not.

We have insurance coverage that covered the vast majority of our costs. Spring is in-network for our insurance. We paid somewhere around $8,000 total for everything associated with IVF at Spring.

Given my scenario, Dr. Fisher recommend we try IVF first instead of attempting IUI’s or other interventions. We did a freeze all cycle as my age and reaction to the medications put me at higher risk for OHSS. He let me make the decision for a fresh or frozen transfer just a few days before the retrieval to give me the most control over my options.

Every interaction with Dr. Fisher it was clear that he not only cared about you as his patient, but that you were an individual and he was thinking of you individually and not as one of many. This was apparent from our countless conversations leading up to the egg retrieval but in also how he thought of my care, it was individualized and not a set protocol used on each women. His bedside manner is A+++.
Mel [nurse at Spring Fertility] was always easy to get ahold of and answered all of my questions. She is also super kind and supportive...Mid-luteal protocol. Apparently Spring has had a lot of success with this protocol, I had not heard of it prior to my treatment at this clinic, and I have to say, I am very happy with it...8 embryos made it to day 5 and of those, five were euploid...For the retrieval, it was approximately $18,000. For transfer, approximately $5-$6,000.

Dr. Fisher was the second reproductive endocrinologist I treated with over the course of about a year. I came to him very disheartened by my prior experience - both in the results and the relationship with my prior clinic and provider. Dr. Fisher was very different in every aspect from my prior experience. From the initial visit, he went through different protocols we could try during this egg retrieval to see if we could get better results, he took significant time explaining his thinking, he spent an enormous amount of time explaining medicine to me and assuring me when I was nervous. Dr. Fisher truly cares about his patients, I have not encountered this kind of care routinely and I will always be appreciative.

Don't be afraid to ask questions, Dr. Fisher will take the time to explain until you understand.

Putting aside specifically the treatment many women receive during fertility treatment where they are often treated as a number, Dr. Fisher is just a genuine person. Every interaction with Dr. Fisher it was clear that he not only cared about you as his patient, but that you were an individual and he was thinking of you individually and not as one of many. This was apparent from our countless conversations leading up to the egg retrieval but in also how he thought of my care, it was individualized and not a set protocol used on each women. His bedside manner is A+++.

Mid-luteal protocol. Apparently Spring has had a lot of success with this protocol, I had not heard of it prior to my treatment at this clinic, and I have to say, I am very happy with it. At the prior clinic I treated with, I underwent the antagonist protocol and the same amount of eggs were retrieved as at Spring (in the 20s). But the prior clinic, I only ended up with one embryo after PGS testing (3 were tested but only one was euploid. At Spring, 8 embryos made it to day 5 and of those, five were euploid.

Mel was always easy to get ahold of and answered all of my questions. She is also super kind and supportive.

Over the course of my treatment, I met almost every provider. It was nice to be able to know each doctor personally. However, I could always choose to see Dr. Fisher for all procedures (except the egg retrieval which is more time-sensitive) but felt comfortable with the skills of all the doctors at the clinic.

All of my costs were unfortunately out-of-pocket as Spring does not accept United Healthcare. For the retrieval, it was approximately $18,000. For transfer, approximately $5-$6,000.

I had diminished ovarian reserve, that caused me a lot of anxiety and confusion. Dr. Fisher made himself available to answer questions and ease my mind. His ability to cite studies offhead, combined with his humor and kindness really helped build trust and confidence. I am a black woman, and it’s important to note that I didn’t worry about whether Dr. Fisher treated me differently than any other patients. I really felt that he understood and empathized with me and that he was committed to optimizing our results. With his help, I froze 19 eggs. Given my ovarian reserve, this is more than twice what we initially thought I would be able to freeze. This was a huge victory. When doing ultrasounds, he took his time to be sure no eggs were missed. When you don’t have a lot to work with, it’s important. I also note that he was kind and had good rapport with nurses and support staff. You get the sense that he tries to be a good person in all areas of his life.

[1ST CYCLE: 24 retrieved, 21 mature] Started the shots with the period, progesterone priming for 10 days before the period. Started with 150IU Gonal f/ 150IU Menopur. It went down to 75 IU each eventually & I added Cetrotide on day 5. Lupron only trigger to prevent OHSS. Triggered on day 11, and vaginal Cabergoline insertion for 8 days since the trigger day. [2ND CYCLE: 42 retrieved, 33 mature] Mid-luteal protocol to see if we can get the eggs grow a little tighter. Started the shots 1 month after the retrieval date. Started priming 5 days after positive ovulation test. Started with 150IU Gonal f/ 150IU Menopur. It went down to 100IU Gonal f/ 75 IU Menopur eventually & I added Cetrotide on day 6. Lupron only trigger to prevent OHSS. Triggered on day 11, and vaginal Cabergoline insertion for 8 days since the trigger day. Was back on Cetrotide again for 3 days after ER to minimize OHSS risk.

Absurdly excellent bedside manner. He went out of his way to personally perform the second transfer and it meant the world to me (travelled into the clinic in the city on an afternoon he would normally have at a different clinic). He was always direct, explained things well, asked sensitive questions, etc. I have been incredibly happy with his care—especially after some experiences of my friends with other providers at Spring not being this excellent. I would recommend Dr. Fisher without reservation.

Don’t be afraid to ask questions. Even after appointments, I would think of question and email him or his staff and they would get back to me really quickly.

[Dr. Thomas Fisher] has been with us on a very long journey to finally arrive at our first IVF cycle. I had three surgeries between the time we met with him for our consultation until we started our IVF cycle. He checked in on us often and we have formed a great relationship with him. We feel like he is truly invested in helping us reach our goal...He has always been very open, honest and up front with us about our fertility and the hurdles we have with my diagnosis of endometriosis and adenomyosis. Though we have had many difficult and tear-filled conversations, we know that he has our best interest in mind, he understands that we are willing to do any and everything we need to do in order to make it to the phase when we do have an embryo to freeze, and he is supporting us along the way. He has been willing to have phone consultations with us since we live six hours from the clinic, which has been extremely helpful.
I wish that the nursing staff [at ORM] would communicate with the doctors better to help the patients get the correct answer tailored to our specific situation. i know they probably receive lots of questions or concerns during a day, but I don't think its too much to ask to get the help or advice we need, especially if we are in pain. I also wish they would just call us back with an answer rather than sending everything through their patient portal. A phone call gets the job done faster and more effectively.

Dr. Fisher has been very supportive through all of the ups and downs in my health and getting us through our first IVF cycle. Even though our first egg retrieval didnt result in any successful embryos, Dr. Fisher has been in contact with us personally to discuss options for the future, to lend his condolences to us, and to check in our physical, emotional and mental well being after a failed cycle. He has always been very open, honest and up front with us about our fertility and the hurdles we have with my diagnosis of endometriosis and adenomyosis. We had an AM regimen of Lupron and Menopur, and a PM regimen of Lupron and Follistim, followed by a baby aspirin and a pre-natal vitamin. Because i have a low AMH and endometriosis and have had many pelvic surgeries, we knew that my total eggs retrieved would be low. We were pleasantly surprised when we retrieved 7 eggs, 5 mature and 3 fertilized however none made it to the day six blastocyst stage unexpectedly. If we move forward with a second round, we are taking 3 mg of melatonin along with 600 mg of CoQ10 to see if this helps with my egg quality.

Heidi was extremely helpful to us and all of our questions we had both in person and over the phone during this entire process. There were a few documents that the nursing staff had us fill out twice because they lost the first copy we had signed, so that was frustrating in the sense that where did our paperwork end up if they didnt have it? Some of the nursing staff that we talked to on the phone tried to put us off with some of our questions, or they tried to give their own opinion on care questions, such as, can I use a heating pad during STIMS? The nursing staff I spoke to gave me the blanket answer to this question, but when i saw Dr. Fisher the next day, he told me his answer to the question and said that we have to tailor procedures to each of our patients and their individual needs and side effects. I wish that the nursing staff would communicate with the doctors better to help the patients get the correct answer tailored to our specific situation. i know they probably receive lots of questions or concerns during a day, but I don't think its too much to ask to get the help or advice we need, especially if we are in pain. I also wish they would just call us back with an answer rather than sending everything through their patient portal. A phone call gets the job done faster and more effectively.

Strengths: clean, good locations, always offered bottled water to us, private settings, great doctors, somewhat helpful nursing staff when we finally got the go ahead to start IVF. Weaknesses: fairly poor communication methods - if I called in with a question and asked for a return phone call, I didn't like getting a message in the Patient Portal, sometimes I had follow up questions or just wanted to reaffirm what was told to me previously. Overall it was a good experience, even though our cycle failed.
Describe your experience with your monitoring appointments at ORM Fertility Oregon.
We went in at our scheduled appointment time and the office ran like a well oiled machine: blood draw, ultrasound, schedule the following appointment, they would call later in the day to let us know our blood test results and if we needed to change any of our dosing instructions. it went very well.

STIM medications were roughly $5,000; IVF cycle part 1 for the ultrasounds, bloodwork, egg retrieval, embryology and then storing of any embryos was almost $16,000; anesthesia for the egg retrieval was roughly $600. If we would have had any embryos, there would have been additional fees for testing of the embryos, testing of my uterus before transfer, and then everything involved with transfer as well and other medications to prepare for transfer as well.

The clinic [ORM] is well run. Hours open are from 8 am to 3 pm and closed from noon to 1 pm. Weekend hours are from 7:30 to 9:30 at the downtown office. Ultrasounds are only scheduled for Monday through Fridays.... The clinic only offers eSET and most patients in this clinic pursue PGS testing which adds another $4800.... Each RE has their own nurse. It is easy to communicate questions through their online portal by sending messages which are promptly answered.

Dr. Fisher is a great reproductive endocrinologist, knowledgeable and friendly. Communicating with his staff on next steps was a little vague and I felt like in person visits were the best way to get information from the rude coordinator and financial staff.
What's one piece of advice would you give a prospective patient of Thomas Fisher at ORM Fertility Oregon?
Make sure you stay the second half of the initial consult visit to meet with the financial coordinator which takes a half hour. We could only stay the first hour and felt like we were pretty lost not having finished the appointment knowing what next steps are.

I love being on the west side clinic [of Oregon Reproductive Med] because it is way smaller then the downtown clinic. It is quieter. And by sheer volume, it more personal. You do need to go downtown for injection class, IVF retrieval, and on weekends if you need an ultrasound, but we love not having to go downtown every time.... Our IVF with PGS testing was roughly $20,000. That did not include the medications which were about $4,500

Just very poor bedside manner, communication, and overall care. I transferred to a dofferent clinic and after one appt they realized ORM had me take a specific test that proved to be very important. Basically they put me through 5 IUIs when I should have had a surgery first. But they never tested my uterine lining so they didn’t know I had multiple polyps on my inner lining. Making it almost impossible to get pregnant. Had they done that test I wouldn’t have had to suffer through 5 IUIs. Luckily my new clinic tested for it before doing anything else.

Heidi simplified the IVF process so much for me. She highlighted my calendar with important things and coordinated all of my medications with the pharmacy directly.

Don't hesitate to ask questions, and don't be afraid to express your thoughts/concerns. You will feel better having all of your questions answered!
During treatment, were you treated like a number or a human with Thomas Fisher at ORM Fertility Oregon?
Dr. Fisher was very compassionate with his care, he shared his personal experiences with infertility and helped my husband and I feel comfortable and confident with treatments. We didn't feel like we got the "standard" treatment, but that our protocol was tailored to our needs and desires as patients. For example, it was initially recommended that we have a freeze-all cycle, but it was very important to me that we were able to do a fresh transfer. Dr. Fisher made sure to monitor me closely throughout the stimulation process so that we could safely move forward with a fresh transfer. I appreciated the fact that I was being heard, and that we were able to make decisions about our care. Dr. Fisher and his team answered all of our questions every step of the way, and we are so thankful to have worked with him.

We started treatment with medicated cycles and IUI, after 3 unsuccessful attempts we asked to move on to IVF. We moved forward with IVF + ICSI and a fresh embryo transfer. The first transfer, unfortunately, resulted in miscarriage. We are currently pregnant with our miracle baby from our second transfer.

The costs differed depending on the treatment. The clinic bills for the IVF cycle which covers ultrasounds, egg retrieval and embryology. There is an additional cost for anesthesia during the egg retrieval, and medications are purchased directly from the pharmacy.

My experience was that Dr. Fisher tends to be conservative and in our situation pushed toward single embryo transfer. Safety of both mother and baby are extremely important to him, and I respect him for urging us to only transfer one embryo. That being said, had we chosen to transfer 2 embryos I know that he would have supported us in that decision.

He talked to me and my husband about our experience so far, what our hopes and goals were as well as what our fears and uncertainty are. He also shared his personal experience with infertility.

So far, we have done 4 rounds of IUI with Clomid. I developed cysts on the first two rounds and we switched medicine (I can’t remeber the name of he second medicine) and I didn not develop cysts again. When none of those cycles were effective he told us that the chances of another one working are very low. Because of our age and health he suggested taking a few months to try naturally again before considering IVF which we agreed with.

Each nurse interacted with was kind, patient and a good communicator. They did not make me feel rushed or insecure in any way and made a very uncomfortable situation feel normal.

So far, I have not found any weaknesses with the clinic. They always get me in right away when I need an ultrasound (even on the weekends) and are always respectful and professional. For our second round of retrieval, Dr Fisher hastened the taper of some of the medications, as the first round saw an unexpected jump in follicle size from one ultrasound to the next...[he] is very positive, he maintains a sunny disposition, even while cautioning about potential negative outcomes. If anything, sometimes we wished he was upfront about our results, if they were negative in someway.
[the Spring Fertility] Clinic is excellent with doctor patient communication. Most sessions are with your assigned physicians and the doctors are very easy to reach by phone. The nursing staff varied in level of experience, but for the most part were very good. Less experienced nurses seemed to have less pivotal/demanding roles...Medication fees, procedure fees- each ivf session from start ot extraction cost around 18k.

He [Dr. Thomas Fisher] was only my doctor for part of the process as he left the practice but he was always very informative about what was happening and what I needed to do to prepare myself. He never made me feel like a number and always spoke to my husband and I openly about the process.... He was very kind, never rushed through the appointment and always made sure I knew what to expext

When Dr. Fisher left it felt like some of the morale went down or that people were all over the place. However it was temporary and seemed to go back to normal shortly after.,... The staff there is out of this world! They are all so sweet and very organized. They also never rushed And genuinely happy to see their patients!

I was on clomid for three cycles but had to switch to letrozol because of the side affects. Which he was okay with.

Testing and some medications were covered under insurance. We paid for injections but don’t remember the cost. Each round of iui was $200
